Özet
ÖZET Rektum kanserlerinin preoperatif evrelemesi, tedavi planı ve cerrahi girişim tipinin seçiminde önemli bir rol oynamaktadır. Bu konuda rektosigmoidoskopi ve baryum lavmanlı kolon grafısi gibi konvansiyonel yöntemler gerek duvar invazyonunun derecesi, gerek selenfatik metastazların varlığını belirlemede oldukça sınırlı bir etkinliğe sahiptir. Modern teknoloji MRI ve Endoluminal ultrasonografi gibi yöntemleri bu konuda kullanım alanına sokmuş ve bunların kullanım yararlılığı konusunda anlamlı sonuçlar bildirilmektedir. Klasik tanı yöntemlerine eklenecek MRI ve Endorektal ultrasonografinin rektum kanserlerinin preoperatif evrelenmesindeki etkinliğini araştırmak amacıyla prospektif bir çalışma yapıldı. Çalışmada rektum kanserli 13 olguda rektal tuşe, rektosigmoidoskopiye ek olarak MRI ve Endoluminal ultrasonografik inceleme yapıldı. Belirlenen preoperatif evreleıne bulguları, postoperatif histopatolojik bulgularla karşılaştırıldı. Sadece rektal tuşe ve rektosigmoidoskopik muayene sonucu düşünülen preoperatif evrelemenin doğruluk oranı % 61.6 iken, Endorektal ultrasonografi eklendiğinde bu oran %84.6 oldu, MRI eklenmesiyle ise % 88.8 'e yükseldi. Bu sonuçlarla ; rektal tuşe, rektosigmoidoskopi, Endorektal ultrasonografi ve Magnetik Rezonans Görüntüleme yöntemleri rektum kanserlerinin doğru bir preoperatif evreleme ve uygun tedavi şeklinin seçiminde oldukça etkin ve yararlı yöntemler olarak görülmektedir. 45

SUMMARY Preoperative staging of rectal cancer plays an important role on planning therapy and choice of the surgical procedure Conventional examinations for rectal cancer; such as rectosigmoidoscopy and contrast barium enema yield limited evidence regarding the depth of mural invasion and lymphatic metastases. Modern technology introduced Magnetic Resonance and Endoluminal Ultrasonography as diagnostic tools and there are reports of favorable results by using these methods. We searched for the efficacy of MRI and Endorectal Ultrasonography in preoparitive staging of rectal cancer when added to classic diagnostic procedures. In the study; MRI and Endoluminal Ultrasonography are used beside rectal digital examination and rectosigmoidoscopy on 13 cases with rectal cancer. We compared our preoperative staging and findings with the postoperative histopathologic findings. The accuracy rate of preoperative staging solely with rectal digital examination and rectosigmoidoscopy was 61.6% and with the addition of Endorectal Ultrasonography and MRI the rate of accuracy rised to 84.6 % and 88.8 % ; respectively. So, these results show the significant importance and advantages of rectal digital examination, rectosigmoidoscopy, Endorectal Ulstrasonography and MRI techniques in a current preoperative staging and the choice of appropriate surgical treatment for rectal cancer. 46
