Özet
Yüzeyel mantar enfeksiyonları, dermatoloji polikliniklerinde en sık karşılaşılan enfeksiyon hastalıkları arasında yer almakta olup dünya genelinde yaklaşık %20–25 oranında görülmektedir. Kronikleşme eğilimleri, pruritus, ağrı, fissür oluşumu ve sekonder bakteriyel enfeksiyonlara zemin hazırlamaları nedeniyle hastaların yaşam kalitesini belirgin şekilde olumsuz etkileyebilmektedir. Bu enfeksiyonların en yaygın etkenleri dermatofitler olup, dermatofitozlar klinik olarak“tinea”olarak adlandırılmaktadır. Dermatofitler keratinize dokulara yerleşerek deri, saç ve tırnakları enfekte edebilmekte ve farklı anatomik lokalizasyonlara göre çeşitli klinik alt tiplerde ortaya çıkmaktadır. Bu alt tipler arasında tinea pedis (ayak), tinea manuum (el), tinea kruris (kasık), tinea korporis (gövde), tinea kapitis (saçlı deri) ve tinea unguium (tırnak) yer almaktadır. Bu çalışmamızda kutanöz yüzeyel mantar enfeksiyonlarında Lulikonazol % 1 topikal uygulamanın klinik semptom,bulgular ve mikolojik sonuçlar üzerine etkisini değerlendirmek amaçlanmıştır. Dahil edilme ve dışlanma kriterlerini karşılayan, fizik muayene ve anamnez bulguları ile birlikte %10 potasyum hidroksit (KOH) ile yapılan direkt mikroskobik inceleme sonucunda yüzeyel mantar enfeksiyonu tanısı alan ve topikal lulikonazol tedavisi uygulanan tinea korporis, tinea kruris ve tinea pedis tanılı hastalar çalışmaya dahil edilmiştir. Klinik değerlendirme kapsamında kaşıntı, eritem, maserasyon ve skuam gibi semptom ve bulgular; yok (0), hafif (1), orta (2) ve şiddetli (3) olarak puanlanmış, bu parametreler tedavi öncesi ve tedavi sonrası dönemde karşılaştırmalı olarak analiz edilmiştir. Mikolojik değerlendirme ise tedavi sonrası %10 KOH ile yapılan direkt mikroskobik inceleme sonuçlarına göre gerçekleştirilmiştir. Genel tedavi etkinliğinin belirlenmesinde, klinik iyileşme ve mikolojik iyileşme kriterlerini birlikte karşılayan hastalar esas alınarak kombine başarı değerlendirmesi yapılmıştır. Çalışmaya toplam 100 hasta dahil edilmiştir. Olguların yaş ortalaması 44,36 ± 17,06 yıl olup hastaların %52,0'sinin (n=52) erkek, %48,0'inin (n=48) kadın olduğu saptanmıştır. Tinea türleri açısından değerlendirildiğinde olguların %49,0'unun (n=49) tinea korporis, %30,0'unun (n=30) tinea pedis ve %21,0'inin (n=21) tinea kruris olduğu belirlenmiştir. Hastalık süresi açısından olguların %61,0'inin (n=61) 6 aydan kısa süreli, %39,0'unun (n=39) ise 6 ay ve daha uzun süreli olduğu görülmüştür. On dört günlük tedavi sonrası değerlendirmede olguların %68,0'inde (n=68) pruritus saptanmazken, %27,0'sinde (n=27) hafif düzeyde pruritus devam ettiği belirlenmiştir. Eritem açısından değerlendirildiğinde tedavi sonrası olguların %57,0'sinde (n=57) eritem bulunmadığı, %41,0'inde (n=41) hafif eritem olduğu saptanmıştır. Skuam değerlendirmesinde ise olguların %58,0'inde (n=58) skuam bulunmazken, %38,0'inde (n=38) hafif skuam varlığı devam etmiştir. Klinik bileşik skor ortalaması tedavi öncesinde 5,61 ± 1,61 iken, 14 günlük lulikonazol tedavisi sonrasında 1,54 ± 1,42 olarak bulunmuş olup yaklaşık %72,6 oranında azalma saptanmış ve bu farkın istatistiksel olarak anlamlı olduğu belirlenmiştir (t=25,285; p<0,001). Klinik bileşik skorda en az %50 gerileme gösteren olguların,klinik iyileşme, oranı %87,0 (n=87) olarak saptanmıştır. Tinea türlerine göre incelendiğinde bu oran tinea korporis/kruris grubunda %90,0 (n=63), tinea pedis grubunda ise %80,0 (n=24) olarak bulunmuştur. Direkt mikroskop