Özet
Amaç: Sık üst solunum yolu enfeksiyonu geçirme öyküsü veren ve Paradise kriterlerine göre tanısı kesinleşen çocukların prospektif olarak çinko ve demir eksikliği açısından incelenmesi amaçlandı. Gereç ve Yöntemler: İstanbul Üniversitesi Çocuk Sağlığı ve Hastalıkları Anabilim Dalı Genel Pediatri Bilim Dalı Polikliniğine sık üst solunum yolu enfeksiyonu geçirme şikâyetiyle başvuran 2-18 yaş arası hastalar çalışmaya alındı. Sık üst solunum yolu enfeksiyonu geçiren çocuk tanımı olarak Paradise kriterleri doğrultusunda yılda 7 veya daha fazla tonsillofarenjit enfeksiyonu geçirilmesi veya geçen son 2 yıl içerisinde her yıl en az 5 tonsillofarenjit enfeksiyonu geçirilmesi veya geçen 3 yıl içerisinde her yıl en az 3 tonsillofarenjit enfeksiyonu geçirilmesi olarak kabul edildi. Hastalar sık üst solunum yolu enfeksiyonu geçirenler ve kontrol grubu olmak üzere ikiye ayrıldı. Ayrıca sık üst solunum yolu enfeksiyonu geçiren grup, hastalık geçirme sıklıklarına göre 3 alt gruba daha ayrılarak çalışma yürütüldü. 1. grup; yılda ≥7 ÜSYE geçiren çocuklar, 2. grup; 2 yılda ≥5 ÜSYE/yıl geçiren çocuklar, 3. grup; 3 yılda ≥3 ÜSYE/yıl geçiren çocuklar olarak tanımlandı. Yaş, boy, tartı değerleri ve persantilleri saptandı. Günlük diyet öyküleri çinko ve demir içeriği açısından sorgulanarak diyet analizi Beslenme Bilgi Sistemi 6 (BEBİS6) programı kullanılarak yapıldı. Hastalara enfeksiyonsuz dönemlerinde gelmeleri söylenerek, semptomsuz ve ilaç tedavisi almadıkları dönemlerde hastaların ve benzer yaş ve risk faktörlerini içeren kontrol grubunun tam kan sayımı, serum demir düzeyi, total demir bağlama kapasitesi, serum ferritin düzeyleri ve serum çinko düzeyleri araştırıldı. Çalışma sırasında, ek başka bir hastalığı tespit edilenler çalışmadan çıkarıldı. Tam kan sayımı İ.Ü. İTF Çocuk Sağlığı ve Hastalıkları Biyokimya Laboratuvarında Abbottt Cell-Dyn 3700 adlı cihazla MAPPS lazer ölçüm sistemiyle, serum demir düzeyi ve total demir bağlama kapasitesi ölçümü İ.Ü. Çocuk Sağlığı ve Hastalıkları Biyokimya Laboratuvarında Roche Cobas Integra 800 cihazında spektrofotometrik (Guanidine/FerroZine) yöntemiyle, serum ferrritin düzeyi İ.Ü. Klinik Biyokimya Laboratuvarında Roche Cobas Modular Analytics E170 cihazında elektrokemilüminesans immünolojik test yöntemiyle ve serum çinko düzeyi İ.Ü. Çocuk Kliniği Eser Element Laboratuvarında Varian marka atomik absorbsiyon spektrofotometresi ile ölçüldü. Bunun için sekiz saatlik açlığı takiben tam kan sayımı için bir adet 2 cc K3 EDTA'lı tüpe, serum demir düzeyi ve total demir bağlama kapasitesi için bir adet 2 cc Na Sitrat'lı tüpe ve serum çinko düzeyi içinde yine bir adet 2 cc eser element analizine uygun Becton-Dickinson XIV tüpüne venöz kan örnekleri alındı. Ayrıca sık ÜSYE veya diğer nedenlerle önceden bakılan immünglobulin düzeyleri de değerlendirildi. Elde edilen veriler SPSS 21,0 programı yardımıyla incelendi. İstatistiksel olarak p<0.05 anlamlı olarak kabul edildi. Bulgular: Çalışmaya 100 hasta ve 100 kontrol ile başlandı. Çalışma ve kontrol gruplarından birer hasta β-talasemi minör ön tanısıyla, kontrol grubundan da 1 hasta AAA, 1 hasta hafif IgA eksikliği ön tanısıyla, 9 hasta da venöz kan örneklerinde hemoliz olması nedeniyle çalışma dışı kaldılar. Sık ÜSYE geçiren grupta 99, kontrol grubunda 88 çocuk yer aldı. Alt gruplardan 1.grupta 57, 2. grupta 25 ve 3. grupta 17 kişi vardı. Sık ÜSYE grubunun yaş ortalaması (5,35±2,14 yıl) kontrol grubuna (6,55±2,92 yıl) göre daha düşük saptandı. Buna bağlı olarak da sık ÜSYE grubunun ortalama boy ve kilo değerleri de daha düşük