Özet

Adli tıp uygulamalarında ölüm zamanı tayini, kimliklendirme ve ölüm sebebinin araştırılması açısından kritik öneme sahiptir. Postmortem değişiklikler pek çok endojen ve eksojen faktörden etkilenmekte olup, özellikle antemortem dönemde kullanılan ilaçların postmortem süreç üzerindeki etkileri yeterince aydınlatılamamıştır. Bu çalışmada, antibiyotik (amoksisilin-klavulanik asit) ve sitotoksik ajan (metotreksat) uygulamalarının postmortem çürüme süreci üzerindeki etkileri, kontrollü deneysel koşullar altında biyokimyasal, histopatolojik, moleküler ve makroskobik parametreler üzerinden değerlendirilmiştir. Toplam 36 rat, antemortem ilaç uygulamalarına göre antibiyotik, sitotoksik ve kontrol gruplarına ayrılmış; ayrıca her grupta çevresel koşullar (iç ortam/dış ortam) ve cinsiyet (erkek/dişi) faktörleri de dengeli biçimde dağıtılmıştır. Nekropsiler ölüm sonrası 0., 24. ve 96. saatlerde gerçekleştirilmiş, organ ve dokular adli otopsi protokollerine uygun şekilde örneklenmiş ve makroskobik bulgular kaydedilmiştir. Histopatolojik incelemeler, çift-kör yöntemle iki deneyimli patoloji uzmanı tarafından yarı-kantitatif skorlamayla değerlendirilmiş; ayrıca çekirdek sayısı (ÇS), çekirdek alanı (ÇA) ve çekirdek boyanma yoğunluğu kaybı/intensite kaybı (İK) parametreleri ImageJ yazılımı kullanılarak kantitatif olarak ölçülmüştür. Moleküler düzeyde RNA Integrity Number (RIN) ve Nanodrop spektrofotometrisi ile RNA bütünlüğü ve konsantrasyonu analiz edilmiştir. Biyokimyasal düzeyde, putresin ve kadaverin seviyeleri yüksek performanslı ince tabaka kromatografisi (HP-TLC) yöntemiyle belirlenmiştir. Makroskobik değerlendirmelerde, dış ortam koşullarına maruz bırakılan örneklerde çürümenin daha hızlı geliştiği, iç ortamda ise geciktiği gözlenmiştir. Antibiyotik uygulanan gruplarda 96. saatte çürüme bulgularının kontrol grubuna kıyasla geciktiği tespit edilmiştir. Histopatolojik analizlerde, 24. saatte özellikle bazofili kaybı (İK) parametresinde antibiyotik ve sitotoksik gruplarda kontrol grubuna göre anlamlı farklılık saptanmıştır (p=0,007). 96. saatte antibiyotik grubunda nükleer bütünlüğün daha iyi korunduğu, çekirdek kaybının daha az olduğu ve bazofili kaybının daha düşük seyrettiği belirlenmiştir. Sitotoksik grupta da bazı parametrelerde kontrol grubuna göre doku bütünlüğünün daha iyi korunduğu görülmüş, ancak etki antibiyotik kadar belirgin olmamıştır. Çift-kör histopatolojik incelemede gözlemciler arası uyum yüksek (κ=0,66– 0,74) bulunmasına rağmen gruplar arasında anlamlı fark tespit edilememiş; buna karşılık ImageJ tabanlı kantitatif ölçümlerin daha duyarlı olduğu görülmüştür. Moleküler düzeyde, tüm gruplarda RNA bütünlüğünün postmortem süreçle hızla bozulduğu tespit edilmiştir. Ölüm sonrası 0. saatte RIN değerleri 3,8–4,5 arasında değişirken, 96. saatte tüm gruplarda yaklaşık 2,4 seviyelerine gerilemiştir. Antibiyotik ve sitotoksik gruplar arasında anlamlı fark gözlenmemiştir. Bu sonuç, beyin dokusunun kraniyal koruma altında olması nedeniyle ilaç etkilerinden görece bağımsız bir stabilite sergileyebileceğini düşündürmektedir. Biyokimyasal analizlerde, postmortem 96. saatte putresin düzeylerinde belirgin bir artış saptanmış; bu artışın kontrol grubunda antibiyotik grubuna göre istatistiksel olarak daha yüksek olduğu görülmüştür (p=0,048). Sitotoksik grup ise ara düzeyde seyretmiş ancak istatistiksel olarak anlamlı fark göstermemiştir. Kadaverin yalnızca 96. saatte ölçülebilir düzeylere ulaşmış, fakat gruplar arasında anlamlı farklılık saptanmamıştır. Bu çalışma, antemortem ilaç tedavisinin yanı sıra çevresel koşulların ve cinsiyetin de postmortem bozunma süreçlerini modüle edebileceğini deneysel olarak ortaya koymuştur. Antibiyotik uygulamaları, mikrobiyal metabolizmayı baskılayarak postmortem otoliz ve çürümeyi geciktirmiş; sitotoksik ilaçlar ise bazı histopatolojik parametrelerde doku bütünlüğünü korusa da kontrol grubuna kıyasla heterojen bir etki sergilemiştir. RNA bütünlüğü, tüm gruplarda postmortem interval (PMI) ile doğrusal şekilde azalmış ancak ilaçlardan bağımsız seyretmiştir. Putresin düzeylerinin antibiyotik uygulamalarına duyarlı olması, adli tıp açısından özgün bir bulgu olup, antemortem medikal tedavi öyküsünün PMI tahmininde mutlaka dikkate alınması gerektiğini göstermektedir. Sonuç olarak, ilaç öyküsü, çevresel faktörler ve biyolojik değişkenler ölüm zamanı tayininde kritik parametreler olarak değerlendirilmelidir. Histopatolojik incelemelerde kantitatif dijital analizlerin (ImageJ) duyarlılığı artırdığı, RNA bütünlüğünün tek başına yeterli biyobelirteç olmadığı, putresinin ise geç postmortem dönemde güvenilir bir biyokimyasal gösterge sunduğu ortaya konmuştur.