altında nativ preparat incelemesine göre 14 günlük tedavi sonunda genel mikolojik iyileşme oranı %81,0 (n=81) olup, tinea türlerine göre değerlendirildiğinde; tinea korporis/kruris grubunda mikolojik iyileşme oranı %85,7 (n=60) iken, tinea pedis grubunda bu oran %70,0 (n=21) olarak bulunmuştur.Klinik bileşik skorda ≥%50 azalma ve nativ negatifliğin birlikte değerlendirilmesi ile belirlenen genel klinik ve mikolojik başarı oranı %77,0 (n=77) olarak bulunmuştur. Tinea türlerine göre dağılım incelendiğinde; tinea korporis/kruris grubunda başarı oranı %84,3 (59/70), tinea pedis grubunda ise %60,0 (18/30) olup gruplar arasındaki fark istatistiksel olarak anlamlı bulunmuştur (Pearson ki-kare=6,994; sd=1; p=0,008; Fisher exact p=0,018). Tam klirens (klinik bulguların tamamen gerilemesi ve mikolojik iyileşme) genel olarak olguların %29,0'unda (n=29) saptanmıştır. Tinea korporis/kruris grubunda olguların %32,9'unda (n=23) tam klirens sağlanırken, tinea pedis grubunda bu oran %20,0 (n=6) olarak saptanmıştır., Tam klirens açısından tinea türleri arasında istatistiksel olarak anlamlı fark bulunmamıştır (Pearson Ki-kare, p=0,194). Bu çalışmada, dermatofit enfeksiyonlarının klinik ve mikolojik yanıtı üzerinde etkili olan faktörler çok yönlü olarak değerlendirilmiş ve %1 topikal lulikonazol tedavisinin kısa süreli kullanım sonrası etkinliği ortaya konmuştur. Elde edilen bulgular, dermatofitozların yalnızca etken mikroorganizmaya bağlı değil; konak faktörleri, enfeksiyonun lokalizasyonu, tedaviye uyum ve farmakokinetik özellikler gibi çok sayıda değişkenin etkileşimi sonucunda şekillenen kompleks bir klinik tablo olduğunu göstermektedir. Çalışmamızda tinea korporis/kruris olgularında daha yüksek klinik ve mikolojik yanıt elde edilmesi, bu bölgelerde ilacın daha iyi penetrasyon göstermesi ile açıklanabilir. Buna karşılık, tinea pedis olgularında saptanan daha düşük yanıt oranları; hiperkeratoz, nemli ortam ve kronik irritasyon gibi lokal faktörlerin yanı sıra, hasta popülasyonuna ait özellikler ve eşlik eden hastalıklar gibi sistemik faktörlerle ilişkili olabilir.On dört günlük lulikonazol tedavisi sonrası klinik bileşik skorda anlamlı azalma, pruritus, eritem ve skuam gibi temel klinik bulgularda belirgin düzelme ile birlikte %87 oranında klinik iyileşme sağlanması, ilacın kısa süreli kullanımda dahi güçlü bir klinik etkinliğe sahip olduğunu göstermektedir. Ayrıca %81 oranında mikolojik iyileşme ve %77 oranında genel klinik ve mikolojik başarı,etkin tedavi, elde edilmesi, lulikonazolün yalnızca semptomatik değil, aynı zamanda antifungal eradikasyon açısından da etkili olduğunu ortaya koymaktadır. Bununla birlikte tam klirens oranının %29 olarak saptanması, klinik bulguların tamamen ortadan kalkmasının mikolojik iyileşmeye kıyasla daha uzun süre gerektirdiğini düşündürmektedir. Bu durum, tedavi sonrası dönemde iyileşmenin devam edebileceğini ve daha uzun takip sürelerinde tam klirens oranlarının artabileceğini göstermektedir. Sonuç olarak, %1 topikal lulikonazol dermatofit enfeksiyonlarında kısa süreli kullanımda etkili bir tedavi seçeneği olarak öne çıkmaktadır. Ancak yalnızca kısa dönem tedavi yanıtının değerlendirilmiş olması nedeniyle uzun dönem etkinlik ve nüks oranlarına ilişkin kesin yargılara varılamamaktadır. Bu nedenle, elde edilen bulguların daha geniş hasta popülasyonlarını kapsayan, farklı klinik alt grupları içeren ve daha uzun süreli takip verileri içeren çalışmalarla desteklenmesi gerekmektedir.