bulundu. Alt gruplardan 1. grubun 3. ve kontrol grubuna göre yaş, boy ve kilo değerleri düşüktü. Sık ÜSYE ve kontrol grubu arasında ve alt grupların karşılaştırılmasında gomez, relatif tartı ve yaşa göre boy değerleri arasında fark saptanmadı. En küçük yaş 1. gruptaydı (4,8±1,91 yıl). Düşük yaşlarda daha fazla enfeksiyon geçirme şikayetinin olduğu görüldü. Prematürite (p=0,029) ve aynı yatak odasını paylaşan kişi sayısı (p=0,035) sık ÜSYE açısından risk faktörü olarak tespit edildi. Alt gruplar arasında farklılık saptanmadı. Anne sütü ile beslenmede hedeflenen 6 ay sadece anne sütü verilmesi ve 2 yaşına kadar anne sütüne devam edilmesi hedeflerine ulaşılmadığı, gruplar arasında fark olmamakla beraber sadece anne sütü verilmesi süresinin yaklaşık 4,5 ay, toplam anne sütü ile beslenme süresinin ortalama 15,5 ay civarlarında olduğu görüldü. Beslenme analizlerine bakıldığında sık ÜSYE geçiren çocukların kontrol grubuna oranla daha az çinko (5,64±1,86mg ve 6,8±2,13 mg), protein (%74,23±23,1 ve %82,06±21,55) ve yağ (%76,94±28,81 ve %98,58±81,53) tükettikleri görüldü (p=0,001, 0,018 ve 0,014). Alt gruplardan 1. ve 2. grubun çinko tüketim miktarları (5,4±1,67 ve 5,51±1,64 mg) ile kontrol grubu (6,8±2,13mg) arasında istatistiksel olarak anlamlı fark saptandı (p=0,0001 ve 0,022). Ayrıca 1. grubun protein tüketim yüzdesi (%70,05±20,01) kontrol grubundan (%82,06±21,55) ve 3. gruptan (%88,28±26,2) istatistiksel olarak anlamlı derece düşüktü (p=0,008 ve 0,016). İştahsızlık (%71 p=0,0001), solukluk (%33 p=0,002), angüler çeliyit (%24 p=0,023) ve ishal (%11 p=0,005) oranları sık ÜSYE geçiren grupta kontrol grubuna göre (sırasıyla %23,XV %14,%11 ve %1) daha yüksekti. Alt gruplara bakıldığında her dört bulgunun görülme oranları 1. grupta kontrol grubuna göre daha yüksekti (%77, %42, %30 ve %14)(p<0.05). Ayrıca iştahsızlık 2. (%64) ve 3. grupta (%59) da kontrol grubuna göre daha yüksek oranda saptandı. Sık ÜSYE geçirenlerde en sık şikâyetin iştahsızlık (%71) olduğu görüldü. Serum çinko (72±8 ve 84±9 µg/dL, p=0,0001), serum demir (70,52±28,18 ve 85,35±35,15 µg/dL,p=0,002), hemoglobin (11,98±0,78 ve 12,38±0,69 g/dL, p=0,0001), hematokrit (%35,52±2,26 ve 36,87±2,01, p=0,0001), MCV (78,37±3,83 ve 80,27±4,34 fL, p=0,002), MCH (26,39±1,45 ve 27,03±1,65 pg, p=0,005), MCHC (33,04±1,50 ve 34,08±2,02 g/dL, p=0,006) ve IgG (926,99±214,5 ve 1035,54±217,96 mg/dL, p=0,001) ortalama değerlerinin sık ÜSYE grubunda kontrol grubuna göre istatistiksel olarak anlamlı derecede düşük olduğu görüldü. Sık ÜSYE alt gruplarından 1. ve 2. grubun (69±7 ve 75±8 µg/dL) kontrol grubuna ( p=0,0001) ayrıca 1. grubun 2. ve 3. gruba (79±6 µg/dL ) göre de anlamlı şekilde çinko düzeyleri düşük saptandı (p=0,011 ve 0,0001). Demir ortalama değerleri içinse yalnızca 1. grupta (69,19±30,11 µg/dL) kontrol grubuna göre anlamlı düzeyde demir düşüklüğü saptandı (p=0,017). Aynı şekilde yalnızca 1. grupta kontrol grubuna göre hemoglobin (11,86±0,88 g/dL), hematokrit (%35,3±2,52), MCV (78,19±4,36 fL), MCH (26,27±1,59g/dL), MCHC (32,79±1,48 g/dL) ve IgG (895,08±214,17 mg/dL) ortalamaları (normal sınırlar içinde olmasına karşın) anlamlı olarak düşüktü (p<0.05). 