Özet
Tiroid nodüllerinin ultrason görüntüleri üzerinden malignite riskinin doğru ve güvenilir biçimde tahmin edilmesi, bilgisayarla destekli tanı sistemleri açısından önemli ve zorlu bir problem olarak öne çıkmaktadır. Mevcut derin öğrenme tabanlı yaklaşımların büyük bölümü, problemi ya yalnızca lezyon bölgesine (Region of Interest, ROI) odaklanan sınıflandırma modelleriyle ya da tüm görüntüyü tek bir girdi olarak ele alan yöntemlerle çözmeye çalışmaktadır. Ancak bu yaklaşımlar, lezyon çevresindeki bağlamsal dokuların ve bu bölgeler arasındaki uzamsal ilişkilerin sınıflandırma sürecine katkısını yeterince modelleyememektedir. Özellikle peri-lezyonel bağlamın yapısal olarak ihmal edilmesi, modelin ayırt edici temsil öğrenme kapasitesini kısıtlamaktadır. Bu tez çalışmasında, sitolojik açıdan belirsiz tiroid nodüllerinin malignite riskinin tahmini amacıyla, ROI ve çevresel bağlam bilgisini birlikte ele alan çizge sinir ağları (Graph Neural Networks, GNN) temelli bir öngörü modeli önerilmektedir. Önerilen yaklaşımda, her bir ultrason görüntüsü küçük ölçekli bir çizge olarak temsil edilmektedir. Çizgenin merkezinde lezyon ROI'sini temsil eden bir düğüm yer alırken, bu düğüm etrafında sabit kaydırmalarla örneklenen peri-lezyonel bağlam bölgeleri ek düğümler olarak tanımlanmaktadır. Her düğüm, önceden ImageNet üzerinde eğitilmiş bir ResNet-50 mimarisi aracılığıyla elde edilen görsel özniteliklerle ve düğümler arası göreli konum, ölçek oranı ve örtüşme gibi hafif geometrik özelliklerle temsil edilmektedir. Oluşturulan çizgeler üzerinde GraphSAGE tabanlı bir mesaj geçiş mekanizması uygulanarak düğümler arası ilişkisel bilgi birikimi sağlanmakta, ardından dikkat tabanlı bir okuma (attention pooling) katmanı ile görüntü düzeyinde tekil bir temsil elde edilmektedir. Model, malignite riskini olasılıksal bir çıktı olarak üretmektedir. Deneysel değerlendirmeler, kamuya açık büyük ölçekli bir tiroid ultrason veri kümesi üzerinde gerçekleştirilmiş ve hem tek bölmeli doğrulama hem de 5-kat × 3 tohumlu çapraz doğrulama (k-fold cross validation) protokolü uygulanmıştır. Elde edilen sonuçlar, önerilen ROI+Bağlam temelli GNN yaklaşımının, yalnızca ROI'ye dayalı veya CNN tabanlı klasik yöntemlere kıyasla daha tutarlı ve daha yüksek genellenebilirliğe sahip olduğunu göstermektedir. Ayrıca kapsamlı kalibrasyon analizi ile modelin ürettiği olasılıkların sayısal güvenilirliği değerlendirilmiş ve sıcaklık ölçekleme (temperature scaling) yöntemiyle iyileştirilmiştir. Bu çalışma, tiroid ultrason görüntülerinin ilişkisel bir temsil ile modellenmesinin, malignite risk tahmininde etkili ve tekrarlanabilir bir çerçeve sunduğunu ortaya koymaktadır.

