Özet

Amaç: Astım tüm dünyada ve ülkemizde kronik solunum yolları hastalıkları arasında en sık başvuru nedenlerinden biridir. Bir solunum yolu patojeni olan SARS-CoV-2'nin neden olduğu COVID-19 pandemisi sürecinde astım hastalarında hastalık seyrinde yaşanan değişiklikler önemli olmuştur. Bu çalışma ile astım hastalarının COVID-19 pandemisi dönemindeki hastalık kontrolü ve yönetimine ait değişiklikleri değerlendirmek amaçlanmıştır. Gereç ve yöntem: Çalışmaya 25.04.2023-25.08.2023 tarihleri arasında Ankara Eğitim ve Araştırma Hastanesi Göğüs Hastalıkları Polikliniklerine ayaktan başvuran, Mart 2020 öncesi astım hastalığı tanısı olan ve 18-80 yaş arası 333 hasta dahil edildi. Katılımcılara sosyodemografik özellikleri ile astım hastalığının pandemi öncesi ile şu anki seyrini tanımlayacak veri formu ve Astım Kontrol Testi (AKT) yüz yüze görüşme yöntemi ile uygulandı. Hastalara kontrolleri esnasında yapılan solunum fonksiyon testi sonuçları da çalışmaya dahil edildi. İstatistiksel analizler IBM SPSS 26.0 programı ile yapıldı. p<0,05 değeri istatistiksel olarak anlamlı kabul edildi. Bulgular: Katılımcıların %17,4'ü (n=58) erkek, %82,6'sı (n=275) kadın olup, genel yaş ortalaması 52,22±14,75 yıl (min=18, maks=80, ortanca=53) olarak hesaplandı. Hastaların %62,2'sinin (n=207) pandemide astım nedeniyle hastaneye başvuru sıklığı azalmıştı. %48,6'sının (n=162) pandemi öncesine göre sonrasında astım semptomu artmıştı. Yine %48,9'unun (n=163) pandemi öncesine göre sonrasında astım nedeniyle günlük faaliyet kısıtlaması artmıştı. COVID-19 geçirme oranı %44,4 (n=148), COVID-19 sebebiyle hastaneye yatış oranı %22,3'tü (n=33). Katılımcıların AKT puanı ortalaması 16,98±4,36 (min=6, maks=25) olarak hesaplandı. Buna göre %66,7'sinin (n=222) astımının kontrol altında olmadığı, %30,9'unun (n=103) kısmi kontrolde olduğu, %2,4'ünün (n=8) ise tam kontrolde olduğu görüldü. Pandemi sonrası semptomu ve atak sıklığı artanların astımı semptomları ve atak sıklığı azalan veya fark olmayanlara göre daha kontrolsüzdü (p<0,001). Pandemi sonrasında astım kontrolü kötü olanlarda pandemi öncesinde iyi olma sıklığı orta ve kötü olma sıklığından, pandemi öncesinde orta olma sıklığı kötü olma sıklığından yüksekti ve bu fark istatistiksel olarak anlamlıydı (p<0,001). Sonuç: Katılımcıların büyük kısmında astım kontrol altında değildi. Pandemi döneminde hastane başvuru sıklığı azalanlarda ve komorbid hastalığı olanlarda astım daha kontrolsüzdü. Pandemiden sonra astım semptomları ve buna bağlı olarak günlük faaliyetlerdeki kısıtlılık artmıştı. Pandemi sonrası dönemde astım semptomları, atak sayısı ve gece uyanma sıklığı artan hastalarda astım daha kontrolsüzdü. Çalışmamızda COVID-19 geçirmenin astım hastalığının kontrolünü değiştirmediği sonucuna ulaşıldı.

Aim: Asthma is one of the most common causes of chronic respiratory diseases all over the world today. During the COVID-19 pandemic caused by SARS-COV-2, a respiratory pathogen, changes in the course of the disease in asthma patients have been important. This study does not take into account asthma patients' assessments of deterioration in disease control and management during the COVID-19 pandemic. Materials and methods: The study included 333 outpatients who applied to Ankara Training and Research Hospital Chest Diseases Polyclinics between 25.04.2023 and 25.08.2023, who were diagnosed with asthma before March 2020 and who were between the ages of 18-80. The data form and Asthma Control Test (ACT), which will describe the sociodemographic characteristics of the participants and the pre-pandemic and current course of asthma, were administered to the participants by face-to-face interview method. Respiratory function test results performed on the patients during their follow-up were also included in the study. Statistical analyzes were performed with IBM SPSS 26.0 program. A value of p<0.05 was considered statistically significant. Results: 17.4% (n=58) of the participants were male, 82.6% (n=275) were female, and the overall average age was 52.22±14.75 years (min=18, max=80, median= It was calculated as 53). The frequency of hospital admission due to asthma decreased for 62.2% (n=207) of the patients during the pandemic. 48.6% (n=162) had increased asthma symptoms after the pandemic compared to before. Again, 48.9% (n=163) had increased daily activity restrictions due to asthma compared to before the pandemic. The rate of COVID-19 was 44.4% (n=148), and the rate of hospitalization due to COVID-19 was 22.3% (n=33). The average ACT score of the participants was calculated as 16.98±4.36 (min=6, max=25). Accordingly, it was seen that 66.7% (n=222) had their asthma under control, 30.9% (n=103) had partial control, and 2.4% (n=8) had full control. The asthma of those whose symptoms and attack frequency increased after the pandemic was more uncontrolled than those whose symptoms and attack frequency decreased or had no difference (p<0.001). Among those with poor asthma control after the pandemic, the frequency of being well before the pandemic was higher than the frequency of being moderate or worse, and the frequency of being moderate before the pandemic was higher than the frequency of being poor, and this difference was statistically significant (p<0.001). Conclusion: Asthma was not under control in the majority of participants. Asthma was more uncontrolled in those with reduced frequency of hospital admissions and those with comorbid diseases during the pandemic period. After the pandemic, asthma symptoms and the resulting limitation in daily activities increased. In the post-pandemic period, asthma was more uncontrolled in patients whose asthma symptoms, number of attacks, and frequency of waking up at night increased. Our study concluded that having COVID-19 did not change the control of asthma disease. Again, the asthma of those whose daily activities were restricted due to asthma was more uncontrolled than before the pandemic. Our study concluded that having COVID-19 did not change the control of asthma disease. Patients stated that their asthma got worse after the pandemic.