1. grupta eksiklikler en belirgindi. Sık ÜSYE ile kontrol grubu arasında anlamlı farklılık çıkmasının esas sorumlu görüldüğü grup 1. grup olarak belirlendi. Toplamda çinko eksikliği %18,2 olarak tespit edildi. Tüm gruplarda günlük tüketilen çinko ihtiyacı karşılama yüzdesinin %100'ün üstünde olmasına rağmen çocuklarda saptanan çinko eksikliği sıklığı, diyetlerdeki çinko kaynağının hayvansal veya bitkisel olmasının, çinko biyoyararlanımının ve diyet analizlerinin ülkemiz şartlarına göre daha dikkatli yapılmasının önemini gösterdi. Çinko eksikliği oranları sık ÜSYE grubunda (%33,3), kontrol grubuna (%1,1) göre belirgin daha yüksekti (p=0,0001). Alt gruplara bakıldığında 1. grupta (%49,1) çok daha belirgin olmak üzere 1. ve 2. grupta (%16) çinko eksikliği görülme oranı, kontrol grubuna göre anlamlı şekilde yüksekti (p=0,0001 ve 0,001). Ayrıca 1. grupta çinko eksikliği görülme oranı, 2. ve 3. gruplara (%5,9) göre de anlamlı şekilde yüksekti (p=0,0046 ve 0,0013). 1. grupta hem kontrol hem de 2. ve 3. gruplara göre, 2. grupta ise kontrol grubuna göre daha sık çinko eksikliği saptandı.XVI Toplamda anemi görülme oranı %16 olarak tesbit edildi. Sık ÜSYE grubunda anemi varlığı (%21,2), kontrol grubundan (%10,2) yüksek bulunsa da istatistiksel olarak anlamlı değildi. (p=0,065). Alt gruplara bakıldığında 1. grupta anemi (%29,8) görülme oranı 2. ve kontrol grubuna göre (%8 ve 11,8) yüksekti (p =0,003 ve 0,031). Diğer gruplar arasında istatistiksel olarak anlamlı fark saptanmadı (p>0,05). Toplamda demir eksikliği %9,1 olarak tespit edildi. Demir eksikliği açısından sık ÜSYE grubu (% 7,1) ile kontrol grubu (%11,4) arasında ve alt gruplar (1. grup %5,3, 2. grup %12 ve 3. grup %5,8) arasında fark saptanmadı (p>0,05). Demir eksikliği olanlar içinde çinko eksikliği birlikteliği saptanmadı. Toplamda demir eksikliği anemisi %7 olarak saptandı. Sık ÜSYE grubu (%9,1) ile kontrol grubu (%4,5) arasında demir eksikliği anemisi görülme oranları arasında istatistiksel olarak anlamlı farklılık bulunmadı (p>0,05). Alt gruplara bakıldığında sık ÜSYE grubundaki tüm demir eksikliği anemisi saptanan hastaların 1. grupta yer aldığı ve dolayısıyla 1. grupta demir eksikliği anemisi (%15,8) görülme oranının 2., 3. (%0,0) ve kontrol grubuna göre istatistiksel olarak anlamlı düzeyde daha fazla olduğu görüldü (p=0,035, 0,001 ve 0,020). Demir eksikliği anemisi olup çinko eksikliği de olan 5 hastanın tümü sık ÜSYE grubundaydı. Sık ÜSYE grubunda demir eksikliği anemisi olanlar içinde çinko eksikliği görülme oranı (%5,1), kontrol grubundan (%0,0) daha yüksek saptanmasına karşın bu farklılık istatistiksel olarak anlamlı bulunmadı. (p>0,05). Ancak alt gruplar açısından bakıldığında bu 5 hastanın tümünün 1. grupta yer aldıkları görüldü. Bundan dolayı demir eksikliği anemisi olup çinko eksikliği de görülmesi durumu 1. grupta (%8,8) kontrol grubuna (%0,0) göre istatistiksel olarak anlamlı derecede yüksek saptandı (p=0,0046). Tüm gruplar içinde toplamda demir eksikliği veya demir eksikliği anemisi görülmesi %16 olarak tespit edildi. Sık ÜSYE ve kontrol grupları arasında demir eksikliği veya demir eksikliği anemisi görülme oranları (%16,2 ve 15,9) arasında istatistiksel olarak anlamlı fark saptanmadı (p>0,05). Demir eksikliği veya demir eksikliği anemisi olup çinko düzeyi normal veya düşük saptananlar açısından da sık ÜSYE grubu ile kontrol grubu arasında istatistiksel olarak anlamlı farklılık saptanmadı (p>0,05). Alt gruplara bakıldığında demir eksikliği veya demir eksikliği anemisi görülmesi açısından istatistiksel olarak anlamlı fark saptanmadı. (p>0,05) Demir eksikliği veya demir eksikliği anemisi ile birlikte çinko eksikliği görülmesi XVII durumları 1. grupta (%8,8) kontrol grubuna (%0,0) göre istatistiksel olarak anlamlı derecede yüksek bulundu (p=0,08). En sık enfeksiyonun görüldüğü 1. grupta DE/DEA ile çinko eksikliğinin birlikte görülmesinden dolayı DE/DEA ve çinko eksikliği birlikteliğinin enfeksiyon sıklığını