Özet

Giriş: Meme kanseri kadınlarda en sık görülen kanserdir. Bu nedenle erken tanı kritik öneme sahiptir ve tarama programları da erken tanıda büyük rol oynamaktadır. COVID-19 pandemisi döneminde, meme kanseri tedavi algoritmalarında bazı değişiklikler gözlendi. COVID-19 salgınının başlangıcında, birçok sağlık hizmeti gibi kanser tarama programları da büyük ölçüde ertelenmek zorunda kaldı. Hastaların sağlık kurumlarına başvuramamaları ve taramaların ertelenmesi, tanı ve tedavi süreçlerinde gecikmelere neden oldu. Biz de çalışmamızda COVID-19 pandemisinin meme kanserli hastalarda sebep olduğu değişimleri ortaya koymayı amaçladık. Gereç ve yöntem: Çalışmamızda S.B.Ü. Ankara Eğitim ve Araştırma Hastanesi Genel Cerrahi Kliniğinde 10 Ocak 2017-10 Mart 2020 tarihleri ile (COVID-19 öncesi), 11 Mart 2020- 11 Mart 2022 tarihleri aralığında (COVID-19 dönemi), 18-100 yaş arası ve cinsiyet fark etmeksizin meme kanseri nedeniyle servise yatırılarak ameliyat edilen hastalar retrospektif olarak incelendi. Bu veriler arasında meme kanseri tanısı alan 158 hastadan; aynı memeden biyopsi ve/veya cerrahi hikayesi olanlar, meme kanseri tanısı ile ameliyat olmuş olanlar, patolojisi duktal karsinoma in situ ve lobüler karsinoma in situ olanlar dışında kalan hastalar çalışmaya dahil edildi. Her iki grup için demografik özellikler, tümör özellikleri, patolojik ve cerrahi özellikleri incelendi. Bulgular: Hastaların COVID-19 öncesi ve COVID-19 dönemine göre yaptığımız çalışmamızda; tanı tarihi ile ameliyat tarihi arasında geçen ortalama sürenin COVID-19 öncesi dönemde 69,2±78,8 gün, COVID-19 döneminde ise 137,9±111,5 gün olduğu tespit edildi (p<0.05). COVID-19 öncesi hastaların %32'sinde (n:16) 2 cm üzeri tümör olduğu, COVID-19 dönemindeki hastaların %52'sinde (n:26) 2 cm üzeri tümör olduğu ve pandemi döneminde tümör büyüklüğünün arttığı tespit edildi (p<0.05). COVID-19 döneminde uzak metastaz oranlarında belirgin artış olduğu tespit edildi. COVID-19 döneminde HER 2 pozitifliğinde anlamlı bir düşüş olduğu tespit edildi (p<0.05). Hastaların preoperatif lenf nodu değerlendirmesinde pandemi döneminde biyopsi ile malign tanı konulan hasta sayısında anlamlı bir artış olduğu tespit edildi (p<0.05). COVID-19 döneminde neoadjuvan kemoterapi alan hasta sayısında COVID-19 öncesi döneme göre istatistiksel olarak anlamlı bir artış olduğu tespit edildi (p<0.05). Çıkarılan toplam lenf nodu sayısının pandemi döneminde azaldığı fakat metastaz tespit edilen lenf nodu sayısının yine aynı dönemde arttığı tespit edildi. Hastaların ortalama yaş, cinsiyet, östrojen ve progesteron reseptörü, mitoz skoru, Ki-67 indeksi, nükleer grade, tümör alt tipleri ve odakları, yapılan ameliyat çeşitleri açısından gruplar arasında istatistiksel olarak anlamlı fark olmadığını tespit ettik. Yine pandemi dönemi vaka sayılarının artış ve azalış dönemlerine göre gruplandırıp kıyasladığımızda anlamlı bir fark olmadığı tespit edildi. Sonuç: COVID-19 pandemisi nedeni ile hastane işleyişlerinin bozulması tanı tarihi ile ameliyat tarihi arasında geçen süre arasında belirgin artışa neden olduğu ve hastaların tarama programlarını aksatması gibi sebeplerle de tümör boyutunda önceki döneme kıyasla belirgin artışa neden olduğu görülmektedir. Cerrahi işlemleri ertelemek veya hastaların hastanede kalış sürelerini azaltmak için neoadjuvan tedavinin bu süreçte önceki döneme kıyasla daha fazla tercih edildiği görüldü. Bu ve buna benzer olası bir pandemi sürecinde hastanelerin ve özellikle poliklinik hizmetlerinin ulaşılabilirliği arttırılmalı ve tarama programlarının kanser hastalarının evre ve prognozunda ne kadar önemli olduğu hakkında halk bilinçlendirilmelidir.