Superficial fungal infections are among the most frequently encountered infectious diseases in dermatology outpatient clinics and are seen worldwide at a rate of approximately 20–25%. Due to their tendency to become chronic, pruritus, pain, fissure formation, and their predisposition to secondary bacterial infections, they may significantly and negatively affect patients' quality of life. The most common causative agents of these infections are dermatophytes, and dermatophytoses are clinically referred to as“tinea.”Dermatophytes colonize keratinized tissues and can infect the skin, hair, and nails, and may present in various clinical subtypes according to different anatomical localizations. These subtypes include tinea pedis (foot), tinea manuum (hand), tinea kruris (groin), tinea korporis (body), tinea capitis (scalp), and tinea unguium (nail). Patients diagnosed with tinea korporis, tinea kruris, and tinea pedis who met the inclusion and exclusion criteria, were diagnosed with superficial fungal infection as a result of physical examination and anamnesis findings together with direct microscopic examination performed with 10% potassium hydroxide (KOH), and received topical luliconazole treatment were included in the study. Within the scope of clinical evaluation, symptoms and findings such as pruritus, erythema, maceration, and scaling were scored as absent (0), mild (1), moderate (2), and severe (3), and these parameters were comparativ analyzed in the pre-treatment and post-treatment periods. Mycological evaluation was performed according to the results of direct microscopic examination carried out with 10% KOH after treatment. In determining the overall treatment efficacy, a combined success evaluation was made based on patients who met both the clinical improvement and mycological improvement criteria together. A total of 100 patients were included in the study. The mean age of the cases was 44.36 ± 17.06 years, and it was determined that 52.0% (n=52) of the patients were male and 48.0% (n=48) were female. When evaluated in terms of tinea types, it was determined that 49.0% (n=49) of the cases had tinea korporis, 30.0% (n=30) had tinea pedis, and 21.0% (n=21) had tinea kruris. In terms of disease duration, it was observed that 61.0% (n=61) of the cases had a duration of less than 6 months, whereas 39.0% (n=39) had a duration of 6 months or longer. In the evaluation after fourteen days of treatment, while pruritus was not detected in 68.0% (n=68) of the cases, it was determined that mild pruritus persisted in 27.0% (n=27). When evaluated in terms of erythema, it was determined that 57.0% (n=57) of the cases had no erythema after treatment, whereas 41.0% (n=41) had mild erythema. In the evaluation of scaling, while 58.0% (n=58) of the cases had no scaling, the presence of mild scaling persisted in 38.0% (n=38). While the mean clinical composite score was 5.61 ± 1.61 before treatment, it was found to be 1.54 ± 1.42 after 14 days of luliconazole treatment, showing an approximately 72.6% decrease, and this difference was determined to be statistically significant (t=25.285; p<0.001). The rate of clinical improvement among the cases showing at least a 50% reduction in the clinical composite score was determined to be 87.0% (n=87). When examined according to tinea types, this rate was found to be 90.0% (n=63) in the tinea korporis/kruris group and 80.0% (n=24) in the tinea pedis group. According to native preparation examination under direct microscopy, the overall mycological improvement rate at the end of 14 days of treatment was 81.0% (n=81); when evaluated according to tinea types, the mycological improvement rate was 85.7% (n=60) in the tinea korporis/kruris group, whereas this rate was found to be 70.0% (n=21) in the tinea pedis group. The overall clinical and mycological success rate determined by the combined evaluation of a ≥50% reduction in the clinical composite score and native negativity was found to be 77.0% (n=77). When the distribution according to tinea types was examined, the success rate was 84.3% (59/70) in the tinea korporis/kruris group and 60.0% (18/30) in the tinea pedis group, and the difference between the groups was found to be statistically significant (Pearson chi-square=6.994; df=1; p=0.008; Fisher exact p=0.018). Complete clearance (complete resolution of clinical findings and mycological improvement) was detected overall in 29.0% (n=29) of the cases. While complete clearance was achieved in 32.9% (n=23) of the cases in the tinea korporis/kruris group, this rate was found to be 20.0% (n=6) in the tinea pedis group. No statistically significant difference was found between tinea types in terms of complete clearance (Pearson chi-square, p=0.194). In this study, factors affecting the clinical and mycological response of dermatophyte infections were evaluated comprehensively, and the efficacy of short-term treatment with 1% topical luliconazole was demonstrated. The findings obtained indicate that dermatophytosis represents a complex clinical condition shaped not only by the causative microorganism, but also by the interaction of numerous variables such as host factors, localization of infection, treatment adherence, and pharmacokinetic properties. In our study, the higher clinical and mycological response observed in cases of tinea corporis/cruris may be explained by better penetration of the drug in these regions. In contrast, the lower response rates detected in cases of tinea pedis may be associated with local factors such as hyperkeratosis, moist environment, and chronic irritation, as well as systemic factors including characteristics of the patient population and accompanying comorbidities. Following fourteen days of luliconazole treatment, the significant reduction in clinical composite score, together with marked improvement in major clinical findings such as pruritus, erythema, and scaling, and the achievement of an 87% clinical improvement rate, demonstrate that the drug has strong clinical efficacy even with short-term use. In addition, the achievement of an 81% mycological cure rate and a 77% overall clinical and mycological success rate indicates that luliconazole is effective not only symptomatically but also in terms of antifungal eradication. However, the complete clearance rate being determined as 29% suggests that the complete resolution of clinical findings may require a longer period compared with mycological improvement.This finding indicates that recovery may continue during the post-treatment period and that complete clearance rates may increase with longer follow-up durations. In conclusion, 1% topical luliconazole appears to be an effective treatment option for dermatophyte infections in short-term use. However, since only short-term treatment response was evaluated, definitive conclusions regarding long-term efficacy and recurrence rates cannot be drawn. Therefore, these findings should be supported by further studies including larger patient populations, different clinical subgroups, and longer follow-up data
