Özet

Postoperatif ağrı, modern cerrahi pratiğinde önemli bir klinik sorundur. Etkin ağrı kontrolü, yalnızca hasta konforunu artırmakla kalmaz, aynı zamanda iyileşmeyi hızlandırır, komplikasyon oranlarını azaltır ve erken mobilizasyonu destekler. Karın germe ameliyatı (abdominoplasti), estetik ve fonksiyonel amaçlarla yapılan, geniş doku disseksiyonu ve kas plikasyonu içeren majör cerrahi bir girişimdir. Bu operasyonlarda postoperatif dönemde orta-şiddetli ağrı sık görülmekte ve analjezi ihtiyacı artmaktadır. Geleneksel olarak ağrı yönetiminde opioidler kullanılsa da, solunum depresyonu, bulantı, kusma ve bağımlılık gibi yan etkiler nedeniyle güvenli ve etkili alternatif yöntemlere ihtiyaç vardır. Son yıllarda Transversus Abdominis Plane (TAP) blok, abdominal cerrahiler sonrası ağrı kontrolünde öne çıkan bir bölgesel anestezi yöntemidir. TAP blok, ultrason eşliğinde internal oblik ve transversus abdominis kasları arasına lokal anestezik verilerek T7-L1 segmentlerinden çıkan torakoabdominal sinirlerin duyusal iletiminin bloke edilmesini sağlar. Literatürde, TAP bloğun postoperatif ağrı skorlarını düşürdüğü, opioid tüketimini azalttığı ve hasta memnuniyetini artırdığı gösterilmiştir. Ancak çoğu çalışmada blok, cerrahi işlem tamamlandıktan sonra uygulanmıştır. Abdominoplasti gibi kas plikasyonu içeren operasyonlarda cerrahi sonrası doku bütünlüğünün bozulması, ultrason görüntüsünü ve lokal anestezik yayılımını olumsuz etkileyebilir. Bu durum, postoperatif TAP blok etkinliğini sınırlayabilir. iii Amaç:Bu çalışmada, abdominoplasti operasyonlarında TAP blok uygulamasının cerrahi öncesi ve cerrahi sonrası yapılmasının postoperatif analjezi etkinliği üzerindeki etkileri karşılaştırılmıştır. Bu yönüyle çalışma, TAP bloğun preoperatif ve postoperatif uygulamalarını doğrudan karşılaştıran ilk araştırmadır. Gereç ve Yöntem:Plastik Rekonstrüktif ve Estetik Cerrahi kliniğinde abdominoplasti geçiren 32 kadın hasta çalışmaya dahil edilmiştir. Hastalar rastgele iki gruba ayrılmıştır:  Grup 1: Cerrahi öncesi TAP blok uygulananlar  Grup 2: Cerrahi sonrası TAP blok uygulananlar Postoperatif 1., 2., 6., 12. ve 24. saatlerde NRS ağrı skorları kaydedilmiştir. NRS >5 olduğunda 50 mg IV deksketoprofen uygulanmıştır. İlk 24 saatte toplamtramadol tüketimi, kurtarıcı analjezik ihtiyacı ve yan etkiler kaydedilmiştir. Bulgular:Demografik veriler ve ASA sınıfları açısından gruplar arasında anlamlı fark bulunmamıştır (p>0,05).  Toplam tramadol tüketimi: o Cerrahi öncesi grup: 278 mg o Cerrahi sonrası grup: 144 mg (p<0,001)  Ağrı skorları: Gruplar arasında anlamlı fark saptanmamıştır.  Blok uygulama süresi: Cerrahi sonrası uygulanan blokların süresi anlamlı derecede daha kısa bulunmuştur (p=0,013).  Ciddi komplikasyon gelişmemiş, yan etki profilleri benzer bulunmuştur. Sonuç:Cerrahi sonrası uygulanan TAP blok, opioid ihtiyacını anlamlı derecede azaltarak güvenli ve etkili analjezi sağlamaktadır. Bu nedenle abdominoplasti operasyonlarında postoperatif TAP blok uygulaması tercih edilebilir.

Postoperative pain is one of the most important clinical issues in modern surgical practice. Effective pain management not only increases patient comfort but also accelerates recovery, reduces complication rates, and facilitates early mobilization. Abdominoplasty, a major surgical procedure involving extensive tissue dissection and muscle plication, is commonly performed for aesthetic and functional purposes. In these operations, moderate-to-severe pain is frequently observed in the postoperative period, leading to an increased need for effective analgesia. Traditionally, opioids are used for postoperative pain management; however, adverse effects such as respiratory depression, nausea, vomiting, dependence, and delayed mobilization highlight the need for safer and more effective alternatives. In recent years, the Transversus Abdominis Plane (TAP) block has gained prominence as a regional anesthesia technique for postoperative pain control following abdominal surgeries. The TAP block involves the ultrasound-guided injection of a local anesthetic between the internal oblique and transversus abdominis muscles, blocking the sensory transmission of thoracoabdominal nerves arising from the T7-L1 segments. Previous studies have demonstrated that TAP block significantly reduces postoperative pain scores, decreases opioid consumption, and improves patient satisfaction. However, in most studies, the block was performed after the surgical procedure. In operations such as abdominoplasty, where muscle plication is performed, disruption of tissue integrity may impair ultrasound image quality and limit the spread of the local anesthetic, potentially reducing the efficacy of a postoperative TAP block. v Objective:This study aimed to compare the effectiveness of preoperative versus postoperative TAP block applications in patients undergoing abdominoplasty, in terms of postoperative analgesia and opioid consumption. To our knowledge, this is the first study directly comparing these two timing approaches. Materials and Methods:A total of 32 female patients undergoing abdominoplasty were included and randomly divided into two groups:  Group 1: Preoperative TAP block  Group 2: Postoperative TAP block Postoperative pain was assessed using the Numeric Rating Scale (NRS) at the 1st, 2nd, 6th, 12th, and 24th hours. Patients with NRS >5 received 50 mg IV dexketoprofen as rescue analgesia. Total tramadol consumption, the number of rescue analgesic requirements, and side effects were recorded during the first 24 hours postoperatively. Results:Demographic data and ASA classifications were similar between groups (p>0.05).  Total tramadol consumption: o Preoperative group: 278 mg o Postoperative group: 144 mg (p<0.001)  Pain scores: No significant differences were observed between groups at any time point.  Block performance time: Significantly shorter in the postoperative group (p=0.013).  No major complications occurred, and the side effect profiles were comparable. Conclusion:Postoperative TAP block significantly reduces opioid requirements and provides safe and effective analgesia after abdominoplasty. Therefore, TAP block performed after surgery may be considered a preferable approach for postoperative pain management in abdominoplasty procedures.
