Özet

GİRİŞ VE AMAÇ: Bu çalışmada, Covid-19 pandemisi süresince akut apandisit nedenli ameliyat edilen ve tanısı patoloji ile doğrulanmış hastalarda; ameliyat öncesindeki değerlendirme sürecinde kullanılan Akut Apandisit İnflamatuar Yanıt Skoru, Alvarado ve Modifiye Alvarado skorlarının içerisinde yer alan, beyaz küre yüksekliği, nötrofil hakimiyeti ve CRP parametrelerinin, Covid-19 PCR testi pozitif veya negatif olan akut apandisit hastalarında karşılaştırılması amaçlanmaktadır. GEREÇ VE YÖNTEM: Retrospektif olaran dizayn edilen bu çalışmaya, T.C. Sağlık Bakanlığı Ankara Şehir Hastanesi'ne Mart 2020- Mayıs 2022 tarihleri arasında başvurmuş, akut apandisit nedenli opere edilen, patoloji sonucu ile tanısı doğrulanmış ve Covid-19 PCR test sonucu olan 866 hasta dahil edildi. Çalışma verileri IBM SPSS 26 yazılımı kullanılarak analiz edildi. Tüm testlerde p<0,05 değerleri anlamlı kabul edildi. BULGULAR: Çalışmaya dahil edilen 866 hastanın yaş ortalaması 39,19'du. Hastaların %59'u erkek, %41'i kadındı ve hastaların %86,1'inin Covid-19 PCR test sonucu negatif, %13,9'unun ise pozitifti. Bu iki grubun ameliyat öncesi bakılan laboratuvar parametrelerinden nötrofil yüzdesi, Covid-19 PCR testi pozitif olanlarda anlamlı şekilde yüksek iken(pNEU%=0,003), lenfosit sayısı, lenfosit yüzdesi ve CRP düzeyi Covid-19 PCR testi negatif olan grupta anlamlı şekilde daha yüksekti(pLYM=0.002, pLYM%=0.001 ve pCRP=0.014). SONUÇ: Akut Apandisit tanısında nötrofil yüzdesi, lenfosit sayısı, lenfosit yüzdesi ve CRP değerleri için Covid-19 PCR testi negatif ve pozitif olan hastalarda istatistiksel olarak anlamlı fark bulundu. Sonuçlarımız Covid-19 hastalığının akut apandisit ile birlikte olduğu durumlarda, uzun süredir ve yaygın şekilde kullanılan Alvarado, Modifiye Alvarado ve Apandisit İnflamatuar Yanıt Skoru üzerine etkisi olabileceğini düşündürmektedir.

INTRODUCTION AND AIM: In this study, in patients who underwent surgery for acute appendicitis during the Covid-19 pandemic and whose diagnosis was confirmed by pathology; It is aimed to compare the white blood cell elevation, neutrophil dominance and CRP parameters, which are included in the Acute Appendicitis Inflammatory Response Score, Alvarado and Modified Alvarado scores used in the preoperative evaluation process, in acute appendicitis patients with positive or negative Covid-19 PCR test. MATERIALS AND METHODS: This retrospectively designed study, 866 patients who were admitted to T.C. the Ministry of Health Ankara City Hospital between March 2020 and May 2022, who were operated for acute appendicitis, whose diagnosis was confirmed by pathology results, and who had Covid-19 PCR test results were included in the study. Study data were analyzed using IBM SPSS 26 software. P values of <0.05 were considered significant in all tests. RESULTS: The mean age of 866 patients included in the study was 39.19 years. 59% of the patients were male, 41% were female, and the Covid-19 PCR test result of 86.1% of the patients was negative and 13.9% of them were positive. While the percentage of neutrophils, one of the preoperative laboratory parameters of these two groups, was significantly higher in those with positive Covid-19 PCR test (pNEU%=0.003), lymphocyte count, lymphocyte percentage and CRP level were significantly higher in the group with negative Covid-19 PCR test (pLYM=0.002, pLYM%=0.001 and pCRP=0.014). CONCLUSION: A statistically significant difference was found in patients with negative and positive Covid-19 PCR test for neutrophil percentage, lymphocyte count, lymphocyte percentage and CRP values in the diagnosis of acute appendicitis. Our results suggest that in cases where Covid-19 disease is associated with acute appendicitis, it may have an effect on the long and widely used Alvarado, Modified Alvarado and Appendicitis Inflammatory Response Score.
