Özet

Amaç: Çocukluk yaş grubu akut semptomatik nöbetlerinde (ASN) merkezi sinir sistemi enfeksiyonları önemli bir etiyolojik yer kaplamaktadır. Bu hastaların nörolojik takiplerinde hangilerinin uzak semptomatik epilepsi riskine sahip olduğunu bilmek klinisyenlerin takip, tedavi ve prognoz planlarını yapmasında önemli yer tutmaktadır. Bu çalışmada amaç, merkezi sinir sistemi enfeksiyonuna bağlı akut semptomatik nöbetlerin klinik, laboratuvar ve elektrofizyolojik özelliklerini değerlendirmek; hastaların tedavi ve uzun dönem nörolojik takip sonuçlarını incelemektir. Gereç ve Yöntem: S.B.Ü. Dr. Sami Ulus Kadın Doğum, Çocuk Sağlığı ve Hastalıkları Eğitim ve Araştırma Hastanesi'nde 1 Ocak 2015 – 31 Aralık 2020 tarihleri arasında merkezi sinir sistemi enfeksiyonu tanısı almış ve Çocuk Nöroloji bölümüne konsülte edilerek takibe alınmış hastaların akut semptomatik nöbetlerinin klinik ve elektrofizyolojik özellikleri retrospektif olarak kesitsel ve tanımlayıcı bir şekilde incelenmiştir. Analiz edilen klinik değişkenler, yaş, cinsiyet, başvuru günü ölçülen en yüksek vücut sıcaklığı değeri, semptom dağılımı, semptom süresi, ateş başlangıcı ve nöbet arasındaki süre, lökosit sayımı, CRP değerleri, status epileptikus varlığı, başvuru Glaskow Koma Skoru (GKS), yatış süresi, yoğun bakım ünitesine (YBÜ) yatış gerekliliği, yoğun bakım yatış nedeni, yoğun bakımda destek tedavisi ihtiyacı, elektroensefalografi (EEG) anormalliği ve anti-nöbet tedavi gerekliliğidir. Hastaların 3-6 ay ara ile yapılan nöroloji poliklinik takiplerinde tespit edilmiş nörolojik ve bilişsel sekel gelişimi değerlendirilmiştir. Ayrıca EEG anormalliği olan hastaların takip EEG anormallikleri, anormalliğin süresi, anti-nöbet tedavi süresi, uzak semptomatik nöbet (USN) varlığı, epilepsi tanısı alma durumu incelenmiştir. Bulgular: Çalışmaya 34 kız (%68,0) 16 erkek (%32,0) olmak üzere toplam 50 hasta dahil edilmiştir. Hastaların yaşları 1 ay ile 204 ay (17 yıl) arasında olup ortanca yaş 36 ay olarak bulunmuştur. Hastaların yaş dağılımının; <2 yaş 22 hasta (%44,0), 2-<6 yaş 11 hasta (%22,0), 6-<12 yaş 12 hasta (%24,0), ≥12 yaş 5 hasta (%10,0) olduğu görülmüştür. Hastaların 27'sinde (%54,0) menenjit, 23'ünde (%46,0) ensefalit tanısı konulduğu saptanmıştır. On hastanın (%20,0) bakteriyel, 40 hastanın (%40,0) viral etiyolojiye sahip olduğu düşünülmüştür. Hastaların 19'unda (%38,0) BOS kültür veya PCR yoluyla etken saptanmış, en sık saptanan viral etken HSV-1 ve bakteriyel etken Meningokok olmuştur. Çalışmada 29 hastanın (%58,0) ASN geçirdiği saptanmıştır. Bu hastaların yaşları 1 ay ile 156 ay (13 yaş) arasında olup ortanca 12 ay olarak bulunmuştur. Çalışmada patojenden bağımsız olarak viral ensefalitlerin %86,3'ünde, bakteriyel menenjitlerin %37,5'inde ASN görülmüştür. Akut semptomatik nöbet görülen hastalarda en sık jeneralize tonik (JT) nöbet (%41,38) gözlenmiştir. Akut semptomatik nöbet görülen hastaların 13'ünde (%44,8) Status Epileptikus geliştiği saptanmış ve bu hastaların başvurudaki GKS'leri daha düşük ve semptomlar ile nöbet arasında geçen süre daha kısa olarak bulunmuştur (p<0,001; p=0,043). Başvuru döneminde MSS enfeksiyonu tanılı hastaların 39'una (%78,0) EEG çekilmiş olup, bu hastaların 19'unda (%48,7) anormallik tespit edilmiş; en sık fokal epileptiform anormallik (%52,6) ve en sık temporal lokalizasyon (%63,2) tespit edilmiş olduğu görülmüştür. Yapılan çalışmalarda 17 hastanın (%34,0) patolojik nörogörüntüleme bulgusuna sahip olduğu saptanmıştır. Antinöbet ilaç tedavisinin ASN geçiren 29 hastadan 24'üne (%82,75) başlandığı; 5 hastanın nöbet tekrarı ve EEG anormalliği olmadığı için ASN kontrolü sonrası antinöbet ilaç idame tedavisine devam edilmediği tespit edilmiştir. Ensefalit görülen grupta menenjit görülen gruba göre ASN, status epileptikus, YBÜ ihtiyacı, başvuru dönemi EEG patolojisi, USN daha fazla olduğu ve daha düşük GKS değerine sahip olduğu bulunmuştur (p<0,05). Sekiz hastada (%16) USN görülmüştür. Uzak semptomatik nöbeti olan hastaların başvurudaki GKS'leri olmayanlara göre daha düşük; ensefalit sıklığı, status epileptikus varlığı, başvuru dönemi EEG patolojisi, kontrol dönemi EEG patolojisi, psikiyatrik/bilişsel bozukluk ve nörolojik sekel gelişimi daha fazla görülmüştür (p<0,05). Epilepsi gelişen 14 hastanın başvuru nörogörüntüleme çalışmalarında %50 oranında patolojik bulgu saptandığı tespit edilmiştir. Sonuç: Çalışmamızda MSS enfeksiyonu tanısı almış çocuk hastaların yaklaşık yarısında hastalıkla ilişkili akut semptomatik nöbetlerinin geliştiği, bu hastaların çoğunluğunun akut ensefalit kliniğine sahip olduğu, hastaların beşte birinde uzak semptomatik nöbet gözlendiği, hastaların yaklaşık üçte birinde uzak semptomatik epilepsi tanısı konulduğu tespit edilmiştir. Merkezi sinir sistemi enfeksiyonu ilişkili ASN geçiren hastaların klinik ve elektrofizyolojik bulgularının standart takibi için daha ileri prospektif randomize kontrollü çalışmalara ihtiyaç vardır.