artırdığı görüldü. Sık ÜSYE grubunda çinko eksikliği görülme riskinin kontrol grubuna göre 43,5 kat fazla olduğu saptandı (p=0,002; %95 Cl: 5,80-326,28). Demir eksikliği veya DE/DEA ile çinko eksikliği birlikte görülmesi sık ÜSYE grubunda 10,3 kat fazla bulunsa da (p=0,1162; %95 Cl: 0,56-189,03) çinko eksikliği olmayan DE/DEA görülmesi durumunda herhangi bir risk artışından söz etmenin mümkün olmaması nedeniyle, esas etkenin çinko eksikliği olduğu düşünüldü. ÜSYE sıklığına çinko eksikliği, anemi, demir eksikliği ve demir eksikliği anemisinin etkisini değerlendirdiğimizde, çinko eksikliğinin tek başına ÜSYE sıklığını 40,4 kat arttırıcı etkisi olduğu görüldü. Anemi, demir eksikliği ve demir eksikliği anemisinin modele etkisi istatistiksel olarak anlamlı saptanmaması (p>0,05) yukardaki düşüncemizi destekler nitelikte esas faktörün çinko eksikliği olduğunu düşündürdü. Diyet ile alınan günlük çinko miktarı ve alınan proteinin günlük ihtiyacı karşılama yüzdesi ile serum çinko düzeyi arasında istatistiksel olarak anlamlı pozitif korelasyon saptandı (p=0,0001 ve 0,0001; r=0,351 ve 0,295). Ferritin ile günlük alınan demir miktarı ve alınan demirin günlük ihtiyacı karşılama yüzdesi arasında anlamlı pozitif korelasyon saptandı (p=0,003 ve 0,001; r=0,213 ve 0,233). Hemoglobin ve hematokrit düzeyleri ile beslenme ile alınan demir (p=0,04 ve 0,01; r=0,212 ve 0,231), çinko (p=0,01 ve 0,0001; r=0,251 ve 0,303), karbonhidrat yüzdesi (p=0,0001ve 0,0001; r=0,270 ve 0,271), ve protein yüzdesi (p=0,03 ve 0,0001; r=0,212 ve 0,273) arasında ayrıca hematokrit ile demir yüzdesi arasında (p=0,006 r=0,201) anlamlı pozitif korelasyon saptandı. MCV ile alınan çinko ve demir miktarları arasında anlamlı pozitif korelasyon görülse de (p=0,001 ve 0,02; r=0,241 ve 0,162) anlamlılık çinko düzeyi ile MCV arasında daha belirgindi. Ayrıca IgG düzeyi ile diyetteki demir miktarı (p=0,001 r=0,261),çinko miktarı (p=0,0001 r=0,345), protein yüzdesi (p=0,0001 r=0,301), yağ ( p=0,003 r=0,233)ve karbonhidrat (p=0,0001 r=0,330) alım yüzdeleri arasında da anlamlı pozitif korelasyon saptandı. Buna ek olarak günlük alınan çinko miktarı ile IgA arasında da anlamlı pozitif korelasyon olduğu görüldü (p=0,002 r=0,239).XVIII Sonuç: Çinko eksikliği çocukluk çağında ciddi bir eser element eksikliğidir. Çinko eksikliği üst solunum yolu enfeksiyonlarına yatkınlığı artırmaktadır. Ülkemiz, demir eksikliği anemisinden daha yüksek oranda çinko eksikliği açısından risk altındadır ve demir eksikliği olan çocuklar çinko eksikliği açısından da mutlaka araştırılmalıdır. Sık ÜSYE geçiren çocuklarda çinko ve demir düzeylerinin normal toplumdan düşük olduğu (çinko daha belirgin olmak üzere), çinko ve demir düzeylerinin özellikle enfeksiyon sıklığının en çok olduğu grupta, belirgin olarak daha düşük olduğu görülmüştür. Demir ve özellikle çinko eksikliği iştahsızlığa neden olmakta bu da çocuğu beslenme açısında bir kısır döngü içerisine sokarak, eksikliklerin daha da artmasına yol açmaktadır. Beslenmenin bağışıklık sistemi üzerine önemli etkileri bulunmaktadır. Sık ÜSYE geçiren hastalar, gereksiz testlere tabi tutulmadan önce çinko başta olmak üzere eser element ve protein-enerji eksiklikleri açısından değerlendirilmelidir.