In forensic medicine, estimating the time since death plays a central role in both individual identification and the evaluation of death circumstances. The progression of postmortem alterations is shaped by a wide range of internal and external variables; nevertheless, the impact of drugs administered prior to death on postmortem biological processes remains incompletely understood. In this study, the effects of antibiotic (amoxicillin-clavulanic acid) and cytotoxic agent (methotrexate) administration on the postmortem decomposition process were evaluated under controlled experimental conditions through biochemical, histopathological, molecular, and macroscopic parameters. Thirty-six rats were included in the study and assigned to antibiotic, cytotoxic, and control groups based on antemortem drug exposure, with environmental setting (indoor/outdoor) and sex (male/female) evenly distributed across all groups. Necropsies were performed at 0, 24, and 96 hours postmortem, during which organs and tissues were sampled in accordance with forensic autopsy protocols, and macroscopic findings were documented. Histopathological examinations were conducted in a semi-quantitative manner by two experienced pathologists using a double-blind method; furthermore, nuclear count (NC), nuclear area (NA), and loss of basophilia/intensity loss (IL) parameters were quantitatively measured using ImageJ software. At the molecular level, RNA integrity and concentration were analyzed by RNA Integrity Number (RIN) assessment and Nanodrop spectrophotometry. At the biochemical level, putrescine and cadaverine levels were determined using high-performance thin-layer chromatography (HP-TLC). In macroscopic assessments, decomposition progressed more rapidly in specimens exposed to outdoor conditions, whereas it was delayed under indoor conditions. In the antibiotic-treated groups, signs of decomposition were delayed at 96 hours compared to the control group. Histopathological analyses revealed a significant difference at 24 hours, particularly in the parameter of loss of basophilia (IL), between the antibiotic and cytotoxic groups versus the control group (p = 0.007). At 96 hours, nuclear integrity was better preserved in the antibiotic group, with reduced nuclear loss and lower levels of basophilia loss. In the cytotoxic group, tissue integrity was also better preserved compared to the control group in certain parameters, although this effect was comparatively weaker than the response detected in the antibiotic group. Despite high inter-observer agreement in the double-blind histopathological evaluation (κ = 0.66– 0.74), group-wise comparisons did not reveal statistically meaningful differences; by contrast, ImageJ-based quantitative measurements demonstrated greater sensitivity. At the molecular level, RNA integrity was found to rapidly deteriorate with the progression of the postmortem process across all groups. At 0 hours postmortem, RIN values ranged between 3.8 and 4.5, whereas by 96 hours they had declined to approximately 2.4 in all groups. Comparisons between the antibiotic and cytotoxic groups did not yield statistically significant differences. This finding suggests that brain tissue may exhibit relative stability due to cranial protection, rendering it less susceptible to the effects of pharmacological interventions. In the biochemical analyses, putrescine levels showed a marked increase at 96 hours, with significantly higher concentrations observed in the control group compared to the antibiotic group (p=0.048). The cytotoxic group exhibited intermediate levels; however, the differences were not statistically significant. Cadaverine reached measurable levels only at 96 hours, but no significant differences were detected between the groups. This study experimentally demonstrated that, in addition to antemortem pharmacological treatments, environmental conditions and sex can also modulate postmortem decomposition processes. Antibiotic administration delayed postmortem autolysis and putrefaction by suppressing microbial metabolism, whereas cytotoxic agents preserved tissue integrity in some histopathological parameters but exhibited a heterogeneous effect compared to the control group. RNA integrity declined linearly with postmortem interval (PMI) across all groups, yet remained independent of drug exposure. The sensitivity of putrescine levels to antibiotic treatment represents a novel forensic finding, underscoring that antemortem medical history must be taken into account in PMI estimation. In conclusion, medical history, environmental factors, and biological variables should be considered as critical parameters in the estimation of the postmortem interval. Histopathological assessments demonstrated that quantitative digital analyses (ImageJ) enhanced sensitivity, while RNA integrity alone was found to be insufficient as a biomarker. Conversely, putrescine emerged as a reliable biochemical indicator during the late postmortem period.
