Özet

Amaç: Bu çalışmanın temel amacı, başta böbrek olmak üzere çeşitli doku biyopsilerinde amiloid birikimini yalnızca H&E boyalı kesitlerden saptayabilen bir yapay zeka modeli geliştirmektir. Amiloid birikiminin amorf (paternsiz) yapısının yarattığı tanısal güçlüğün aşılarak, diğer patolojilerin morfolojisinin saptanması için yapay zekanın kullanılmasında bir yol haritası oluşturulması ve gerçekleştirilebilirliğin gösterilmesi hedeflenmiştir. Modelin klinik güvenilirliğini ve başarısını test etmek amacıyla performans sonuçları diğer önemli modeller ve tıbbi patologların değerlendirmeleriyle, aynı test setinde karşılaştırılmıştır. Gereç ve Yöntem: Çalışmaya, Ankara Üniversitesi Tıbbi Patoloji Arşivi'nden seçilen, 83 amiloid birikimi pozitif ve ayırıcı tanıda güçlük yaratan lezyonları içeren 83 amiloid birikimi negatif böbrek biyopsisi ile farklı dokulara ait test amaçlı 41 doku örneği (toplam 207 vaka) dahil edilmiştir. Görüntüleme yöntemi olarak, gelecekteki trinoküler kamera entegrasyonlu sistemlere zemin hazırlamak ve tarama testi olarak ulaşılabilirliği artırmak üzere, standart ışık mikroskobu (Olympus BX50) okülerinden akıllı telefon (iPhone 15) aracılığı ile görüntüleme kullanılmış ve 400 kat büyütmede dijital görüntüler elde edilmiştir. Amiloid birikimlerinin paternsiz (amorf) ve heterojen yapısının yarattığı tanısal zorluğu aşmak amacıyla, standart mimariler yerine çok ölçekli özellik çıkarımı yapan ve öğrenilebilir havuzlama (GeM) kullanan özgün bir derin öğrenme mimarisi (helloAmyloid v2) geliştirilmiştir. Model, ayrıntılı performans metrikleri ile, farklı deneyim düzeylerine sahip 9 tıbbi patolog ve önemli diğer yapay zeka modelleri ile karşılaştırmalı olarak test edilmiştir. Bulgular: Geliştirilen özgün model (helloAmyloid v2), böbrek biyopsilerinde görüntü düzeyinde tekrarlanabilir şekilde 0.949 ROC-AUC ve 0.895 F1-skoru değerlerine ulaşmıştır. Modelin diğer performans metrikleri 0.889 doğruluk, 0.906 duyarlılık ve 0.870 özgüllük olarak saptanmıştır. Vaka düzeyinde ise izole denemelerde 0.986 ROC-AUC ve %100 özgüllük değerlerine ulaşarak, test edilen standart transfer öğrenme modellerinden (EfficientNet, DenseNet vb.) daha üstün performans sergilemiştir. Aynı dış doğrulama setinde (orijinal görüntülerde) yapılan karşılaştırmalı analizde model, %90.09 genel doğruluk oranı ile deneyimli nefropatologlar (%83.80), uzmanlar (%78.70) ve araştırma görevlilerinden (%69.48) oluşan tüm tıbbi patolog gruplarından daha iyi performans göstermiştir. Model (%91.62) dengeli sonuç veren eşik kullanıldığında dahi hassasiyet açısından, en iyi performansı gösteren nefropatoloğun (%88.83) üzerine çıkarak tarama güvenliğini kanıtlamıştır. Farklı doku tiplerinde yapılan testlerde, özgün mimari detayları sayesinde model hassasiyetini büyük ölçüde koruyabilirken eğitim setinde diğer dokuların olmaması sebebiyle özgüllükte belirgin düşüş izlenmiştir. Sonuç: Bu çalışma, akıllı telefon tabanlı görüntüleme ve derin öğrenme entegrasyonunun, amiloid gibi amorf ve heterojen yapıdaki patolojilerin tespitinde dahi yüksek bir tarama ve tanısal amaçlı uygulanabilirliğe sahip olduğunu göstermiştir. Geliştirilen özgün model, paternsiz birikim özelliklerini beklenilenin aksine başarıyla öğrenerek, deneyimli nefropatologlarla yarışan bir performans sergilemiştir. Elde edilen bulgular, yüksek maliyetli dijital patoloji altyapılarına alternatif olarak ışık mikroskoplarına entegre edilebilen düşük maliyetli sistemlerin, patologlar için güvenilir bir“ikinci göz”ve karar destek mekanizması olarak kullanılabileceğini kanıtlamaktadır. Bu yaklaşım, benzer morfolojik zorluklar barındıran diğer patolojilerin tanısı için de bir yol haritası sunmaktadır.