Objectives: Prospective examination of zinc and iron deficiency in patients with a history of recurrent upper respiratory tract infection (RURTI) confirmed in accordance with the Paradise criteria. Materials and Methods: The study covers patients between the ages of 2 to 18 that applied to the Istanbul University Istanbul Faculty of Medicine Department of Pediatrics for General Pediatrics with the complaint of recurrent upper respiratory tract infections. The diagnosis of RURTI in these children was defined in accordance with the Paradise criteria as having at least 7 or more tonsillopharyngitis infections per year, having at least 5 tonsillopharyngitis infections per year in the last 2 years or having at least 3 or more tonsillopharyngitis infections per year in the last 3 years. The patients were first categorized into two groups, those having recurrent upper respiratory tract infections and the control group. Second, the group having recurrent upper respiratory tract infections was separated into three sub-groups corresponding to the frequency of infections. Groups were defined as; first group ≥7 upper respiratory tract infections (URTI) per year, second group ≥5 URTI per year and the 3 rd group ≥3 URTI per year. Age, height, weight and percantiles were determined. Patterns of daily diet of the patients were questioned from the perspective of zinc and iron content and diet analyses was carried out using the Diet Information System 6 (BEBİS6) software. The patients were asked to return for a consultation during an infection free period, thus making it possible to to examine the complete blood count, serum iron level, total iron binding capacity, serum ferritine levels and serum zinc levels during this disease and treatment free period of both the patients and the control group having the similar age and risk factors. Patients diagnosed with additional infections were excluded from the study. Complete blood count was carried out at the Istanbul University Istanbul Faculty of Medicine, Biochemical Laboratory of Pediatrics with the Abbott Cell-Dyn 3700 device using the MAPSS laser measurement system, serum iron level and total iron binding capacity were measured at the Istanbul University Istanbul Faculty of Medicine Biochemical Laboratory of Pediatrics with the Roche Cobas Integra 800 device using the spectrophotometric (Guanidine/FerroZine) method, serum ferritine level was measured at the Istanbul University Istanbul Faculty of Medicine Clinical Biochemistry Laboratory using the Roche Cobas Modular Analytics E170 device with the electroluminescent immunological test method and the serum zinc level was measured at the Istanbul University Istanbul Faculty of XX Medicine Pediatric Clinic Trace Element Laboratory with the Varian device using the method of atomic absorption spectrophotometer. For these measurements venous blood samples were taken following an eight hours fasting period and placed in; one 2 cc K3 tube with EDTA for the complete blood count, one 2 cc tube with Na Citrate for the serum iron level and total iron binding capacity, and again one 2 cc with Becton-Dickinson tube which is befitting for trace element analysis for the serum zinc level. Previously viewed immunglobulin levels for RURTI or other reasons were also evaluated. The data collected was examined through the SPSS 21.0 software and was found to be statistically significant at p<0.05. Results: This study was initiated with 100 patients in the study group and 100 children in the control group. One child in both groups had β-talasemia minor and were excluded from the study. Additionally 1 patient with FMF, 1 patient with IgA deficiency and a total of 9 venous blood samples that were hemolized in the control group were all excluded from the study. The final count included 99 patients in the group having RURTI and 88 patients in the control