Objective: Central nervous system infections play a significant role in the etiology of acute symptomatic seizures (ASS) in the childhood age group. Knowing which patients are at risk for remote symptomatic epilepsy in the neurological follow-up of these patients is crucial for clinicians to plan follow-up, treatment, and prognosis. The aim of this study is to evaluate the clinical, laboratory, and electrophysiological characteristics of acute symptomatic seizures due to central nervous system infections and examine the treatment and long-term neurological follow-up outcomes of patients. Material and Methods: The clinical and electrophysiological features of acute symptomatic seizures in patients diagnosed with central nervous system infection and followed up in the Pediatric Neurology Department between January 1, 2015, and December 31, 2020, at S.B.Ü. Dr. Sami Ulus Maternity, Child Health and Diseases Training and Research Hospital were retrospectively analyzed in a cross-sectional and descriptive manner. The analyzed clinical variables included age, gender, the highest body temperature measured on the day of admission, symptom distribution, symptom duration, fever onset, and duration between fever onset and seizure, leukocyte count, CRP values, presence of status epilepticus, Glasgow Coma Scale (GCS) at admission, duration of hospitalization, need for intensive care unit (ICU) admission, reason for ICU admission, need for supportive treatment in the ICU, EEG abnormality, and the need for anti-seizure treatment. The neurological and cognitive sequelae detected during neurology outpatient follow-ups conducted every 3-6 months were evaluated. Additionally, follow-up EEG abnormalities, duration of abnormality, duration of anti-seizure treatment, presence of remote symptomatic seizures (RSS), and the development of epilepsy were investigated for patients with EEG abnormalities. Results: A total of 50 patients were included in the study, consisting of 34 girls (68.0%) and 16 boys (32.0%). The age of the patients ranged from 1 month to 204 months (17 years) with a mean age of 57.6 ± 58.0 months and a median age of 36 months. The distribution of patients' ages was as follows: <2 years in 22 patients (44.0%), 2-<6 years in 11 patients (22.0%), 6-<12 years in 12 patients (24.0%), and ≥12 years in 5 patients (10.0%). Meningitis was diagnosed in 27 patients (54.0%), and encephalitis was diagnosed in 23 patients (46.0%). Ten patients (20.0%) were considered to have bacterial etiology, and 40 patients (40.0%) were considered to have viral etiology. Causative agents were identified in 19 patients (38.0%) through cerebrospinal fluid culture or PCR, with HSV-1 being the most commonly identified viral agent and Meningococcus being the most common bacterial agent. In the study, 29 patients (58.0%) were found to have experienced ASS. The ages of these patients ranged from 1 month to 156 months (13 years) with a mean of 43.7 ± 49.9 months and a median of 12 months. Regardless of the pathogen, ASS was observed in 86.3% of viral encephalitis and 37.5% of bacterial meningitis cases. Generalized tonic (GT) seizures were the most common type observed in patients with ASS (41.38%). Status epilepticus was observed in 13 patients (44.8%) with ASS, and these patients had lower GCS at admission and a shorter duration between symptoms and seizures (p<0.001; p=0.043). During the admission period, EEG was performed in 39 patients (78.0%) diagnosed with CNS infection, and abnormalities were detected in 19 patients (48.7%), with focal epileptiform abnormalities (52.6%) and temporal localization (63.2%) being the most common. Neuroimaging findings were pathological in 17 patients (34.0%). Anti-seizure medication was initiated in 24 out of 29 patients (82.75%) with ASS, and in 5 patients, treatment was not continued after ASS control due to the absence of seizure recurrence and EEG abnormalities. In the group with encephalitis, ASS, status epilepticus, the need for ICU, EEG abnormalities during the admission period, and RSS were more common, and they had lower GCS values compared to the group with meningitis (p<0.05). Remote symptomatic seizures were observed in eight patients (16%). Patients with RSS had lower GCS at admission, higher frequency of encephalitis, presence of status epilepticus, EEG abnormalities during admission, EEG abnormalities during the control period, psychiatric/cognitive impairment, and higher frequency of neurological sequelae compared to those without RSS (p<0.05). It was found that 50% of the 14 patients who developed epilepsy had pathological findings in the initial neuroimaging studies. Conclusion: In our study, it was found that approximately half of the pediatric patients diagnosed with CNS infection developed acute symptomatic seizures, most of whom had a clinical picture of acute encephalitis. Remote symptomatic seizures were observed in one-fifth of the patients, and approximately one-third of the patients were diagnosed with remote symptomatic epilepsy. Further prospective randomized controlled studies are needed for the standard follow-up of the clinical and electrophysiological findings of patients with acute symptomatic seizures associated with central nervous system infections.
