Özet
ÖZET Rektum kanserli olgularda sfinkter koruyucu işlemlerden sonra yaşam kalitesi daha iyi olmakla birlikte, bazı hastalarda defakasyon sayısında artış, gaz-gaita inkontinensi, rektal kapasitede azalma, rektoanal inhibitor refleks yokluğu ve duyusal eşik kaybı gibi semptom ve bulgular ortaya çıkmaktadır. Diyare ve inkontinens, radyasyon hasarına bağlı sekonder olarak gelişen semptomlar olarak kabul edilir. Bu çalışmada amacımız sfinkter koruyucu cerrahi girişim uygulanan rektum kanserli olgularda preoperatif veya postoperatif radyoterapinin, postoperatif dönemde anal sfinkter fonksiyonları üzerine etkisini ortaya koymaktır. Böylece sfinkter koruyucu rezeksiyon ve radyoterapi uygulanan hastaların fonksiyonel sonuçları sfinkter koruyucu rezeksiyon yapılıp radyoterapi uygulanmayan hastalarla karşılaştırılmış ve radyoterapinin sfinkter koruyucu rezeksiyonlarda sfinkter fonksiyonlarına spesifik etkileri belirlenmeye çalışılmıştır. Bu çalışmada çalışma grubunu oluşturan sfinter koruyucu rezeksiyon ile birlikte radyoterapi uygulanan on hastamn sonuçları radyoterapi uygulanmaksızın aynı ameliyatı geçiren on hastalık kontrol gurubu ile karşılaştırılmıştır. Hastalar bir yazı ile kliniğimize davet edilerek anal manometri yapılmıştır. Anal manometri ile maksimum sıkma basıncı, maksimum istirahat basıncı, rektoanal inhibitor refleks varlığı ve duyusal eşik volümü her iki grup hastada ölçülmüş ve karşılaştırılmıştır. Elde edilen sonuçlar Mann Whitney U, Chi-Square testleri ile değerlendirilmiştir. Sonuç olarak aşağı seviyeli rektum kanserlerinde lokal nüks olasılığını azaltmak için tedaviye eklenen preoperatif veya postoperatif radyoterapinin sfinkter fonksiyonları üzerine olumsuz etki göstermediği kanısına varılmıştır. 50

SUMMARY THE EFFECT OF RADIOTHERAPY TO SPİNCTER FUNCTIONS AFTER LOW ANTERIOR RESECTÎON. Although the quality of life is better after sphincter saving procedures, the syuptoms of increased number of defecation, incontinence to stool and gas, decrease in rectal capacity, loss of rectoanal inhibitory reflex and decrease in rectal sensibility occur in some patients. Diarrhea and incontinence are accepted as symptoms secondary to radiation injury. The purpose of this study is to investigate the effect of preoperative or postoperative radiotherapy on bowel function in patients treated with low anterior resection. The functional results of the patients given radiotherapy with low anterior resection were compared to the results of the patients operated with low anterior resection without radiotherapy, so the effects of radiotherapy on sphincter functions in patients with anterior resection are tried to be determined spesifically. In this study, the results of ten patients, study group, treated with sphincter saving procedure with radiotherapy were compared to the results of the patients, control group, who had same operation without radiotherapy. The patients were invated to the clinic with a letter and results were determined by anal manometry. Maximum squeezing pressure, anal canal resting pressure, rectoanal inhibitory reflex and rectal sensibility volumes were measured and compared in both groups. The results were analysed with Mann Whitney U and chi-square tests. As a result, preoperative and postoperative radiotherapy for the purpose of decreasing local recurence in rectal cancers has no unfavorable effect on sphincter functions. 51