group. In the second stage of the study the subgroups were divided as 57 patients in the 1st group, 25 patients in the 2nd group, and 17 patients in the 3rd group. The RURTI groups' mean avarage of age (5.35±2.14 year) was slightly lower than the control groups (6.55±2.92 year) thus resulting in a lower mean average of both height and weight. Among the subgroups the first groups mean age, height and weight were lower than those of third group and the control group. Even than the gomez score, relative score and lenght for age scores were indifferent. The lowest mean age was in the first group (4.8±1.91 year). The number of episodes for infections tended to be higher among younger ages. Prematurity (p=0.029) and the number of people sharing the same bedroom (p=0.035) are determined as risk factors from the perspective of having RURTI. No difference was found between subgroups. In total the goal for breastfeeding exclusively for six months and breastfeeding for at least 2 years were not accomplished, and an average of 4.5 months was achieved for breastfeeding exclusively and an average of 15.5 months was achieved for total months of breastfeeding. Dietary analyses revealed that children with RURTI consume less zinc (5.64±1.86mg and 6.8±2.13 mg), protein (74.23%±23.1 and 82.06%±21.55) and fat (76.94%±28.81 and 98,58%±81.53) in comparison to the control group (p=0.001, 0.018 ve 0.014). StatisticallyXXI significant differences were found fort he zinc intake in the first and second groups (5.4±1.67 and 5.51±1.64 mg) in comparison to the controls (6.8±2.13mg) (p=0.0001 and 0.022). The first groups protein intake percentage (70.05%±20.01) was statistically lower than the control group (82.06%±21.55) and the third group (88.28%±26.2) (p=0.008 ve 0.016). Loss of appetite (71%, p=0.0001), pallor (33%, p=0.002), anguler cheilitis (24%, p=0.023), diarrhea (11%, p=0.005) were much more common in the RURTI group than the control group (23%, 14%,11% and 1% respectively). Among the subgroups all four symptoms were much more common in the first group in comparison to the control group (77%, 42%, 30% ve 14%) (p<0.05). Loss of appetite was found more commonly in the second (64%) and the third group (59%) in comparison to the control group. The most common complaint was found to be loss of appetite (71%). In the RURTI group serum zinc (72±8 µg/dL, p=0.0001), serum iron (70.52±28.18 µg/dL, p=0.002), hemoglobin (11.98±0.78 g/dL, p=0.0001), hematocrit (35,52%±2,26, p=0.0001), MCV (78.37±3.83 fL, p=0.002) MCH (26.39±1.45 pg, p=0.005), MCHC (33.04±1.50 g/dL, p=0.006) and IgG (926.99±214.5 mg/dL, p=0.001) averages were lower at statistically significant levels from those of the control groups (84±9 µg/dL, 85.35±35.15 µg/dL, 12.38±0.69 g/dL, 36.87%±2.01%, 80.27±4.34 fL, 27.03±1.65 pg, 34.08±2.02 g/dL, 1035.54±217.96 mg/dL respectively). The first and second groups zinc levels (69±7 ve 75±8 µg/dL) were lower than control group ( p=0,0001). Zinc levels of the first group were also lower than the second and third groups (79±6 µg/dL) (p=0,011 ve 0,0001). Only for the first group the serum iron levels (69,19±30,11 µg/dL) were statistically lower than the control group (p=0,017). Likewise only in the first group, the hemoglobin (11.86±0.88 g/dL), hematokrit (35.3%±2.52% ), MCV (78.19±4.36 fL), MCH (26.27±1.59g/dL), MCHC (32.79±1.48 g/dL) and IgG (895.08±214.17 mg/dL) (although it is within the normal range) were lower than the controls at statistically significant levels (p<0.05). The lowest values were obtained from