Özet

Amaç: Çalışmamızda, antenatal hidronefroz (ANH) tanısıyla takip edilen vakalarda retrospektif olarak etiyolojik sebeplerin, demografik verilerin, algoritmaya uygun takip edilen hastaların prognozunun saptanması, renal hasar ve diğer komplikasyonlar ile ilgili risk faktörlerini belirlenmesi amaçlanmıştır. Gereç ve Yöntem: Bu çalışma, retrospektif olarak planlanmış ve Diyarbakır SUAM Gazi Yaşargil Eğitim ve Araştırma Hastanesi Çocuk Sağlığı ve Hastalıkları Kliniği, Çocuk Nefroloji Polikliniği' ne 2022 Ocak- 2023 Ocak tarihleri arasında başvuran antenatal hidronefroz tanılı hastalar ile gerçekleştirilmiştir. Hastaların demografik ve laboratuvar verileri, antenatal ve postnatal dönemde ultrasonografi (USG) bulguları, postnatal dönem DMSA ve MAG-3 görüntülemeleri, cerrahi operasyon öyküleri kaydedilmiştir. Bu veriler ile hastaların prognozu, renal hasar ve diğer komplikasyonlar ile ilgili risk faktörleri değerlendirilmiştir. Bulgular: Toplam 144 hasta çalışmaya dahil edildi. Hidronefroz vakaları çoğunlukla erkek bebeklerde (%78,5) görülmüş ve sol böbrekte (%52,1) daha yaygın izlenmiştir. Antenatal hidronefroz tanısı ile izlenen hastaların büyük çoğunluğunda (%69,4) postnatal dönemde geçici hidronefroz saptanmıştır. Antenatal 2. trimesterda USG' de; sol anterior- posterior çap (APÇ) ölçümü (p=0.001), antenatal 3.trimesterda USG' de; sol APÇ (p=0.003), parankim incelmesi (p=0.012), postnatal dönemde 1.USG' de; sol APÇ (p<0.001), postnatal dönemde 2. USG' de; sol APÇ (p<0.001), parankim incelmesi (p=0.02), üreter dilatasyonu (p=0.017), parankimde ekojenite artışı (p=0.04), postnatal dönemde 3. USG' de sağ APÇ (p=0.04), sol APÇ (p=0.006), kalikslerde dilatasyon (p<0.01), üreter dilatasyonu (p<0.01), parankimde ekojenite artışı (p=0.029) gibi radyolojik bulgular renal hasar ile ilişkili bulunmuştur. Sonuç: Antenatal dönemde saptanan hafif dereceli hidronefroz vakaları dahi doğum sonrası mutlaka takip edilmeli, postnatal dönemde USG kontrolü yapılmalıdır. Antenatal dönemde sadece pelvik çap değil; parankim kalınlığı, parankimde ekojenite artışı, kaliks ve üreter dilatasyonu gibi yapısal özellikler de sistematik şekilde değerlendirilmeli ve izlenmelidir. Özellikle APÇ >15 mm, bilateral tutulum, parankim incelmesi ve ekojenite artışı saptanan vakalar yüksek riskli olarak kabul edilip erken 2 dönemde Çocuk Nefroloji ve/veya Çocuk Üroloji uzmanlarına yönlendirilmelidir. Ailelere antenatal hidronefrozun seyri hakkında bilgi verilerek, düzenli takiplerin önemi vurgulanmalı ve uyum artırılmalıdır.

Objective: The aim of our study was to retrospectively evaluate the etiological causes and demographic characteristics of cases diagnosed with antenatal hydronephrosis (ANH), to assess the prognosis of patients managed in accordance with the follow-up algorithm, and to identify risk factors for renal damage and other complications. Materials and Methods: This retrospective study included patients diagnosed with antenatal hydronephrosis (ANH) who presented to the Pediatric Nephrology Outpatient Clinic of Diyarbakır SUAM Gazi Yaşargil Training and Research Hospital between January 2022 and January 2023. Data collected for each patient included demographic characteristics, laboratory results, antenatal and postnatal ultrasonography (USG) findings, postnatal dimercaptosuccinic acid (DMSA) scintigraphy, mercaptoacetyltriglycine (MAG-3) renography results, and history of surgical interventions. These data were analyzed to assess the prognosis of the patients and to identify risk factors for renal damage and other complications. Results: A total of 144 patients were included in the study. Hydronephrosis was observed predominantly in male infants (78.5%) and was more commonly detected in the left kidney (52.1%). The majority of cases (69.4%) diagnosed with antenatal hydronephrosis were found to have transient hydronephrosis in the postnatal period. In the second trimester antenatal ultrasonography (USG), left anteroposterior diameter (APD) measurement (p=0.001), and in the third trimester USG, left APD (p=0.003) and parenchymal thinning (p=0.012) were significantly associated with renal damage. In the first postnatal USG, left APD (p<0.001); in the second postnatal USG, left APD (p<0.001), parenchymal thinning (p=0.02), ureteral dilatation (p=0.017), and increased parenchymal echogenicity (p=0.04); and in the third postnatal USG, right APD (p=0.04), left APD (p=0.006), calyceal dilatation (p<0.01), ureteral dilatation (p<0.01), and increased parenchymal echogenicity (p=0.029) were found to be significantly associated with renal damage. Conclusion: Even mild cases of antenatal hydronephrosis detected during the antenatal period should be followed up after birth, and postnatal ultrasonography (USG) should be performed. During the antenatal period, not only the pelvic diameter but also structural features such as parenchymal thickness, increased parenchymal 4 echogenicity, calyceal dilatation, and ureteral dilatation should be systematically evaluated and monitored. Cases with an anteroposterior diameter (APD) >15 mm, bilateral involvement, parenchymal thinning, or increased echogenicity should be considered high risk and referred early to a pediatric nephrologist and/or pediatric urologist. Families should be informed about the clinical course of antenatal hydronephrosis, and the importance of regular follow-up should be emphasized to improve compliance.
