Özet
Amaç: Bu çalışmadaki amacımız Diyabetik ketoasidoz ve ketoz ile başvuran hastalarda tedavi öncesi ve sonrası hormon düzeylerinde meydana gelen değişimlerin araştırılmasıdır. Çalışmada bu akut durumların tedavisi ile kontraregülatuvar hormonlar üzerinde nasıl bir değişim olduğu sorusuna cevap aranmıştır. Tedavi öncesi ve sonrası glukagon, kortizol, büyüme hormonu ve katekolaminler gibi hormonların düzeylerindeki değişikliklerin incelenmesi, Diyabetik ketoasidoz ve ketoz yönetiminde hormonal dengenin önemini vurgulamakta ve tedavi başarısının izlenmesinde yeni biyobelirteçlerin kullanılabilirliğini gündeme getirmektedir. Özellikle insülin tedavisinin hormonlar üzerindeki etkisi, bu sürecin yönetiminde kritik ipuçları sağlayabilir. Ayrıca bu çalışmada hormon değişimlerinin Tip 1 ile Tip 2 Diabetes Mellitus'lu hastalarda farklı olup olmadığının incelenmesi amaçlanmıştır. Gereç ve Yöntem: Bu çalışmada 2018-2024 yılları arasında Ege Üniversitesi Tıp Fakültesi Endokrinoloji Kliniği'nde yatarak takip edilen Diyabetik Ketoasidoz/Ketoz hastaları retrospektif olarak incelenmiştir. Dahil edilme kriterlerini sağlayan hastaların demografik verileri, diyabet tipi, laboratuvar bulguları ve hormon parametreleri hastane veri kayıt sisteminden elde edilmiştir. Tedavi öncesi ve sonrası biyokimyasal değişimler analiz edilerek istatistiksel değerlendirmeler yapılmıştır. Hastalar cinsiyet, keton seviyesi ve diyabet tipi gibi değişkenlere göre alt gruplara ayrılmış, tedavinin hormon seviyeleri üzerindeki etkisi araştırılmıştır. Veriler IBM SPSS Statistics Standard Concurrent User V 27 ve JASP 0.19.1.0 istatistik paket programlarıyla analiz edilmiştir. Çalışmanın etik onayı, Ege Üniversitesi Tıbbi Araştırmalar Kurulu'ndan 16.04.2024 tarihli 2024-2732 numaralı karar ile alınmıştır. Bulgular, Diyabetik Ketoasidoz/Ketoz tedavisinin hormon düzeyleri üzerindeki etkisini anlamaya katkı sağlamayı amaçlamaktadır. Bulgular: Çalışmaya 41 hasta dahil edilmiş olup hastaların 23'ü kadın ve 18'si erkek idi. Hastalardan 16'sı Tip 1 Diabetes Mellitus , 25'i Tip 2 Diabetes Mellitus tanılı idi. Tedavi öncesi ve sonrası hormon düzeylerinin değişimine bakıldığında TSH, FT3, IGF-1, ACTH, FSH, Prolaktin, Total Testosteron, Serbest Testosteron ve SHBG'de tedavi öncesi ve sonrası arasında istatistiksel olarak anlamlı farklılık vardı (p<0,05). Tip 1 ve Tip 2 Diabetes Mellitus grupları arasında tedavi öncesi ve sonrası hormon değişimi incelendiğinde ise TSH, FT3, IGF-1 ve Total Testosteron ortak olarak anlamlı değişim gösteren parametreler olarak bulundu (p<0,05). Sonuç: Diyabetik Ketoasidoz ve Diyabetik Ketoz'da birçok hormon düzeyi akut tablo sırasında ve tedavi sonrasında artış veya azalış şeklinde değişiklik göstermektedir. Bu durumlar hastalarda gereksiz tedavi başlanmasına neden olabilmektedir. Akut tablo düzeldikten sonra hormon düzeyleri tekrarlanmalı ve hormon bozukluğu mevcut ise gerekli tedaviler başlanmalıdır.

Aims: Our aim in this study is to investigate the changes in hormone levels before and after treatment in patients presenting with diabetic ketoacidosis and ketosis. The study seeks to answer how the treatment of these acute conditions affects counterregulatory hormones. Examining the changes in the levels of hormones such as glucagon, cortisol, growth hormone, and catecholamines before and after treatment highlights the importance of hormonal balance in the management of diabetic ketoacidosis and ketosis and raises the potential use of new biomarkers for monitoring treatment success. In particular, the effect of insulin therapy on hormone levels may provide critical insights into the management of this process. Additionally, this study aims to determine whether hormonal changes differ between patients with Type 1 and Type 2 Diabetes Mellitus. Materials and Methods: In this study, patients with Diabetic Ketosis/Ketoacidosis who were hospitalized and monitored at the Endocrinology Clinic of Ege University Faculty of Medicine between 2018 and 2024 were retrospectively analyzed. The demographic data, diabetes type, laboratory findings, and hormone parameters of patients who met the inclusion criteria were obtained from the hospital records system. Biochemical changes before and after treatment were analyzed, and statistical evaluations were conducted. Patients were categorized into subgroups based on variables such as gender, ketone levels, and diabetes type, and the effects of treatment on hormone levels were investigated. Data analysis was performed using IBM SPSS Statistics Standard Concurrent User V 27 and JASP 0.19.1.0 statistical software packages. Ethical approval for the study was obtained from the Ege University Medical Research Ethics Committee with decision number 2024-2732, dated April 16, 2024. The findings aim to contribute to understanding the impact of Diabetic Ketoacidosis/Ketosis treatment on hormone levels. Results: A total of 41 patients were included in the study, of whom 23 were female and 18 were male. Among the patients, 16 were diagnosed with Type 1 Diabetes Mellitus, while 25 had Type 2 Diabetes Mellitus. When examining the changes in hormone levels before and after treatment, statistically significant differences were observed in TSH, FT3, IGF-1, ACTH, FSH, Prolactin, Total Testosterone, Free Testosterone, and SHBG levels (p<0.05). When the changes in hormone levels before and after treatment were compared between the Type 1 and Type 2 Diabetes Mellitus groups, TSH, FT3, IGF-1, and Total Testosterone were found to be the common parameters showing significant changes (p<0.05). Conclusion: In Diabetic Ketoacidosis and Diabetic Ketosis, the levels of various hormones fluctuate, either increasing or decreasing, during the acute phase and after treatment. These changes may lead to unnecessary treatment initiation in patients. Therefore, hormone levels should be reassessed after the resolution of the acute condition, and if any hormonal disorder persists, appropriate treatment should be initiated.