Accurate prediction of malignancy risk in thyroid nodules using ultrasound imaging remains a challenging task in computer-aided diagnosis due to the heterogeneous appearance of nodules and the ambiguity of many imaging features. While ultrasound is widely used as a first-line imaging modality because of its accessibility and non-invasive nature, the interpretation of ultrasound images is inherently subjective and prone to inter-observer variability. Cytologically indeterminate thyroid nodules constitute a difficult subgroup, where conventional assessment strategies often lead to unnecessary biopsies or surgical interventions. These challenges motivate the development of robust, reproducible, and data-driven predictive models that can support risk stratification in a consistent manner. Recent advances in deep learning have significantly improved ultrasound-based thyroid nodule classification, with convolutional neural networks (CNNs) becoming the dominant approach. Most existing methods formulate the task either as a region-of-interest (ROI)–based classification problem, focusing solely on the lesion area, or as a whole-image classification problem that processes the entire ultrasound frame. ROI-based approaches benefit from reduced background noise but frequently discard diagnostically relevant peri-lesional information. Conversely, whole-image models incorporate surrounding anatomy but often dilute lesion-specific cues with irrelevant background content. Patch-based and Multiple Instance Learning (MIL) methods attempt to bridge this gap by aggregating multiple local regions; however, these approaches typically treat patches as independent and unordered instances, without explicitly modeling their spatial or relational structure. This thesis proposes a graph neural network (GNN)–based predictive framework that explicitly models both lesion-centric information and structured peri-lesional context for malignancy risk prediction in cytologically indeterminate thyroid nodules. The core idea is to represent each ultrasound image as a compact graph rather than as a single image tensor. In the proposed formulation, the lesion ROI is represented as a central anchor node, while a fixed number of peri-lesional context regions are sampled at deterministic spatial offsets around the ROI and represented as additional nodes. This design preserves the strengths of ROI-centric analysis while enabling structured integration of contextual information. Each node in the graph is described by a combination of appearance and geometry features. Visual features are extracted using a convolutional backbone pretrained on large-scale natural image datasets, enabling effective transfer learning despite limited medical data. In addition, lightweight geometric descriptors—such as relative spatial offsets, scale ratios, and overlap measures with respect to the ROI—are appended to the node features to provide explicit spatial cues. These enriched node representations are then processed by a graph neural network encoder based on message passing, allowing information to propagate between spatially related regions. Through this mechanism, the model learns not only which regions are informative, but also how their relative arrangement contributes to malignancy prediction. To obtain an image-level prediction, node embeddings produced by the GNN are aggregated using an attention-based readout mechanism. This pooling strategy assigns adaptive importance weights to nodes, enabling the model to emphasize diagnostically relevant peri-lesional regions while down-weighting less informative context. The resulting global representation is passed to a classification head that outputs a probabilistic malignancy-risk estimate. The proposed approach is evaluated on a publicly available, large-scale thyroid ultrasound dataset using the official data splits, complemented by a rigorous cross-validation protocol with multiple random seeds. Performance is assessed using threshold-independent metrics such as the area under the receiver operating characteristic curve (AUROC) and the area under the precision–recall curve (AUPRC), which are particularly suitable for imbalanced classification settings. In addition to discrimination performance, the numerical reliability of predicted probabilities is examined through calibration analysis, including reliability diagrams and post-hoc temperature scaling. Experimental results demonstrate that the ROI+context GNN framework provides consistent and robust performance across validation folds, outperforming or matching conventional CNN-based baselines while offering improved stability and interpretability. Ablation analyses further indicate that both peri-lesional context nodes and explicit geometric features contribute meaningfully to predictive performance. The calibration study shows that the proposed model yields well-behaved probability estimates after simple post-hoc scaling, an important property for downstream decision-support applications. Overall, this work highlights the advantages of relational modeling for ultrasound-based thyroid nodule analysis. By reformulating image classification as a graph-based learning problem, the proposed method introduces a principled inductive bias that aligns well with the structured nature of ultrasound interpretation. The resulting framework serves as a transparent and reproducible baseline for future research on graph-based approaches to medical image analysis and demonstrates the potential of graph neural networks for malignancy risk prediction in challenging, clinically relevant settings. Beyond discrimination performance, this thesis emphasizes methodological rigor and clinical reliability by systematically addressing evaluation stability and probability calibration. In contrast to many prior thyroid ultrasound studies that rely on a single train–validation split, this work adopts a repeated cross-validation protocol with multiple random seeds, providing a variance-aware assessment of model behavior. This evaluation strategy reduces optimism bias and enables more transparent reporting of performance uncertainty, which is critical for medical decision-support systems operating under class imbalance and heterogeneous imaging conditions. Furthermore, this study explicitly investigates the calibration properties of graph-based malignancy predictions. While high AUROC and AUPRC values indicate strong ranking performance, calibrated probabilities are essential for downstream clinical use, such as threshold selection for biopsy recommendation or active surveillance. Calibration is assessed using multiple complementary metrics, including reliability diagrams, expected calibration error, Brier score, and negative log-likelihood. A simple post-hoc temperature scaling procedure is applied to demonstrate that the proposed ROI+context GNN can yield well-behaved probability estimates without modifying the underlying discriminative model. From a broader perspective, this thesis positions graph neural networks as a principled intermediate modeling paradigm between ROI-only convolutional classifiers and unordered patch-based or MIL approaches. By encoding both visual appearance and explicit spatial relationships within a compact and deterministic graph structure, the proposed framework introduces a clinically meaningful inductive bias that aligns with radiological reasoning. The resulting model is lightweight, interpretable at the node level, and reproducible across runs, making it suitable as a baseline reference for future research on relational learning in thyroid ultrasound analysis and, more generally, context-aware medical image classification.
