Özet

Amaç Covid-19 pandemi öncesi dönem ile pandemi sürecinde karın ağrısı şikayetisonucu akut apandisit tanısı alan hastaların hastaneye başvuru oranı, semptom başlama süresi, Alvarado skoru, tedavi şekli, komplikasyon gelişme riski, hastanede kalış süresini belirlemeyi amaçladık. Materyal ve Metod Retropektif çalışma olarak tasarlanan çalışmamızda01.03.2019-31.08.2019 ile 01.03.2020-31.08.2020tarihleri arasında Bakırköy Dr. Sadi Konuk EAH erişkin acil tıp kliniğinekarın ağrısı şikâyetisonucu başvurmuş akut apandisit tanısı alan 18 yaş üstü toplam 420 hasta değerlendirildi. Hastaların yaş, cinsiyet, vital bulguları, kronik hastalıkları, geçirilmiş cerrahi öyküsü ve ilaç kullanım bilgileri, Alvarado skoruolgu formunakaydedildi. Hastaların acil serviste kalış süresi, hastanede kalış süresi, tedavi şekli, morbidite, yoğun bakım yatış ve mortaliteleri gibi bulgular vaka formuna kaydedildi. Bulgular Çalışmaya alınan vakaların 267'si (%63,6) pandemi öncesi, 153'ü (%36,4) pandemide başvuran hastalardı. Çalışmaya alınan vakaların 271'i (%64,5) erkek, 149'u (%35,5) kadındı.Pandemi öncesi başvuran hastaların Alvarado skor ortalaması 5,95 ± 1,467 şeklinde iken, pandemidebaşvuran hastaların Alvarado skor ortalaması 6,26 ± 1,490 şeklinde bulundu.Ultrasonografi görüntüleme sonuçlarında pandemi öncesi vakaların 98'inde (%56,6), pandemide vakaların 27'sinde (%24,1) akut apandisit bulgusu vardı. Bilgisayarlı tomografi çekilen hastaların pandemi öncesi 143'ünde (%78,6), pandemide 113'ünde (%84,3) akut apandisit bulgusu saptandı. Pandemi öncesi vakaların 266'sında (%99,6), pandemide vakaların 148'inde (%96,7) cerrahi tedavi uygulandı. Pandemi öncesi vakaların 41'inde (%15,4), pandemide vakaların 33'ünde (%21,6) komplike apandisit bulgusu saptandı. Sonuç Yaptığımız çalışmada pandemi sürecinin hastaneye başvuruları azalttığını ve önemli yaşamsal şikâyetleri olan hastaların hastane başvurusunun gecikmesine neden olduğunu saptadık. Çalışmada pandemi dönemindebaşvuran hastaların Alvarado skorları daha yüksek saptanmıştır. Pandemi dönemi akut apandisit olgularının komplikasyon riskini artırmamıştır.

Objective We aimed to determine the hospital admission rate, symptom onset time, Alvarado score, type of treatment, risk of complication development and length of hospital stay of patients who were diagnosed with acute appendicitis as a result of abdominal pain during the pre-Covid-19 pandemic period and the pandemic period. Material and Method In our study, which was designed as a retrospective study, a total of 420 patients over the age of 18 who were diagnosed with acute appendicitis who applied to the Emergency Medicine Clinic of Bakırköy Sadi KonukTraining and Research Hospital with the complaint of abdominal pain between March 1, 2019 to August 31, 2019 and March 1, 2020 to August 31, 2020 were evaluated. Data was included age, sex, patient demographics, comorbidities, vital sign in the application, previous surgical history, medicines, Alvarado scores, treatment, complications and length of hospital stay. Results Of the cases included in the study, 267 (63.6%) were admitted before the onset of the pandemic, and 153 (36.4%) were admitted after the onset of the pandemic.271 (64.5%) of the cases were male and 149 (35.5%) were female.While the mean Alvarado score of the patients who applied before the onset of the pandemic was 5.95 ± 1.467, the mean Alvarado score of the patients who applied after the onset of the pandemic was 6.26 ± 1.490.Findings of acute appendicitis in ultrasonography were detected in 98 (56.6%) of the cases before the pandemic periodand in 27 (24.1%) of the cases during the pandemic period.Of the patients who underwent computed tomography, 143 (78.6%) before the pandemic and 113 (84.3%) during the pandemic had signs of acute appendicitis. Surgical treatment was performed in 266 (99.6%) of the pre-pandemic cases and in 148 (96.7%) of the pandemic cases.Complicated appendicitis was found in 41 (15.4%) of the pre-pandemic cases and in 33 (21.6%) of the pandemic cases. Conclusion In our study, we found that the pandemic process reduces hospital admissions and causes delays in hospital admissions of patients who had important vital complaints. Our results showed that the Alvarado scores of the patients who applied during the pandemic period were found to be higher than those who applied in the pre-pandemic period.When the pre-pandemic period and the pandemic period were compared, no significant difference was found in terms of complication development.
