Özet
Homosistein metabolizmasında vitamin B12, folat, vitamin B6 kofaktör olarak görev almaktadır. Bu çalışmanın amacı konjenital kalp hastalığı (KKH) bulunan çocuk hastalarda ve annelerinde homosistein, vitamin B12, folat ve vitamin B6'nın konsantrasyonlarını belirlemek ve doğumsal kalp hastalıklarıyla ilişkisini saptamaktır.Çalışmaya konjenital kalp hastalığı tespit edilen yaş ortalaması 3,6 ay olan 50 çocuk ile bu çocukların yaş ortalaması 30,4 yıl olan anneleri alındı. Kontrol grubundaki 50 çocuğun yaş ortalaması 4,4 ay, annelerin yaş ortalaması 29,1 yıldı.Homosistein düzeyleri KKH'ı olan olgularda 12,1±2,76 mmol/L, kontrol grubu olgularda 7,21±2,40 mmol/L bulunmuş olup çalışma grubunda homosistein seviyesinin yüksek olması istatistiksel olarak anlamlıydı (p<0,05). Vitamin B12 düzeyleri KKH'ı olan olgularda 375,1±236,3 pg/mL, kontrol grubu olgularda 474,3±270,7 pg/mL bulunmuş olup çalışma grubunda vitamin B12 seviyesinin düşük olması istatistiksel olarak anlamlıydı (p<0,05). Folat düzeyleri KKH'ı olan olgularda 13,2±4,55 ng/mL, kontrol grubu olgularda 12,5±4,21 ng/ml bulunmuş olup gruplar arasında istatistiksel olarak anlamlı fark olmadığı görülmüştür (p>0,05). Vitamin B6 düzeyleri KKH'ı olan olgularda 12,02±3,75 ng/ml, kontrol grubu olgularda 10,42±3,64 ng/ml bulunmuş olup gruplar arasında istatistiksel olarak anlamlı fark olmadığı görülmüştür (p>0,05).Homosistein düzeyleri KKH'ı olan olguların annelerinde 14,9±2,35 mmol/l, kontrol grubu olgularda 12,4±3,04 mmol/l bulunmuş olup çalışma grubunda homosistein seviyesinin yüksek olması istatistiksel olarak anlamlıydı (p<0,05). Vitamin B12 düzeyleri KKH'ı olan olguların annelerinde 234,8±91,6 pg/ml, kontrol grubu olgularda 299,5±146,1 pg/ml bulunmuş olup çalışma grubunda vitamin B12 seviyesinin düşük olması istatistiksel olarak anlamlıydı (p<0,05). Folat düzeyleri KKH'ı olan olguların annelerinde 5,88±3,46 ng/ml kontrol grubu olgularda 7,96±3,87 ng/ml bulunmuş olup çalışma grubunda folat seviyesinin düşük olması istatistiksel olarak anlamlıydı (p<0,05). Vitamin B6 düzeyleri KKH'ı olan olguların annelerinde 11,1±3,41 µg/l, kontrol grubu olgularda 11,4±3,13 µg/l bulunmuş olup gruplar arasında istatistiksel olarak anlamlı fark olmadığı görülmüştür (p>0,05).Sonuç olarak, konjenital kalp hastalığı olan çocukların annelerinde homosistein düzeyinin yüksek, vitamin B12 ve folat düzeyinin düşük olduğu görüldü. Maternal hiperhomosisteineminin doğumsal kalp hastalığı riskinde artışa yol açabileceği düşünüldü.

Vitamin B12, folate and vitamin B6 are cofactors of homocysteine metabolism. The purpose of this study is to identify the levels of homocysteine, vitamin B12, folate and vitamin B6 in child patients with congenital heart disease (CHD) and to determine the relationship of homocysteine and these cofactors between children and their mothers.The average ages of 50 children with congenital heart disease and their mothers are respectively 3,6 months and 30,4 years. In control group the average ages of 50 children and their mothers are 4,4 months and 29,1 years, respectively.Homocysteine levels from the study group with congenital heart disease and control group were 12,1±2,76 mmol/L and 7,21±2,40 mmol/L, respectively. Homocysteine levels from the case group with congenital heart disease were significantly higher than group (P<0,05). Vitamin B12 levels from the study group with congenital heart disease and control group were 375,1±236,3 pg/mL and 474,3 ± 270,7, respectively. Folate levels from the mothers of the study group with congenital heart disease were significantly lower than our control group (P<0,05). Vitamin B6 levels from the mothers of the study group with congenital heart disease and control group were 11,1±3,41 ng/mL and 11,4±3,13 ng/mL, respectively. There was no significance in vitamin B6 levels between study group and control group (P>0,05).As a result, levels of homocysteine were higher and vitamin B12 and folate levels were lower in mothers of children with congenital heart disease. İt was thought maternal hyperhomocysteinemia might an increased risk in the children with congenital heart disease.
