Özet
Amaç: Diyabetes Mellitus (DM) ve koronavirüs enfeksiyonu (COVID-19) uzun dönemde olumsuz sonuçları olan, morbidite ve mortalite oranı ve bakım maliyetleri yüksek olan hastalıklardır. Diyabetes Mellitus tek başına COVID-19 enfeksiyonu açısından bir risk faktörüdür. Literatürde diyabet ve kognitif fonksiyonlar arasındaki ilişkiyi inceleyen birçok çalışma olduğu gibi, COVID-19 enfeksiyonu ve kognitif fonksiyonlar arasındaki ilişkiyi inceleyen de çalışmalar mevcuttur ancak koronavirüs enfeksiyonu geçirmiş Tip 2 DM'li hastalarda kognitif fonksiyonlara etkiyi gösteren ulusal çalışma verisi kısıtlıdır. Bu çalışmada; koronavirüs enfeksiyonunun Tip 2 DM'li hastalarda, hastalık ve hospitalizasyonun kognitif fonksiyonlar üzerine etkisininin araştırılması amaçlanmıştır. Materyal ve Metod: Çalışmamıza İstanbul Eğitim ve Araştırma Hastanesi 27 Eylül 2024-24 Kasım 2024 tarihleri arasında dahiliye-diyabet polikliniğine başvuran Tip 2 DM'li hastalardan, dahil etme ve dışlama kriterlerine göre randomize olarak seçilen 120 hasta alınmıştır. Çalışmaya alınan tüm olgular anamnez ve geçmiş verileri sorgulanarak koronovirüs enfeksiyonu geçirmeyen, geçiren ancak hastanede yatmayan ve geçiren ve hastane yatış öyküsü olan hastalar olarak üç eşit gruba ayrıldı. COVID-19 enfeksiyonu geçiren hasta grubu COVID-19 PCR testi pozitif ve/veya tipik COVID-19 enfeksiyonu ile uyumlu bilgisayarlı tomografi tutulumu olan hastalar olarak belirlendi. Hastalara ait verilere hasta dosyalarından ve hastane bilgi yönetim sistemi üzerinden ulaşıldı. Tüm olgulara başvuru anında yüz yüze ortamda hekim kontrolünde Montreal bilişsel değerlendirilme ölçeği testi (MoCA) yapılarak skoru hesaplandı ve kaydedildi. Test olarak ülkelere göre düzenlenmiş olan Montreal bilişsel değerlendirilme Testi Türkiye versiyonu (MoCA-TR) kullanıldı. MoCA-TR' nin Türk Psikiyatri Derneği tarafından kabul edilen toplam puanın 21 ve üzerinde olmasının katılımcının normal sınırlar içinde olduğunu (1) gösterdiği baz alınarak kesme değeri 21 olarak kabul edildi. Ayrıca Montreal bilişsel değerlendirme ölçeği içeriğinde kullanılan parametrelere göre de hastaların bellek, dikkat, dil, görsel uzaysal beceriler, yer zaman oryantasyonu, yönetici işlevler gruplarından aldığı puanlar ayrı ayrı hesaplanarak kaydedildi. Grupların başvuru anındaki yaş, cinsiyet, kronik hastalıklar, kullandığı ilaçlar, laboratuvar testleri (üre, kreatinin, hemogram, açlık kan şekeri, HbA1c, T.kolesterol, TG, LDL, HDL, AST, ALT, Total protein, Albümin, B12 vitamini, Folik asit düzeyleri) sistem üzerinden kaydedildi. COVID-19 enfeksiyonu geçirme durumu ve Montreal bilişsel değerlendirilme ölçeği skoru değerlerine göre belirlenen gruplar önce grup içi daha sonra da birbirleriyle belirlenen parametreler açısından karşılaştırıldı. Sonuçlar SPSS 28.0 istatistiksel analiz yöntemiyle değerlendirildi. Bulgular: Çalışmaya 72 kadın (%60), 48 erkek (%40) olmak üzere toplam 120 kişi dahil edildi. Katılımcıların yaş ortalaması 54.3±6.7 yıl idi. Hastaların %65.8'inde (79 kişi) diyabet dışı eşlik eden komorbid hastalık mevcutken %34.2'sinin (41 kişi) ek hastalığı bulunmamaktaydı. Katılımcıların %77.5'inde (93 kişi) sadece oral antidiyabetik kullanımı mevcutken, %22,5'i (27 kişi) kan şekeri regülasyonu için hem oral antidiyabetik hem de insülin kullanmaktaydı. Katılımcılar COVID-19 enfeksiyonu geçirmemiş, ayakta COVID-19 enfeksiyonu geçirmiş ve yatarak COVID-19 enfeksiyonu geçirmiş olarak eşit sayıda sınıflanmış olup her grup 40 kişi idi. Katılımcıların Montreal skoru değerlendirildiğinde Montreal skoru<21 olup bilişsel fonksiyonu bozuk olan hasta oranı %55 (66 kişi) iken ≥21 olan %45 (54 kişi) idi. Montreal skoru ortalama toplam puanı 18.6±3.9 idi. Çalışmamıza