Özet

GİRİŞ VE AMAÇ:Diyabetes mellitus (DM) vücutta insülinin üretilmediği, az üretildiği veya üretilen insülinin etkili bir şekilde kullanılamadığı durumlarda ortaya çıkan, kan şekeri yüksekliğiyle seyreden kronik bir hastalıktır . Tüm diyabet hastalarının %75-90'ını Tip 2 DM hastaları oluşturmaktadır . 31 Aralık 2019'da Çin'in Wuhan vilayetinde zatürre vakaları gözlenmiş ve etken olarak yeni tip koronavirüs saptanmıştır. SARS-CoV'a benzerliğinden SARS-Cov-2 olarak adlandırılmış, hastalığın ismi 'COVID-19' kabul edilmiştir. Salgın başlangıcından itibaren Çin haricinde 113 ülkede vakaların tespit edilmesiyle Dünya Sağlık Örgütü (DSÖ) tarafından 11 Mart 2020 de küresel salgın ( pandemi) ilan edilmiştir . Çalışma 65 yaş ve üzeri Tip 2 DM hastalarında Covid-19 pandemi sürecinin diyabet regülasyonu ve lipit profili üzerindeki etkisini göstermek amacıyla yapılmıştır. GEREÇ VE YÖNTEM: Sağlık Bilimleri Üniversitesi Bozyaka Eğitim ve Araştırma Hastanesi Endokrinoloji ve Metabolizma Hastalıkları Polikliniği'nde , Mart 2019-Mart 2021 tarihleri arasında takip edilen 65 yaş ve üzeri Tip 2 DM tanılı ve COVID-19 ile enfekte olmamış 129 hastanın verileri retrospektif olarak incelendi. Her iki dönemde de başvurmuş hastaların pandemi öncesi 1 yıl ve pandemi sonrasındaki 1 yıl içindeki verileri karşılaştırıldı. Açlık plazma glukozu (APG), tokluk post prandial glukoz (PPG) , Glikolize hemoglobin (HbA1c), total kolesterol ( TK), düşük yoğunluklu lipoprotein(LDL), yüksek yoğunluklu lipoprotein (HDL), trigliserid (TG) parametreleri değerlendirildi. Verilerin karşılaştırılması ve değerlendirilmesi SPSS (Statistical Package for the Social Sciences) versiyon 25.0 yazılım ile yapılmıştır. P <0,05 anlamlı olarak kabul edilmiştir. BULGULAR : COVID-19 ile enfekte olmamış 65 yaş ve üzeri 129 Tip 2 DM hastası incelenmiş, bu hastalardan 65'i (%50,4) erkek ve 64'ü (%49,6) kadınlardan oluşmaktadır.Hastaların yaş ortalaması 70,33 ± 4,488 (min. 65- max. 84) olup hastaların %50,4'ü 65-70 yaş arasındadır. Hastaların pandemi öncesi ve pandemi dönemi laboratuar bulgularında APG 144,39±51,41 mg/dl (min. 87- max. 315)'den ,162,79±60,88 mg/dl (min.84- max. 341)'ye çıkmıştır. Benzer şekilde PPG ortalaması 195,50±56,88 mg/dl (min. 96- max. 350)'den 218,05±69,91 mg/dl (min. 110- max. 431)' e belirgin bir yükseliş göstermiştir. HbA1c ise pandemi öncesi %7,56 iken pandemi dönemi ortalama %7,96' dır. Total kolesterol ortalama değeri pandemi öncesi 194,01±47,37 mg/dl, pandemi dönemi 205,87±51,41 mg/dl olmuştur. HDL ise 50,23±12,53 mg/dl (min. 27- max. 96) olan pandemi öncesi değerden 47,95±12,00 (min.24 - max. 86) olan değere düşmüştür. LDL kolesterol ortalaması 114,24±36,96 mg/dl (min.52- max.220)'den , 121,93±40,04 mg/dl (min. 57- max. 243)'e yükseliş göstermiştir. Trigliserid pandemi öncesi ortalaması 152,39±70,61 mg/dl (min. 40- max. 348)'den , pandemi dönemi ortalaması 173,42±84,32 mg/dl (min. 44- max.470)'e yükselmiştir. Laboratuvar bulgularında değişimi analizlerine göre; pandemi döneminde öncesine göre açlık plazma glukozu, tokluk post prandial glukoz, total kolesterol, LDL, trigliserid, HDL ve HbA1c'teki artış istatistiksel olarak anlamlı saptanmıştır. (p<0,05) SONUÇ: Yapabildiğimiz literatür taramasına göre bu çalışma ülkemizde pandemi sürecinde yapılan 65 yaş ve üzeri Tip 2 DM hastalarıyla ilgili ilk çalışmadır. Diyabet gibi kronik hastalıklarda tedavi süreci ve komplikasyonları önlemek beraberinde uzun ve maliyetli bir sürece yol açmaktadır .Böylece dünyada yaşlanan nüfusla beraber geriatrik dönemdeki kronik hastalıklara yönelik çalışmaların artması gerektiğini göstermektedir.

