Özet

ÖZET Amaç: Lomber disk cerahisi geçiren hastalarda erektör spina plan (ESP) bloğu ve retrolaminar bloğun (RLB) analjezik etkinliklerinin kontrol grubu ile karşılaştırılması. Gereç ve Yöntem: Etik kurul ve hasta onamı alınmasını takiben, ASA I-III olan 75 hasta ESP, RLB ve Kontrol gruplarına ayrıldı. Erektör spina plan ve RL blokları ultrasonografi (USG) eşliğinde bilateral uygulandı. Tüm gruplardaki hastalara intravenöz hasta kontrollü analjezi (HKA) uygulandı. Standart anestezi indüksiyonunu takiben sevofluran ve remifentanil infüzyonu ile idame sağlandı. Postoperatif analjezik etkinlik 0., 2., 4., 8., 12., ve 24. saatlerde istirahat ve hareket halindeki sayısal değerlendirme skalası (NRS), tramadol tüketimi, ek analjezik gereksinimi ile değerlendirildi. Grupların intraoperatif opioid tüketimi, hemodinamik değişkenleri, komplikasyon görülme sıklığı ve son olarak memnuniyetleri karşılaştırıldı. Bulgular: ESP ve RLB grublarında, kontrol grubuna göre tüm saatlerde daha az tramadol tüketilmiştir ve aralarındaki fark anlamlı bulunmuştur. [Grup ESP: 159,2±76,04 mg; Grup RLB: 145,6±103,36 mg; Grup Kontrol: 260,8±122,95 mg sırasıyla p=0,002, p=0,001]. ESP ve RLB grubları tramadol tüketimleri arasında anlamlı fark bulunmamıştır (p=0,641). Grupların postoperatif istirahat ağrı skorları 0., 2., 8. ve 24. saat haricinde benzerdi; 0., 2. saat ve 24. saat NRS-i Grup ESP, Grup RLB'den ve Grup Kontrol'den düşük (p=0,001, p=0,001, p=0,029), 12. saat NRS-i Grup ESP ve Grup RLB, kontrol grubundan düşük bulundu (p=0,028, p=0,027). 0. saat NRS-h Grup ESP ve Grup RLB, kontrol grubundan düşük bulundu (p=0,001, p=0,007). 2. ve 12. saat NRS-h Grup ESP, Grup RLB'den ve kontrol grubundan düşük bulundu (p=0,042 p=0,040). Grupların intraoperatif remifentanil tüketimleri Grup ESP'de ve RLB'de Kontrol grubuna göre anlamlı düşük bulundu (p<0,001, p<0,001). Araştırma gruplarında komplikasyon gözlenmedi. Sonuç: ESP bloğu ve RLB lomber disk operasyonlarında intraoperatif daha az remifentanil tüketimi ve postoperatif daha az tramadol tüketimi sağlaması nedeniyle postoperatif analjezik etkinlik bakımından sadece HKA yöntemi uygulanan kontrol grubundan üstündür. Ayrıca ESP grubunun RLB grubuna göre NRS skor takiplerinde analjezik etkinliğinin üstünlüğü gösterildi. Gruplar arasında yan etki ve komplikasyon açısından anlamlı bir farklılık bulunmadı.

ABSTRACT Objective: In the present study it is aimed to compare the analgesic efficacy of erector spinae plane (ESP) block and retrolaminar block (RLB) in patients who had lumbar disc surgery with the control group. Materials and Methods: Following local ethics committee approval and patient consent, 75 patients with ASA I-III were divided into ESP, RLB, and Control groups. Erector spinae plane and RL blocks were performed bilaterally under ultrasonography (USG) guidance. Intravenous patient-controlled analgesia (PCA) was administered to patients in all groups. Following standard anesthesia induction, maintenance was provided with sevoflurane and remifentanil infusion. Postoperative analgesic efficacy was evaluated at 0, 2, 4, 8, 12, and 24 hours, using the rest and motion numerical rating scale (NRS), tramadol consumption, and additional analgesic requirement. Intraoperative opioid consumption, hemodynamic variables, the incidence of complications, and finally, overall satisfaction of the groups were compared. Results: In ESP and RLB groups, less tramadol was consumed at all hours compared to the control group, and the difference between groups was found to be significant. [ESP Group: 159.2±76.04 mg; RLB Group: 145.6±103.36 mg; Control Group: 260.8±122.95 mg, p=0.002, p=0.001, respectively]. There was no significant difference between the tramadol consumptions of the ESP and RLB groups (p=0.641). Postoperative rest pain scores of the groups were similar except at 0, 2, 8, and 24 hours; while 0th, 2nd, and 24th-hour NRS-i values were lower in the ESP Group than RLB Group and Control Group (p=0.001, p=0.001, p=0.029), 12th-hour NRS-i values were found to be lower in the ESP Group and RLB Group than the Control Group (p=0.028, p=0.027, respectively). NRS-h value at the 0th hour was lower in the ESP Group and RLB Group than in the control group (p=0.001, p=0.007, respectively). The 2nd and 12th-hour NRS-h values were lower in the ESP Group than in the RLB Group and Control Group (p=0.042, p=0.040, respectively). Intraoperative remifentanil consumption of the groups was found to be significantly lower in the ESP Group and RLB Group compared to the Control Group (p<0.001, p<0.001). No complications were observed in the study groups. Conclusion: ESP block and RLB method are superior to the control group in terms of postoperative analgesic efficacy, as they provide less intraoperative remifentanil and postoperative tramadol consumption in lumbar disc operations. In addition, the analgesic efficacy of the ESP group was shown to be superior to the RLB group in NRS score follow-ups. There was no significant difference between the groups in terms of side effects and complications.