katılan Montreal skoru<21 ve ≥21 olan gruplar arasında hastaların yaşı ve kullanılan ilaç dağılımı anlamlı (p>0.05) farklılık göstermemiştir. Montreal skoru ≥21 olan grupta TG değeri, diyabete ek olarak kronik hastalık varlığı ve COVID-19 enfeksiyonu geçirme oranı Montreal skoru<21 olan gruptan anlamlı (p<0.05) olarak daha düşük iken kreatinin değeri ve erkek hastaların oranı Montreal skoru<21 olan gruptan anlamlı (p<0.05) olarak daha yüksektir. Çalışmamıza katılan Montreal skoru<21 ve ≥21 olan gruplar arasında HbA1c, glukoz, üre, AST, ALT, total protein, albümin, B12, folat, total kolesterol, LDL, HDL, WBC, HB, PLT, MCV, nötrofil değeri açısından anlamlı (p>0.05) farklılık gözlenmemiştir. Çalışmamıza katılan hastalar COVID-19 enfeksiyonu geçirme durumuna göre değerlendirildiğinde COVID-19 enfeksiyonu geçirmeyen, ayakta ve yatarak COVID-19 enfeksiyonu geçiren gruplar arasında hastaların yaşı ve cinsiyet dağılımı anlamlı (p>0.05) farklılık göstermemiştir. COVID-19 enfeksiyonu geçiren gruplarda diyabete ek kronik hastalık varlığı ve oral antidiyabetik+insülin kullanım oranı COVID-19 enfeksiyonu geçirmeyen gruptan anlamlı (p<0.05) olarak daha yüksektir. Hospitalizasyon drumuna göre covid geçiren gruplar arasında ise anlamlı (p>0.05) farklılık saptanmamıştır. Çalışmamıza katılan hastaların COVID-19 enfeksiyonu geçirme durumuna göre laboratuvar parametrelerine göre değerlendirildiğinde yatarak COVID-19 enfeksiyonu geçiren gruplarda HbA1c değeri diğer gruplardan anlamlı (p<0.05) olarak daha yüksektir. Ayakta COVID-19 enfeksiyonu geçiren grupta HbA1c değeri COVID-19 enfeksiyonu geçirmeyen gruptan anlamlı (p<0.05) olarak daha yüksektir. Yatarak COVID-19 enfeksiyonu geçiren grupta glukoz ve TG değeri COVID-19 enfeksiyonu geçirmeyen gruptan anlamlı (p<0.05) olarak daha yüksektir. Ayakta COVID-19 enfeksiyonu geçiren grup ile diğer gruplar arasında glukoz ve TG değeri anlamlı (p>0.05) farklılık göstermemiştir. COVID-19 enfeksiyonu geçirmeyen, ayakta ve yatarak COVID-19 enfeksiyonu geçiren gruplar arasında HB, PLT, MCV, nötrofil, üre, kreatin, ALT, total protein, albümin, B12, folat, total kolesterol, LDL, HDL değeri anlamlı (p>0.05) farklılık göstermemiştir. Ayakta COVID-19 enfeksiyonu geçiren grupta AST değeri, diğer gruplardan anlamlı (p<0.05) olarak daha yüksektir. Ayakta COVID-19 enfeksiyonu geçiren grupta WBC değeri COVID-19 enfeksiyonu geçirmeyen gruptan anlamlı (p<0.05) olarak daha yüksektir. COVID-19 enfeksiyonu geçirmeyen ve yatarak COVID-19 enfeksiyonu geçiren gruplar arasında AST ve WBC değeri açısından anlamlı (p>0.05) farklılık saptanmamıştır. Katılımcılar montreal skoru parametrelerine göre incelendiğinde COVID-19 enfeksiyonu geçirmeyen, ayakta ve yatarak COVID-19 enfeksiyonu geçiren gruplar arasında Montreal skoru bellek, görsel-uzaysal beceriler puanı, yer-zaman oryantasyonu, dil puanı anlamlı (p>0.05) farklılık göstermemiştir. COVID-19 enfeksiyonu geçirmeyen grupta Montreal skoru yönetici işlevler ve dikkat puanı ile Montreal skoru toplam puanı COVID-19 enfeksiyonu geçiren gruplardan anlamlı (p<0.05) olarak daha yüksektir. Ayakta COVID-19 enfeksiyonu geçiren grupta Montreal skoru yönetici işlevler ve dikkat puanı ile Montreal skoru toplam puanı yatarak COVID-19 enfeksiyonu geçiren gruptan anlamlı (p<0.05) olarak daha yüksektir. Sonuç: Çalışmamızda Tip 2 DM tanılı hastalarda covid geçirme durumuna göre ayrılan gruplar ile Montreal bilişsel fonksiyon skoru düzeyleri arasında istatistiksel açıdan anlamlı ilişki saptanmıştır. Bu veriler ışığında çalışmamız; Tip 2 DM tanılı hastalarda COVID-19 enfeksiyonu geçirmenin ve özellikle hastanede yatarak geçirmenin bilişsel fonksiyonlar üzerinde olumsuz etkisinin olduğunu, özellikle de yönetici