the first group. First group was determined as being the foremost accountable for the observed significant differences between the control group and RURTI group. Total zinc deficiency was 18.2 %. The incidence of zinc deficiency among children was high while in all of the groups the daily percentage of dietary zinc intake exceeded 100%, thus emphasizing the importance of dietary source of zinc whether it be of vegetable or animal origin, zinc bioavailability and careful analysis of the diet according to the conditions of our XXII country. Zinc deficiency rates were higher in the RURTI group (33.3%) compared to the control group (1.1%) at statistically significant levels (p = 0.0001). Regarding measurements in the subgroups, the incidence of zinc deficiency for the first group (49,1%) and 2nd (%16) was significantly higher than the control group, although much more pronounced in the first group (p=0,0001 ve 0,001). In addition, the incidence of zinc deficiency in the first group was higher than the second, and third groups (5.9%) (p=0,0046 ve 0,0013). Zinc deficiency was detected more frequently in the first group compared to both the control and the other subgroups and in the second group compared to the control group. The prevalance of anemia was determined to be 16% in total. Although incidence of anemia was higher in the URTI group (21.2%) compared to the control group (10.2%), it was not statistically significant (p=0,065). In the subgroups, the incidence of anemia in the first group (29.8%) was higher than the second (8%) and control groups (11.8%). No statistically significant difference was found between the other groups (p>0,05). The prevalance of iron deficiency (ID) was found to be 9.1% in total. No significant difference was found between RURTI group (7.1%) and the control group (11.4%) and between subgroups (First group 5.3%, second group 12% and third group 5.8%) in terms of incidence of ID. (p>0,05). Concurrent zinc and iron deficiency was not found. The prevalance of iron deficiency anemia (IDA) was 7%. No statistically significant difference was found between the RURTI group (9.1%) and the control group (4.5%) in terms of incidence of IDA whereas in the subgroups, all of the patients with IDA were seen in the first group therefore the incidence was higher in the first group (15.8%) than the second, third (0.0%) and the control group at statistically significant levels (p=0,035, 0,001 ve 0,020). All five patients with concurrent zinc deficiency and iron deficiency anemia were in the RURTI group. The incidence of concurrent zinc deficiency and iron deficiency anemia was higher in the RURTI group (5,1%), compared to the control group (0,0%) however it was not statistically significant (p>0,05) Considering subgroups all of these five patients were found to be in the first group hence the incidence of concurrent iron deficiency anemia and zinc deficiency was higher in the first group (8.8%) than the control group (p=0,0046). Total prevalance of iron deficiency or iron deficiency anemia was found to be 16%. No statistically significant difference between the RURTI group (16.2%) and the control group XXIII (15.9%) was found in terms of incidence of ID/IDA. Similarly no statistical difference was found between the RURTI and the control groups concerning the frequency of patients whom either have concurrent zinc deficiency with ID/IDA or not (p>0,05). Incidence of ID/IDA was not statistically different between the subgroups (p>0,05).Concurrent zinc deficiency with ID/IDA