Özet

Giriş ve Amaç: Bu retrospektif kohort çalışmasının amacı, lokal ileri evre rektum kanseri (LARC) tanılı hastalarda, neoadjuvan kemoradyoterapi (NAKRT) ve total neoadjuvan tedavi (TNT) yaklaşımlarının sağkalım ve tedavi yanıtı üzerindeki etkilerini incelemektir. Ayrıca, nüks ve metastaz riski yüksek olan hastaları tespit etmek, optimal tedaviyi planlama konusunda fayda sağlayacak prognostik faktörleri belirlemek ve LARC tanısı almış hastalarda daha kişiselleştirilmiş ve hedefe yönelik tedavi stratejilerinin geliştirilmesine katkı sağlamak amaçlanmıştır. Gereç ve Yöntem: Ocak 2016- Aralık 2023 tarihleri arasında LARC tanısı alan hastalar multidisipliner tümör konseyinde değerlendirilmiştir. cT3N0-1M0 olan hastalara NAKRT uygulanırken; T4N0-2M0, tümör yükü fazla ve yüksek riskli (N2, EMVI+, MRF+, bulky hastalık, tümör depoziti, budding) olan hastalara TNT uygulanmıştır. NAKRT kolunda KT ile eş zamanlı uzun süreli RT± adjuvan KT şeması uygulanırken; TNT kolunda ise KT ile eş zamanlı uzun süreli RT + indüksiyon/konsolidasyon KT uygulanmıştır. Hastaların tedavi öncesi ve sonrasında klinik, patolojik ve radyolojik verileri karşılaştırılarak tedavi yanıtları ve sağkalım sonuçları analiz edilmiştir. Bulgular: Çalışmaya, LARC tanısı almış 171 hasta dahil edilmiştir. Hastaların 95(%55,6)'ine NAKRT uygulanırken; 76(%44,4)'sına TNT uygulanmıştır. Median takip süresi 44,92±22,00 aydır. TNT uygulanan hasta grubunda NAKRT' ye kıyasla OS oranları anlamlı olarak daha yüksek bulunmuştur (p=0.0489). R0 cerrahi, LVI negatifliği, PNI negatifliği ve düşük ypT evresine sahip olma OS için olumlu prognostik faktörler olarak saptanmıştır. pCR insidansı TNT kolunda %39,5 iken, NAKRT kolunda %30,5 olarak bulunmuştur. PNI negatif, LVI negatif ve grade 1-2 olan tümörlerde pCR' nin daha yüksek olduğu gösterilmiştir. TNT ile pCR ve/veya cCR elde edilen hastalarda, pCR/cCR olmayan hastalara kıyasla istatistiksel anlamlı olarak daha iyi DFS ve DMFS gösterilmiştir (sırasıyla p=0,018/p=0,030 ve p=0,007/0,003). TNT ve NAKRT ile benzer LRFS elde edilirken; pCR elde edilen hastalarda tedavi modalitesinden bağımsız olarak daha uzun LRFS saptanmıştır. PNI ve/veya LVI negatif olması; OS, DFS, LRFS, DMFS, pCR ve cCR'yi istatistiksel anlamlı olarak artırmış olup olumlu prognostik faktörler olarak tespit edilmiştir. Sonuç: TNT, LARC tanılı hastalarda NAKRT' ye kıyasla OS avantajı sağlamaktadır; özellikle LVI ve PNI negatif olan, düşük ypT evresine sahip ve R0 cerrahi uygulanan hastalarda bu avantaj daha belirgin hale gelmektedir. pCR elde edilen hastalarda daha uzun DFS, DMFS ve LRFS saptanmış olup, pCR elde edilmesinin prognozu olumlu etkilediği gösterilmiştir. Ayrıca, PNI ve/veya LVI negatif olması tüm sağkalım analizleri (OS, DFS, LRFS, DMFS) ve tedavi yanıtı için olumlu prognostik faktörler olarak tespit edilmiştir. Gelecekte yapılacak prospektif çalışmalara ihtiyaç olmakla birlikte, mevcut veriler TNT 'nin klinik pratiğe entegrasyonunu desteklemektedir.

Introduction and Objective : This retrospective cohort study aimed to evaluate the effects of neoadjuvant chemoradiotherapy (NACRT) and total neoadjuvant therapy (TNT) on survival and treatment response in patients with locally advanced rectal cancer (LARC). Additionally, we realized to identify the patients with high risk characteristics of recurrence and metastasis, explore prognostic factors useful for planning optimal treatment, and contribute to the development of personalized treatment and targeted strategies for LARC management. Materials and methods: Patients diagnosed with LARC between January 2016 and December 2023 were evaluated by a multidisciplinary tumor council. Those with T3N0-1M0 received NACRT, while patients with T4N0-2M0 and high-risk features (N2, EMVI+, MRF+, bulky disease, tumor deposits, or budding) received TNT. The NACRT group underwent long-course fractionated radiotherapy (RT) with concurrent chemotherapy (CT), with or without adjuvant CT. The TNT group received long-course RT with concurrent CT and additional induction or consolidation CT. Treatment responses and survival outcomes were analyzed by comparing clinical, pathological, and radiological data before and after the treatment. Results: A total of 171 patients with LARC were included in the study, with 95 (55.6%) receiving NACRT and 76 (44.4%) undergoing TNT. The patients treated with TNT were associated with significantly higher overall survival (OS) rates (p=0.0489) compared to NACRT. Positive prognostic factors for OS included R0 resection, absence of lymphovascular invasion (LVI), absence of perineural invasion (PNI), and low ypT stage (p<0.05). The pathological complete response (pCR) rate was 39.5% in the TNT group and 30.5% in the NACRT group. Higher pCR rates were observed in tumors with no PNI, no LVI, and grade 1-2 differentiation. Patients achieving pCR or clinical complete response (cCR) with TNT had significantly better disease-free survival (DFS) and distant metastasis-free survival (DMFS) compared to those without pCR/cCR (p=0.018/p=0.030 and p=0.007/0.003, respectively). While local recurrence-free survival (LRFS) was similar between TNT and NACRT, patients with pCR demonstrated prolonged LRFS regardless of treatment modality. Negative PNI and/or LVI significantly improved OS, DFS, LRFS, DMFS, pCR, and cCR rates and were identified as favorable prognostic factors. Conclusion: TNT provides a survival advantage over NACRT in patients with LARC, particularly in those with LVI-negative, PNI-negative, and low ypT stage, as well as those undergoing R0 resection. Achieving pCR is associated with improved DFS, DMFS, and LRFS, leading to a more favorable prognosis. Negative PNI and/or LVI were found to be favorable prognostic factors across all survival and treatment response metrics. While prospective studies are necessary to confirm these findings, current data support the incorporation of TNT into clinical practice for high-risk LARC patients.