işlevler ve dikkatte azalmaya yol açtığını düşündürmektedir. Çalışmamızda Tip 2 diyabete ek olarak komorbidite varlığının ve kadın cinsiyetin uzun dönemde bilişsel fonksiyonlarda gerileme için daha duyarlı parametreler olduğu gözlemlenmiştir. Tip 2 DM tanılı hastalarda COVID-19 enfeksiyonu geçirenlerde geçirmeyenlere göre uzun dönem kan şekeri regülasyonunun bozuk olduğu, regülasyon için kombine ilaç kullanımı ihtiyacının daha fazla olduğu, ek komorbid hastalık sayısının daha sık olduğu ve HbA1c değerinin daha yüksek olduğu saptanmıştır. Ancak COVID-19 enfeksiyonu nedenli hospitalizasyon durumunun ilaç kullanımını etkilemediği saptanırken yatarak geçiren hastalarda daha yüksek HbA1c ve glukoz değerleri saptanmıştır. Çalışmamız Tip 2 diyabet tanılı COVID-19 enfeksiyonu geçiren ve özellikle bu sebeple hastane yatışı olan popülasyonda gelişebilecek bilişsel sorunlara dikkat çekmek ve bu potansiyel morbiditeyi, gelecekte doğurabileceği ciddi sağlık yükünü de öngörerek erken tanımak açısından önem arzetmektedir. Bu hastaları yakın takip ve tarama programlarına alarak erken teşhisle ileri komplikasyonların önlenmesine imkan tanınması ve mevcut sağlık politikalarının bu yolda şekillendirilmesine katkı sunulması olasılığı umut vericidir.

ABSTRACT Introduction: Diabetes Mellitus (DM) and coronavirus infection (COVID 19) are diseases with long-term consequences, high mortality and morbidity rates, and care costs. Diabetes Mellitus is also a risk factor for COVID-19 infection alone itself. There are many studies in the literature examining the relationship between diabetes and cognitive functions, as well as studies examining the relationship between COVID-19 infection and cognitive functions, but national study data showing the effects on cognitive functions in Type 2 DM patients who had had coronavirus infection are limited. Therefore, in our study, we aimed to investigate the effects of COVID-19 infection on cognitive functions of patients with Type 2 DM. Materials and Methods: 120 Type 2 DM patients that met our study criteria, applied to the internal medicine-diabetes outpatient clinic of Istanbul Training and Research Hospital between September 27, 2024 and November 24, 2024, and were selected randomly by considering inclusion and exclusion criteria were included for our study. All cases included in the study were divided into three equal groups as patients who did not have coronavirus infection, patients who had coronavirus infection but were not hospitalized, and patients who had coronavirus infection and had a history of hospitalization by questioning their anamnesis and past data. All Type 2 diabetic patients who had COVID-19 infection, had a positive COVID-19 PCR test and/or had computed tomography involvement consistent with typical COVID-19 infection were examined and recorded. Data of the patients were accessed from patient files, the hospital information management system, and the MEDULA system of the Ministry of Health. At the time of application, the Montreal Cognitive Assessment Scale (MoCA) test was applied to all cases in a face-to-face environment under the supervision of a physician, and their score was calculated and recorded. The Turkish version (MoCA-TR) of the Montreal Cognitive Assessment Test, which was organized according to countries, was used. The cut-off value was accepted as 21, based on the acceptance of The Psychiatric Association of Türkiye (PAT) that a total score of 21 and above in MoCA-TR indicates that the participant is within normal limits. In addition, according to the parameters used in the Montreal Cognitive Assessment Scale, the patients' scores from memory, attention, language, visual spatial skills, space-time