Aim: The primary aim of this study is to develop an artificial intelligence model capable of detecting amyloid deposition in H&E stained sections of primarily kidney and various other tissue biopsies. We aimed to overcome the diagnostic difficulty caused by the amorphous (patternless) structure of amyloid deposition, thereby creating a roadmap and demonstrating feasibility for the use of artificial intelligence in detecting the morphology of other pathologies. In order to test the clinical reliability of the model on a realistic ground, performance results were compared with other major models and the evaluations of real medical pathologists on the same test set. Materials and Methods: The study included 83 amyloid-positive and 83 amyloid- negative kidney biopsies, which included lesions creating differential diagnosis difficulties, selected from the Ankara University Medical Pathology Archive, as well as 41 tissue samples from different tissues for testing purposes (total 207 cases). As an imaging method, to pave the way for future trinocular camera integrated systems and to increase accessibility as a screening test, imaging via smartphone (iPhone 15) from the ocular of a standard light microscope (Olympus BX50) was used, and digital images were obtained at 400x magnification. To overcome the diagnostic challenge posed by the patternless (amorphous) and heterogeneous structure of amyloid depositions, a novel deep learning architecture (helloAmyloid v2) utilizing multi-scale feature extraction and learnable pooling (GeM) was developed instead of standard architectures. The model was tested comparatively with detailed performance metrics against 9 medical pathologists with different levels of experience and other major artificial intelligence models. Results: The developed novel model (helloAmyloid v2) achieved a reproducible 0.949 ROC-AUC and 0.895 F1-score at the image level in kidney biopsies. Other performance metrics of the model were determined as 0.889 accuracy, 0.906 sensitivity, and 0.870 specificity. At the case level, it reached 0.986 ROC-AUC and 100% specificity in isolated trials, demonstrating superior performance compared to the tested standard transfer learning models (EfficientNet, DenseNet, etc.). In the comparative analysis performed on the same external validation set (without augmentation), the model outperformed all medical pathologist groups consisting of experienced nephropathologists (83.80%), specialists (78.70%), and residents (69.48%) with 4 an overall accuracy rate of 90.09%. Even when the optimum threshold was used, the model (91.62%) proved screening safety by surpassing even the best-performing nephropathologist (88.83%) in terms of sensitivity. In tests performed on different tissue types, while the model could largely maintain its sensitivity thanks to its architecture, a significant decrease in specificity was observed due to the absence of other tissues in the training set. Conclusions: This study demonstrated that the integration of smartphone-based imaging and deep learning has high feasibility for screening and diagnostic purposes even in the detection of pathologies with amorphous and heterogeneous structures such as amyloid. The developed novel model successfully learned patternless deposition features, contrary to expectations, and exhibited performance rivaling experienced nephropathologists. The obtained findings prove that low-cost systems that can be integrated into light microscopes as an alternative to high-cost digital pathology infrastructures can be used as a reliable“second eye”and decision support mechanism for pathologists. This approach offers a roadmap for the diagnosis of other pathologies harboring similar morphological challenges.