Introduction and Aim : Diabetes mellitus (DM) is a chronic disease with high blood glucose, which occurs when the body does not produce insulin, produces less or cannot use the produced insulin effectively . 80-90% of all diabetes patients consist of Type 2 DM patients. The average life expectancy and the number of elderly population are increasing worldwide . On December 31, 2019, cases of pneumonia were observed in Wuhan province of China and a new type of coronavirus was detected as the causative agent. It was named SARS-Cov-2 because of its similarity to SARS-CoV and the name of the disease was accepted as 'COVID-19'. Since the beginning of the epidemic, cases were detected in 113 countries except China, by the World Health Organization (WHO) on March 11, 2020, a global epidemic (pandemic) has been announced . The study was conducted to determine the effect of the Covid-19 pandemic process on diabetes regulation and lipid profile in Type 2 DM patients aged 65 and over. Materials and Methods: In Healt Sciences University Bozyaka Education and Research Hospital Department of Endocrinology outpatient clinic , the data of 129 patients aged 65 and over who diagnosed with Type 2 DM and not infected with COVID-19, followed between March 2019 and March 2021 were retrospectively examined. The data of the patients who applied in both periods were compared in 1 year before the pandemic and 1 year after the pandemic. Fasting plasma glucose (FPG), postprandial glucose (PPG), glycosylated hemoglobin (HbA1c), total cholesterol (TK), low-density lipoprotein (LDL), high-density lipoprotein (HDL), triglyceride (TG) parameters were evaluated. Statistical Comparison and evaluation of the data have been completed with SPSS (Statistical Package for the Social Sciences) version 25.0 software. p < 0,05 was considered and accepted as significant. Findings: 129 Type 2 DM patients aged 65 and over who were not infected with COVID-19 were examined, 65 (50.4%) of these patients were women and 64 (49.6%) were men. The mean age of the patients was 70.33 ± 4.488 (min. 65- max. 84) and 50.4% of the patients are between 65-70 years of age. In the laboratory findings of the patients before and after the pandemic, FPG increased from 144.39±51.41 mg/dl (min. 87-max. 315) to 162.79±60.88 mg/dl (min.84-max. 341). Similarly, the mean PPG showed a significant increase from 195.50±56.88 mg/dl (min. 96- max. 350) to 218.05±69.91 mg/dl (min. 110- max. 431). The average of HbA1c after the pandemic is % 8.9±1.9. While the mean value of total cholesterol was 194.01±47.37 mg/dl before the pandemic, it became 205.87±51.41 mg/dl after the pandemic.HDL decreased from the pre-pandemic value of 50.23±12.53 mg/dl (min. 27-max. 96) to the post-pandemic value of 47.95±12.00 mg/dl (min. 24-max. 86). The mean LDL cholesterol increased from 114.24±36.96 mg/dl (min.52-max.220) to 121.93±40.04 mg/dl (min.57- max.243). The mean of triglycerides increased from 152.39±70.61 mg/dl (min. 40- max. 348) before the pandemic to 173.42±84.32 mg/dl (min. 44-max. 470) after the pandemic. According to the analysis of the change in laboratory findings of the patients before and after the pandemic; A statistically significant difference was found between the pandemic period and fasting plasma glucose, post prandial glucose, total cholesterol, LDL, triglyceride, HDL and HbA1c increment changes. (p<0,05) Results: According to the literatüre review we could; this is the first study on Type 2 DM patients aged 65 and over in our country during the pandemic process. The treatment process and prevention of complications in chronic diseases such as diabetes lead to a long and costly process. This shows that studies on chronic diseases in the geriatric period should increase with the aging population in the world.
