Özet

Amaç: HINTS Plus testi, acil servise saatler-günler süren baş dönmesi şikayeti ile başvuran ve vertigo ön tanısı düşünülen hastaların periferik ve santral vertigo ayırıcı tanısında kullanılan değerli bir araçtır. Güncel olarak acil servislerde santral vertigo tanısında başta beyin BT ve MRG sık olarak kullanılmaktadır. Biz bu çalışmada HINTS Plus testinin periferik ve santral vertigo ayırıcı tanısındaki duyarlılığını ve özgüllüğünü ortaya koymayı amaçladık. Bu sayede görüntüleme oranlarının düşürüleceğini ve tanı süreçlerinin hızlanacağını düşünmekteyiz. Gereç ve Yöntem: Çalışma Ankara Bilkent Şehir Hastanesi Acil Tıp Kliniği acil servisinde 01.07.2023-01.07.2024 tarihleri arasında gerçekleştirildi. Vertigo ön tanısıyla ileri tetkik planlanan hastalara araştırmacılar tarafından HINTS Plus testi uygulandı. Rutin tetkikler olan kan ve referans test olarak nörogörüntüleme yöntemleri olan BT ve MRG kullanıldı. Çalışmaya 18 yaş üstü, bilinci açık, çalışmaya katılmayı kabul eden, HINTS Plus testi için koordine olabilecek, hemodinamik olarak stabil, acil servise baş dönmesi şikayeti ile başvurup herhangi bir nörolojik defisiti olmayan (konuşma bozukluğu, motor güç kaybı, fasiyal asimetri, his kaybı) hastalar dahil edildi. Dahil edilen hastalar için görevli olan nöbetçi hekim tarafından HINTS Plus testi gerçekleştirildi ve sonuçları olgu rapor formuna (EK-2) kaydedildi. Bulgular: Baş dönmesi şikayeti ile başvuran ve vertigo tanısı alan 298 hasta tespit edildi. Referans test sonucuna göre bu hastaların 236'sı periferik, 62'si santral vertigo olarak tanı aldı. HINTS Plus testi sonucuna göre hastaların 207'si periferik, 91'i santral vertigo olarak tespit edildi. Referans tanı ve indeks test olan HINTS Plus testine dair 2*2 çapraz tablo oluşturulup tanısal değerlilik analizi yapıldı. Bu değerlere göre HINTS Plus testinin santral vertigo tanısını koyabilmesi açısından duyarlılığı %80,6, özgüllüğü %82,63, pozitif olabilirlik oranı 4,64, negatif olabilirlik oranı 0,23, pozitif prediktif değer %54,95, negatif prediktif değer %94,2 ve doğruluk %82,21 olarak hesaplandı Sonuç: Çalışmamız HINTS Plus testi uygulanan ve test sonucu periferik vertigo lehine çıkan hastalarda santral nedenlerin tam olarak ekarte edilemediğini göstermiştir. Bu test santral vertigonun ekarte edilmesinde değil sonucun santral vertigo lehine çıkması durumunda ayrıntılı inceleme gerekliliğinin vurgulanması için kullanılması daha uygun olacaktır. Test sonucu santral vertigo olan hastalarda yakın zamanlı inme riskine karşı gözlem altında uygun tedavi verilmesi, gerekirse yakın zamanlı kontrol amaçlı görüntüleme seçeneklerinin kullanılmasının gerektiğini göstermiştir. HINTS Plus testinin özellikle tecrübeli acil servis hekimleri tarafından yapılması, sonuçlar yorumlanırken hasta uyumunun dikkate alınması ve göz hareketlerinin tespiti için kayıt cihazlarının kullanılması ile daha kaliteli sonuçlar elde edilebilir.

Aim: The HINTS Plus test is a valuable tool for differentiating between peripheral and central vertigo in patients presenting to the emergency department with vertigo as a preliminary diagnosis and complaints of dizziness lasting hours to days. Currently, brain CT and MRI are commonly used in emergency departments for the diagnosis of central vertigo. In this study, we aimed to determine the sensitivity and specificity of the HINTS Plus test in distinguishing peripheral from central vertigo. We believe that its use may reduce imaging rates and accelerate diagnostic processes. Materials and Methods: This study was conducted in the Emergency Medicine Department of Ankara Bilkent City Hospital between July 1, 2023, and July 1, 2024. The HINTS Plus test was performed on patients with a preliminary diagnosis of vertigo who were scheduled for advanced diagnostic evaluation. Routine tests, including bloodwork and neuroimaging methods such as CT and MRI, were used as reference standards. Patients aged 18 years and older who were conscious, agreed to participate in the study, were able to cooperate for the HINTS Plus test, were hemodynamically stable, and presented to the emergency department with dizziness without any neurological deficits (speech disturbances, motor weakness, facial asymmetry, or sensory loss) were included. The HINTS Plus test was performed by the on-duty physician and the results were recorded in the case report form (Ek:2). Results: A total of 298 patients presenting with dizziness and diagnosed with vertigo were identified. According to the reference test results, 236 patients were diagnosed with peripheral vertigo and 62 with central vertigo. Based on the HINTS Plus test, 207 patients were identified with peripheral vertigo and 91 with central vertigo. A 2x2 cross-table was created for the reference diagnosis and the index test (HINTS Plus) to conduct diagnostic validity analysis. According to these results, the HINTS Plus test had a sensitivity of 80.6%, specificity of 82.63%, positive likelihood ratio of 4.64, negative likelihood ratio of 0.23, positive predictive value of 54.95%, negative predictive value of 94.2%, and accuracy of 82.21% in diagnosing central vertigo. Conclusion: Our study demonstrated that in patients whose HINTS Plus test results indicated peripheral vertigo, central causes could not be fully ruled out. The test may be better suited for emphasizing the need for detailed investigation when the result suggests central vertigo, rather than solely excluding central vertigo. For patients with test results indicative of central vertigo, appropriate treatment and close observation should be provided to mitigate the risk of recent stroke, and follow-up imaging options may be required. To achieve higher-quality results, the HINTS Plus test should ideally be performed by experienced emergency physicians, with attention to patient compliance and the use of recording devices to capture eye movements.
