Özet
Bu çalışmanın amacı, acil serviste çalışan hemşirelerin deliryum yönetimine ilişkin farkındalıkları ve deneyimlerini belirlemeyi amaçlamaktadır. Araştırma tanımlayıcı kalitatif tipte bir araştırmadır. Çalışma Türkiye'nin İzmir ilinde bir üniversite hastanesinin yetişkin acil servisinde yürütülmüştür. Araştırmada amaçlı örneklem yöntemi kullanılmıştır. Araştırmanın örneklemini en az altı aydır acil serviste çalışan ve deliryum tanısı almış bireylere bakım vermiş olan araştırmaya katılmaya gönüllü 14 hemşire oluşturmuştur. Derinlemesine görüşmeler yarı yapılandırılmış görüşme formu kullanılarak gerçekleştirilmiştir. Görüşmeler içerik analizi yöntemi ile analiz edilmiştir. Araştırmanın sonucunda üç tema ortaya çıkmıştır. Bu temalar; deliryuma giren bireye bakım vermek, deliryum yönetiminde ki engeller ve acil serviste deliryumun yönetilmesi için gereksinimlerdir. Deliryuma giren bireye bakım verme temasının altında deliryuma giren bireyi fark etme, deliryum yönetimi ve deliryum yönetiminde yaşanan duygular olarak üç alt tema ele alınmıştır. Deliryum yönetiminde ki engeller; acil servisin fiziksel ortamı ile ilgili yaşanan problemler, hastane sistemi ile ilgili engeller ve sağlık personeli ile ilgili engeller olmak üzere üç alt tema halinde yer almaktadır. Deliryumun yönetilebilmesi için gereksinimler temasında ise hemşirelerin sisteme, acil servisin fiziksel ortamına ve sağlık çalışanlarına yönelik beklentileri yer almaktadır. Acil servis hemşirelerinin deliryum yönetimine ilişkin acil servisin fiziksel ortamı ve buna bağlı iş yükünün fazla olması, hastane kaynaklı sorunlar, sağlık çalışanlarının sayısının acil servise başvuran hastalara oranla yetersiz olması ve hemşirelerin bilgi eksikliği gibi birçok olumsuz engelle karşı karşıya kaldığı görülmektedir. Bu bağlamda deliryum yönetimine ilişkin varolan farkındalığı korumak, deliryum yönetimini olumsuz etkileyen problemleri çözmek ve geliştirmek önemlidir.

In this study, it was aimed to determine the awareness and experience of the nurses working in the emergency department regarding delirium management. This study designed as a descriptive qualitative research and was conducted in the adult emergency department of a university hospital in Izmir, Turkey. Purposive sampling method was used in this study. The sample of the study consisted of 14 nurses who had been working in the emergency department for at least six months and had given care to individuals diagnosed with delirium, who volunteered to participate in the study. In-depth interviews were conducted using a semi-structured interview form. The interviews were analyzed with the content analysis method. Three themes emerged as a result of the study. These themes are care for the individual with delirium, barriers to delirium management, and requirements for delirium management in the emergency department. Under the theme of giving care to the individual in delirium, three sub-themes as recognizing the individual in delirium, delirium management, and the emotions experienced in delirium management were discussed. Barriers in delirium management; problems related to the physical environment of the emergency department, obstacles related to the hospital system and obstacles related to health personnel. In the theme of requirements for the management of delirium, nurses' expectations for the system, the physical environment of the emergency room and health workers are included. It is seen that emergency department nurses face many negative obstacles related to delirium management, such as the physical environment of the emergency room and the associated workload, hospital-related problems, the insufficient number of health workers compared to the patients who applied to the emergency service, and the lack of knowledge of the nurses. In this context, it is important to maintain the existing awareness about delirium management, and to solve and improve the problems that negatively affect delirium management.