was more frequent in the first group (8.8%) than the control group (0.0%) at statistically significant levels (p=0.08). Zinc deficiency and ID/IDA coexisted solely in the first group where the number of infections is most prevalent, concurrent ID/IDA and zinc deficiency has been found to increase the frequency of infections. The probability of zinc deficiency was 43.5 times higher in the RURTI group than the control group (p=0.002; 95% Cl: 5.80-326.28). Even though concurrent ID or ID/IDA with zinc deficiency was 10.3 times more in the RURTI group (p=0.1162; 95% Cl: 0.56-189.03), there was no increase in the risk when ID/IDA occured without zinc deficiency therefore the main factor seems to be zinc defiency alone. Evaluation of the effect of zinc deficiency, anemia, ID and IDA on the frequency of URTIs revealed that zinc deficiency exclusively increased the frequency 40.4 times. The lack of increased susceptibility to infections due to anemia, ID and IDA at statistically significant levels (p>0,05) supports the previous idea that the main factor is indeed the zinc deficiency. Positive correlation was found between the amount of daily intake of zinc and the percentage of adequate daily intake of protein with serum zinc levels (p=0.0001 and 0.0001; r=0.351 and 0.295). Also positive correlation was found between ferritine levels with the amount and the percentage of adequate daily dietary intake of iron (p=0.003 and 0.001; r=0.213 and 0.233). Statistically significant positive correlations were found between hemoglobin and hematocrit levels with the amount of daily intake of iron (p=0.04 and 0.01; r=0.212 and 0.231) and zinc (p=0.01 and 0.0001; r=0.251 and 0.303), and with percentages of adequate daily dietary intake of carbonhydrate (p=0.0001 and 0.0001; r=0.270 and 0.271), and protein (p=0.03 and 0.0001; r=0.212 and 0.73). Positive correlation was also found between hematocrit levels and the percentage of adequate daily intake for iron (p=0.006 r=0.201). Although positive correlations were both detected between MCV and the amount of daily intake of zinc and iron (p=0.001 ve 0.02; r=0.241 ve 0.162) it was more significant in the zinc intake. Statistically significant positive correlations were found between IgG levels with, the amount of daily intake of iron (p=0.001 r=0.261), zinc (p=0.0001 r=0.345), the percentage XXIV of adequate daily intake of protein (p=0.0001 r=0.301), fat (p=0.003 r=0.233) and carbonhydrate (p=0.0001 r=0.330). Finally a statistically significant positive correlation was found between the amount of daily intake of zinc and IgA levels (p=0.002 r=0.239). Conclusion: Zinc deficiency is an important trace element deficiency during childhood. Zinc deficiency increases susceptibility to upper respiratory tract infections. Turkey is at risk for zinc deficiency more than it is for iron deficiency and therefore children with ID should be strictly examined for zinc deficiency. Zinc and iron levels are found to be lower (more pronounced for zinc levels) in children with RURTIs than the normal population. Zinc and iron levels were distinctively low in the group with the most episodes of infection. Iron and in particular zinc deficiency causes anorexia that turns into a vicious cycle in which the inadequate nutrition of the child leads to a further increase of deficiencies. Nutrition has a significant effect on the immune system. Patients who suffer from RURTI should be evaluated for trace element and protein-energy deficiencies before being subjected to unnecessary testing.
