Özet
Amaç: Her merkezin, cerrahi antibiyotik profilaksisi (CAP) protokolleri oluşturması güncel gereksinimdir. Ancak hekimlerin, özellikle ameliyat sırasında karşılaşabileceği risk faktörleri ve komplikasyonlar sebebiyle antibiyotik kullanımında artış görülmektedir. Bu çalışmanın amacı; eğitimler ve farkındalığın artırılması ile güncellenen CAP protokolüne uyumun izlemi, uygunsuz antibiyotik kullanımının azaltılması ve cerrahi alan enfeksiyonu (CAE) gelişimine etkilerinin gösterilmesidir. Yöntem: Bir üniversite hastanesi Genel Cerrahi Servisinde hepatopankreatobiliyer cerrahi [kolesistektomi, hepatektomi, pankreatektomi ve pankreatikoduodenektomi (PD)] hastalarının verileri, üçer aylık süreler ile retrospektif ve prospektif olarak, rutin işleyişe karışılmadan analiz edildi. Retrospektif dönem (RG) verileri cerrahi ekip ile paylaşılıp, CAP protokolü güncellendi. Eğitim ve farkındalık toplantıları yapılıp, sonrasında prospektif dönem (PG) izlemi yapıldı. Hastalar CAE gelişimi açısından postoperatif bir aylık döneme kadar takip edildi. Retrospektif ve prospektif CAP uygulamaları, CAE gelişimi ve güncellenmiş protokole uyum karşılaştırıldı. Bulgular: Toplam 300 hasta (RG=137, PG=163) takip edildi. Hastaların %11 (n=33)' inde CAE gelişmiş olup, bunun %42 (14/33)' si PD sonrasıdır. Bütün HBC hastalarında CAE oranları; RG' de %11,7 (16/137) ve PG' de %10,4 (17/163) olarak saptandı (p=0,730). PD hariç HBC' de CAE; RG' de %9,5 (12/126) ve PG' de %4,7 (7/148) saptandı (p=0,120). Erkek cinsiyet, ileri yaş (>65 yaş), iki veya üzeri kronik hastalık ve malignite CAE risk faktörü olarak saptanmıştır (sırasıyla p=0.017, p=0,028, p=0,003 ve p=0,009). Laparoskopik kolesistektomide, intraabdominal/ karaciğere yapışıklık ve kolesistit/pankreatit varlığı, her iki dönemde de postoperatif antibiyotik kullanımının en sık sebepleridir (RG p<0,001 ve p=0,014, PG ikisi de p=0,002). CAP doğru dozda uygulama oranı %91,2' den %100' e çıkarılmıştır (p<0,001). Postoperatif dönemde antibiyotik endikasyonu olmayan hastalara (n=190), antibiyotik verilmeme oranı PG'de artmıştır (p=0,048). Yatışında antibiyotik almasa bile taburculukta reçete edilenlerin oranı PG'de azalmıştır (%44,2 ve %23,3, p=0,013). Hepatopankreatobiliyer cerrahide CAP uygulamalarında, RG ve PG arasında istatistiksel olarak anlamlı fark saptanmıştır (p=0,008). Sonuç: Her iki dönemde de benzer CAE oranları saptanmıştır. Cerrahi antibiyotik profilaksi uygulamalarında, eğitim/farkındalık çalışmaları ve cerrahi ekip ile iletişim halinde olmanın uygunsuz antibiyotik kullanımını istatistiksel anlamlı olarak azaltmıştır. Postoperatif antibiyotik endikasyonu olmayan hastalara, taburculukta reçete yazılma oranı da azalmıştır. Hastaya ait veya operasyonda karşılaşılan risk faktörleri de dikkate alınarak, rehberler doğrultusunda bireyselleştirilmiş profilaksi protokolleri uygulanması; CAE oranlarını azaltacaktır.

Aim: It is currently essential for every center to establish surgical antibiotic prophylaxis (SAP) protocols. However, an increase in antibiotic use has been observed due to risk factors and complications that physicians, particularly during surgery, may encounter. The aim of this study was to monitor adherence to the updated SAP protocol through education and increased awareness, reduce inappropriate antibiotic use, and demonstrate its effects on the development of surgical site infections (SSI). Materials and Methods: Data from patients undergoing hepatopancreatobiliary (HPB) surgery [cholecystectomy, hepatectomy, pancreatectomy, and pancreaticoduodenectomy (PD)] in the General Surgery Department of a university hospital were analyzed retrospectively and prospectively in three-month periods without interfering with routine clinical practice. Data from the retrospective period (RP) were shared with the surgical team, and the SAP protocol was updated. Educational and awareness meetings were conducted, followed by prospective period (PP) monitoring. Patients were followed for up to one month postoperatively for the development of SSI. Retrospective and prospective SAP practices, SSI development, and adherence to the updated protocol were compared. Results: A total of 300 patients (RP=137, PP=163) were followed. SSI developed in 11% (n=33) of the patients, of which 42% (14/33) occurred after PD. SSI rates in all HPB patients were 11.7% (16/137) in the RP and 10.4% (17/163) in the PP (p=0.730). Excluding PD, SSI rates in HPB surgery were 9.5% (12/126) in the RP and 4.7% (7/148) in the PP (p=0.120). Male sex, advanced age (>65 years), the presence of two or more chronic diseases, and malignancy were identified as risk factors for SSI (p=0.017, p=0.028, p=0.003, and p=0.009, respectively). In laparoscopic cholecystectomy, intra-abdominal/hepatic adhesions and the presence of cholecystitis/pancreatitis were the most common reasons for postoperative antibiotic use in both periods (RP p<0.001 and p=0.014; both p=0.002 in the PP). The rate of correct-dose SAP administration increased from 91.2% to 100% (p<0.001). Among patients without an indication for postoperative antibiotics (n=190), the rate of not prescribing antibiotics increased in the PP (p=0.048). The proportion of patients who were prescribed antibiotics at discharge despite not receiving antibiotics during hospitalization decreased in the PP (44.2% vs. 23.3%, p=0.013). A statistically significant difference was found between the RP and PP in SAP practices in hepatopancreatobiliary surgery (p=0.008). Conclusion: Similar SSI rates were observed in both periods. Educational/awareness activities and maintaining communication with the surgical team significantly reduced inappropriate antibiotic use in surgical antibiotic prophylaxis practices. The rate of prescribing antibiotics at discharge for patients without an indication for postoperative antibiotics also decreased. Implementing individualized prophylaxis protocols in accordance with guidelines, while considering patient-related and intraoperative risk factors, may reduce SSI rates.
