Özet
Bu çalışmanın amacı; spastik diplejik serebral palsili (SP) çocukların alt ekstremitelerine uygulanan ergometrik fonksiyonel elektrik stimülasyonunun (E-FES), çocukların yürüyüş parametreleri ve günlük yaşam aktivitelerine olan etkilerini araştırmaktı. Çalışmaya Kaba Motor Fonksiyon Sınıflama Sistemi (GMFCS)'ne göre seviyeleri I, II olan, yaşları 6-16 yıl arasında değişen 20 spastik diplejik SP'li çocuk dahil edildi. Çocuklar rasgele örneklem yöntemi ile E-FES grubu (n=10) ve kontrol grubu (n=10) olmak üzere iki gruba ayrıldı. Her iki gruptaki çocuklar aynı fizyoterapist tarafından 8 hafta boyunca haftada 3 gün 45'er dk olacak şekilde fizyoterapi programına alındı. E-FES grubuna; ek olarak 8 hafta boyunca haftada 3 gün 30'ar dk E-FES uygulandı. Tedavi öncesi ve tedavi sonrası değerlendirmeler sağlığın, yeti yitiminin ve fonksiyonelliğin uluslar arası sınıflandırılması (ICF) ile ilişkili olarak ele alıdı. ICF çercevesinde vücut yapı ve fonksiyonları ile ilişkili olarak; fonksiyonel kas kuvveti, alt ekstremite kas tonusu (Modifiye Ashworth Skalası; MAS) ve eklem hareket limitasyonu, enerji tüketimi (VO2max) değerlendirildi. Aktivite kısıtlılıkları ve katılım ile ilişkili olarak ise kaba motor fonksiyonlar (Kaba Motor Fonksiyon Ölçütü; GMFM-88), denge parametreleri (Pediatrik Denge Ölçeği; PDÖ, Zamanlı Ayağa Kalk Otur Testi; TUG), yürüyüşün zaman mesafe özellikleri, yürüyüşün sagital plan kinematik değerleri (üç boyutlu yürüme analizi), günlük yaşam aktivitelerindeki bağımsızlık düzeyi (Pediatrik Özürlülük Değerlendirmesi Anketi; PEDI) ve yaşam kalite düzeyi (Çocuk Sağlık Anketi; CHQ-PF50) değerlendirildi. Tedavi öncesinde her iki grubun ölçümleri arasında anlamlı fark yoktu (p>0,05). Sekiz haftalık eğitim sonrasında kontrol grubundaki ölçüm parametrelerinde tedavi öncesine göre anlamlı bir fark yoktu (p>0,05). Fizyoterapiye ek olarak E-FES uygulanan grupta, ICF'in vücut yapı ve fonksiyon boyutunda; fonksiyonel kas kuvvetinde artış, ve enerji tüketiminde azalma saptandı (p<0,05). Kas tonusu ve eklem hareket limitasyonlarında fark yoktu (p>0,05). Katılım ve aktivite boyutunda ise kaba motor fonksiyonlarda, dengede, fonksiyonel bağımsızlık düzeyi ve yaşam kalite düzeylerinde istatistiksel olarak anlamlı kazanımlar saptanmasına rağmen (p<0,05), yürüyüşün zaman mesafe özelliklerinde ve sagital plan kinematik değerlerinde istatistiksel olarak anlamlı fark yoktu (p>0,05). Sonuç olarak fizyoterapiye ek olarak uygulanan E-FES, çocukların vücut yapı ve fonksiyonları ile aktivite ve katılım düzeylerine olumlu katkı sağladı. Bu nedenle E-FES'in, spastik diplejik SP'li çocuklarda fizyoterapi programına ek olarak uygulanmasının yararlı olacağı düşünüldü.

The aim of this study was to investigate the effects of cycling functional electrical stimulation (E-FES) applied to the lower extremities of children with spastic diplegic cerebral palsy (CP) on the gait parameters and daily living activities. A total of 20 children aged 6 to 16 years with spastic diplegic CP level I and II according to the Gross Motor Function Classification System (GMFCS) were included in the study. The children were divided into two groups with the randomized sampling method as the E -FES group (n=10) and the control group (n=10). Children in both groups were included in a physiotherapy program by the same physiotherapist for 45 minutes a day, 3 days a week for 8 weeks. The E-FES group additionally received E-FES for 30 minutes a day, 3 days a week for 8 weeks. Pre-treatment and post-treatment evaluations were performed with the International Classification of Disability and Functionality (ICF). Functional mass strength, lower extremity muscle tonus (Modified Ashworth Scale; MAS), joint range of motion limitation, and energy consumption (VO2max) were evaluated related to the body structure and functions within the framework of ICF. Gross motor functions (Gross Motor Function Measure; GMFM), balance parameters (Pediatric Balance Scale; PBS, Time up Go; TUG), spatiotemporal characteristics of gait, sagittal plan kinematic gait values (Three Dimensional Gait Analysis), independence level for daily activities (Pediatric Evaluation of Disability Inventory; PEDI) and the quality of life level (The Child Health Questionnaire; CHQ-PF50) were evaluated regarding activity limitations and participation. No significant difference was present between the measurements of the two groups before the treatment (p>0,05). No difference was present between the measurement parameters of the control group compared to pre-treatment after eight weeks of training (p>0,05). The group where E-FES was added to physiotherapy showed an increase in functional mass strength and decrease in energy consumption (p<0,05) in the body structure and function dimension of ICF. There was no difference regarding muscle tonus and joint range of motion limitation (p>0,05). Although a statistically significant improvement was found in the participation and activity dimensions with increased gross motor function, balance, functional independence level and quality of life (p<0,05), no statistically significant difference was present in the spatiotemporal characteristics and sagital plan kinematic values of gait (p>0,05). In conclusion, E-FES added to physiotherapy made a positive contribution to the body structure and functions of the children and increased their activity and participation levels. The use of E-FES in addition to the physiotherapy protocol should be useful in children with spastic diplegic CP.