Introduction: Breast cancer is the most common cancer in women. Therefore, early diagnosis is critical and screening programs play a major role in early diagnosis. During the COVID-19 pandemic, some changes were observed in breast cancer treatment algorithms. At the beginning of the COVID-19 pandemic, cancer screening programs, like many other health services, had to be postponed to a great extent. The inability of patients to apply to health institutions and the postponement of screenings caused delays in diagnosis and treatment processes. In our study, we aimed to reveal the changes caused by the COVID-19 pandemic in patients with breast cancer. Materials and Methods: In our study, we retrospectively analyzed the patients who were hospitalized and operated in the General Surgery Clinic of S.B.U. Ankara Training and Research Hospital between January 10, 2017 and March 10, 2020 (pre-COVID-19 period) and between March 11, 2020 and March 11, 2022 (COVID-19 period) for breast cancer between the ages of 18-100 years and regardless of gender. Among the 158 patients diagnosed with breast cancer, patients with a history of biopsy and/or surgery from the same breast, patients who had undergone surgery for breast cancer, and patients with pathology excluding ductal carcinoma in situ and lobular carcinoma in situ were included in the study. Demographic characteristics, tumor characteristics, pathological and surgical features were analyzed for both groups. Results: In our study of the patients before and during the COVID-19 period, it was determined that the mean time between the date of diagnosis and the date of surgery was 69.2±78.8 days in the pre-COVID-19 period and 137.9±111.5 days in the COVID-19 period (p<0.05). It was determined that 32% (n:16) of the patients before COVID-19 had tumors over 2 cm, 52% (n:26) of the patients in the COVID-19 period had tumors over 2 cm and the tumor size increased during the pandemic period (p<0.05). There was a significant increase in distant metastasis rates during the COVID-19 period. There was a significant decrease in HER 2 positivity during the COVID-19 period (p<0.05). In the preoperative lymph node evaluation of the patients, it was determined that there was a significant increase in the number of patients diagnosed as malignant by biopsy during the pandemic period (p<0.05). There was a statistically significant increase in the number of patients receiving neoadjuvant chemotherapy during the COVID-19 period compared to the pre-COVID-19 period (p<0.05). The total number of lymph nodes decreased statistically during the pandemic period, but the number of metastatic lymph nodes increased in the same period. We found that the mean age, gender, estrogen and progesterone receptors, mitosis score, Ki-67 index, nuclear grade, tumor subtypes and foci, and types of surgeries performed were not statistically significant. Again, when we grouped and compared the pandemic period according to the periods of increase and decrease in the number of cases, we found that there was no significant difference. Conclusion: It is seen that the disruption of hospital functioning due to the COVID-19 pandemic caused a significant increase in diagnosis and operation times and caused a significant increase in tumor size compared to the previous period due to reasons such as patients disrupting screening programs. It was observed that neoadjuvant treatment was preferred more in this period compared to the previous period in order to postpone surgical procedures or to reduce the length of hospitalization of patients. In a possible pandemic like this one, the accessibility of hospitals and especially outpatient services should be increased and the public should be made aware of the importance of screening programs in the stage and prognosis of cancer patients.
