Özet

Amaç: Bu çalışmada, keratoplasti uygulanan hastalarda preoperatif glokom varlığının ve postoperatif takip sürecinde glokom gelişim insidansının değerlendirilmesi amaçlandı. Ayrıca keratoplasti sonrası glokom gelişme sıklığının keratoplasti tiplerine göre karşılaştırılması ve glokom gelişen hastalarda uygulanan medikal ve cerrahi tedavi yaklaşımlarının incelenmesi hedeflendi. Gereç ve Yöntem: Retrospektif olarak planlanan bu çalışmaya keratoplasti uygulanan toplam 679 hastanın 739 gözü dahil edildi. Hastalar uygulanan keratoplasti türüne göre PKP, DALK ve DMEK gruplarına ayrıldı. Olguların preoperatif glokom varlığı, postoperatif takiplerde glokom gelişimi, göz içi basıncı, görme keskinliği, kullanılan antiglokomatöz molekül sayısı, greft başarısı ve glokom cerrahisi gereksinimi değerlendirildi. Glokom varlığı preoperatif dönem, postoperatif 1. ay, 6. ay ve son vizitte incelendi. Bulgular: Çalışmaya dahil edilen hastaların 390'ına PKP, 125'ine DALK ve 224'üne DMEK uygulanmıştı. Preoperatif glokom oranı tüm olgularda %8,0 iken, bu oran postoperatif 1. ayda %25,7, 6. ayda %42,5 ve son vizitte %45,9 olarak saptandı. Son vizitte glokom oranı PKP grubunda %55,4, DALK grubunda %28,8 ve DMEK grubunda %38,8 idi. Glokom varlığı açısından keratoplasti türü, zaman ve keratoplasti türü-zaman etkileşimi istatistiksel olarak anlamlı bulundu (p<0,001). Keratoplasti endikasyonlarına göre son vizitte en yüksek glokom oranları tekrar nakil grubunda %68,3 ve terapötik/tektonik endikasyon grubunda %64,6 olarak izlendi. Antiglokomatöz ilaç sayısı preoperatif dönemde 0,19±0,72 iken son vizitte 1,06±1,34'e yükseldi (p<0,001). Greft başarısı 1. ayda %93,8, 6. ayda %93,1 ve son vizitte %79,4 olarak saptandı (p<0,001). Toplam 40 hastaya glokom cerrahisi uygulandı; bu hastaların %67,5'i PKP, %32,5'i DMEK grubundaydı ve DALK grubunda glokom cerrahisi uygulanan hasta yoktu. Keratoplasti türüne göre glokom cerrahisi gereksinimi anlamlı farklılık gösterdi (p=0,011). Sonuç: Keratoplasti sonrası glokom gelişimi takip sürecinde belirgin artış göstermekte ve bu artış keratoplasti türüne göre farklılık göstermektedir. PKP grubunda glokom oranı ve glokom cerrahisi gereksinimi daha yüksek izlenirken, DALK grubunda glokom cerrahisi gereksinimi görülmemiştir. Bu nedenle keratoplasti uygulanan hastalarda preoperatif glokom varlığının dikkatle değerlendirilmesi, postoperatif dönemde düzenli göz içi basıncı takibi yapılması ve glokom gelişen olgularda uygun medikal veya cerrahi tedavinin zamanında uygulanması önemlidir.

Purpose: This study aimed to evaluate the presence of preoperative glaucoma and the incidence of glaucoma development during postoperative follow-up in patients who underwent keratoplasty. In addition, the frequency of glaucoma development after keratoplasty was compared according to keratoplasty type, and the medical and surgical treatment approaches applied in patients who developed glaucoma were examined. Materials and Methods: A total of 739 eyes of 679 patients who underwent keratoplasty were included in this retrospective study. Patients were divided into PKP, DALK, and DMEK groups according to the type of keratoplasty performed. Preoperative glaucoma status, postoperative glaucoma development, intraocular pressure, visual acuity, number of antiglaucomatous molecules used, graft success, and the need for glaucoma surgery were evaluated. Glaucoma status was assessed preoperatively and at postoperative 1-month, 6-month, and final follow-up visits. Results: Of the patients included in the study, 390 underwent PKP, 125 underwent DALK, and 224 underwent DMEK. The overall rate of preoperative glaucoma was 8.0%, which increased to 25.7% at postoperative 1 month, 42.5% at 6 months, and 45.9% at the final visit. At the final visit, the glaucoma rate was 55.4% in the PKP group, 28.8% in the DALK group, and 38.8% in the DMEK group. Keratoplasty type, time, and the keratoplasty type-time interaction were found to be statistically significant in terms of glaucoma presence (p<0.001). According to keratoplasty indications, the highest glaucoma rates at the final visit were observed in the regraft group at 68.3% and in the therapeutic/tectonic indication group at 64.6%. The number of antiglaucomatous medications increased from 0.19±0.72 in the preoperative period to 1.06±1.34 at the final visit (p<0.001). Graft success was 93.8% at 1 month, 93.1% at 6 months, and 79.4% at the final visit (p<0.001). A total of 40 patients underwent glaucoma surgery; 67.5% of these patients were in the PKP group and 32.5% were in the DMEK group, while no patient in the DALK group underwent glaucoma surgery. The need for glaucoma surgery differed significantly according to keratoplasty type (p=0.011). Conclusion: Glaucoma development after keratoplasty showed a marked increase during the follow-up period, and this increase differed according to keratoplasty type. While the glaucoma rate and the need for glaucoma surgery were higher in the PKP group, no glaucoma surgery was required in the DALK group. Therefore, careful evaluation of preoperative glaucoma status, regular postoperative intraocular pressure monitoring, and timely medical or surgical treatment in patients who develop glaucoma are important in patients undergoing keratoplasty.