orientation, and executive functions groups were calculated and recorded separately. Age, gender, chronic diseases, medications used, laboratory tests (urea, creatinine, hemogram, fasting blood sugar, HbA1c, T. cholesterol, TG, LDL, HDL, AST, ALT, Total protein, Albumin, Vitamin B12, Folic acid levels) of the groups at the time of application were recorded through the system. The groups determined according to COVID-19 infection status and Montreal Cognitive Assessment Scale score values were compared first within the group and then with each other in terms of the determined parameters. The results were evaluated with the SPSS 28.0 statistical analysis method. Results: A total of 120 people consisting of 72 females (60%) and 48 males (40%) were included in the study. The average age of the participants was 54.3±6.7 years. While 65.8% (79 people) of the patients had a comorbid disease other than diabetes, 34.2% (41 people) had no additional disease. While 77.5% (93 people) of the participants were using only oral antidiabetics, 22.5% (27 people) were using both oral antidiabetics and insulin. Participants who did not have COVID-19 infection, had COVID-19 infection without being hospitalized, and had COVID-19 infection in a hospitalized manner were classified equally and were 33.3% (40 people) for each class. When the Montreal score of the participants was evaluated, 55% (66 people) had a Montreal score <21 and impaired cognitive function, while 45% (54 people) had a score ≥21. The mean Montreal score was 18.6±3.9. There was no significant difference (p>0.05) in the age of the patients and the distribution of the drugs used between the groups with a Montreal score of <21 and ≥21 in our study. In the group with a Montreal score of ≥21, the TG value, the presence of chronic disease in addition to diabetes and the rate of COVID-19 infection were significantly lower (p<0.05) than the group with a Montreal score of <21, while the creatinine value and the proportion of male patients were significantly higher (p<0.05) than the group with a Montreal score of <21. No significant difference (p>0.05) was observed in HbA1c, glucose, urea, AST, ALT, total protein, albumin, B12, folate, total cholesterol, LDL, HDL, WBC, HB, PLT, MCV, and neutrophil values between the groups with Montreal score <21 and ≥21 that participated in our study. When the patients participating in our study were evaluated according to their COVID-19 infection status, the age and gender distribution of the patients did not show any significant difference (p>0.05) between the groups that did not have COVID-19 infection,had COVID-19 infection without being hospitalized and had COVID-19 infection in a hospitalized manner. In the groups that had COVID-19 infection, the presence of chronic disease in addition to diabetes and the rate of oral antidiabetic + insulin use were significantly higher (p<0.05) than the group that did not have COVID-19 infection. No significant difference (p>0.05) was found between the groups that had COVID-19 infection according to hospitalization status. When the patients participating in our study were evaluated according to their COVID-19 infection history and laboratory parameters, the HbA1c value was significantly (p<0.05) higher in the group that had COVID-19 infection while hospitalized than in the other group. The HbA1c value in the group that had COVID-19 infection without being hospitalized was significantly higher than the group that did not have COVID-19 infection (p<0.05). Glucose and TG values were significantly higher in the group that had COVID-19 infection without being hospitalized than the group that did not have COVID-19 infection (p<0.05). Glucose and TG values did not differ significantly (p>0.05) between the group with COVID-19 infection outpatients and the other groups. There was no significant difference (p>0.05) in HB, PLT, MCV, neutrophil, urea, creatine, ALT, total protein, albumin, B12, folate, total cholesterol, LDL, HDL values between the groups that did not have COVID-19 infection, had COVID-19 infection in the outpatient setting, and had COVID-19 infection in the inpatient setting. The AST value in the group that had COVID-19 infection without being hospitalized was significantly higher (p<0.05) than the other groups. The WBC value in the group that had COVID-19 infection without being hospitalized was significantly (p<0.05) higher than the group without COVID-19 infection. No significant difference (p>0.05) was found between the groups that did not have COVID-19 infection and the group that had COVID-19 infection while hospitalized in terms of AST and WBC values. When the participants were examined according to the Montreal score parameters, there was no significant difference (p>0.05) in the Montreal score memory, visual-spatial skills score, place-time orientation, and language score between the groups that did not have COVID-19 infection, had COVID-19 infection in the outpatient setting and had COVID-19 infection in the inpatient setting. In the group that did not have COVID-19 infection, the Montreal score's executive functions and attention score and the total Montreal score were significantly (p<0.05) higher than the groups that had COVID-19 infection. In the group that had COVID-19 infection without being hospitalized, the Montreal score's executive functions and attention scores and the total Montreal score were significantly (p<0.05) higher than the group that had COVID-19 infection in a hospitalized manner. Conclusion: In our study, a statistically significant relationship was found between the groups of patients with Type 2 DM that were classified according to their COVID-19 infection status and the Montreal cognitive function score levels. In the light of these data, our study suggests that for patients with Type 2 DM, having COVID-19 infection and especially having it in a hospitalized manner has a negative effect on cognitive functions, and in particular, a decrease in executive functions and attention was detected. In our study, we observed that presence of comorbidities and female gender, in addition to Type 2 diabetes were more sensitive to decline in cognitive functions in long term. Among the patients diagnosed with Type 2 diabetes, those who had a history of COVID-19 infection detected as impaired long term blood glucose regulation to those who did not contract the infection. They also required the use of combination therapy for regulation more, had a higher number of comorbid diseases, and showed elevated HbA1c levels. However, while it was determined that the status of hospitalization that occurs due to COVID-19 infection did not affect medication use, higher HBA1C and glucose values were detected in patients that had COVID-19 infection in a hospitalized manner. Our study is important for drawing attention to the cognitive problems that may develop in the population with type 2 diabetes that had COVID-19 infection and especially those hospitalized for this reason, and additionally for early recognition of this potential morbidity by predicting the serious health burden it may cause in the future. To be given the opportunity of the prevention of further complications through inclusion of these patients in close follow-up and screening programs, and the probability of contribution of the shaping of current health policies in this way are promising.
