Özet

Amaç: Çalışmamızda cinsel istismar mağduru olup Akomodasyon Sendromu tanısı alan çocukların psikiyatrik, psikososyal, adli süreç ve sonrasında oluşan psikopatolojileri Akomodasyon Sendromu olmayan, yaş ve cinsiyet açısından eşleştirilmiş kontrol grubu ile karşılaştırılarak değerlendirildi. Gereç ve Yöntem: Çalışmamızda Ocak 2012-Şubat 2023 tarihleri arasında Adli Olguları Değerlendirme Heyeti'nde değerlendirilen, Akomodasyon Sendorumu tanısı alan 2-18 yaş aralığındaki 61 kız ve 10 erkek 71 olgu ve 55 kız 12 erkek 67 kontrol grubu olmak üzere toplam 138 cinsel istismar olgusu retrospektif olarak araştırıldı. Bulgular: Çalışmamıza katılan olguların ilk kez istismara maruz kalma yaşı ortalama 11,13'idi. İfadesini geri çeken grupta olguların % 5,7'ünde annede ruhsal hastalık öyküsü olduğu, % 29,6'sında ailesinde boşanma/parçalanma öyküsü olduğu, % 77,4'ünün ayrı odasının olmadığı, % 51,6 olgunun ayrı yatağının olmadığı, % 17,6'sının ebeveyn tarafından şiddete maruz bırakıldığı, % 20'sinin hem cinsel hem fiziksel istismara uğradığı, % 30'unun ihmale uğradığı, % 36,2'sinde penetrasyon olduğu, olgu sayısı 48 %69,6'sında cinsel istismarın tekrarladığı, % 61,3'ünde istismara maruz bırakan kişinin aile içi olduğu, % 80,3'ünde istismara eşlik eden alıkoyma, zor kullanma ve tehdit gibi bir durum olduğu, % 70,4'ünde gizleme, % 77,5'inde yardım arama görüldüğü, % 71,1'inde eğitim aksaması, çevre baskısı, aileden ayrılma gibi sosyal sonuçlardan en az birinin olduğu, % 80,3'ünün ailesinin cinsel istismarı gizlediği, % 55,7'sinin ailesinin yardım aradığı, % 68,6'sının ailesinde suçlama/baskı yapma görüldüğü bulundu. Sonuç: Akomodasyon Sendromunda cinsel istismar tekrarı, istismarcının aile içinden olması, istismar olayını mağdurun ve ailesinin gizlemesi, mağdurların yardım aramaması, çevrenin ve ailenin mağdura baskı yapması, ailenin mağdura yardım etmemesi anlamlı bir şekilde yüksek olduğu saptandı.

Objective: In our study, psychiatric, psychosocial, forensic process and subsequent psychopathologies of children who were victims of sexual abuse and diagnosed with Accommodation Syndrome were evaluated in comparison with an age and gender matched control group without Accommodation Syndrome. Materials and Methods: In our study, a total of 138 sexual abuse cases (61 girls and 10 boys aged 3-18 years, 71 cases diagnosed with Accommodation Syndrome and 67 control group (55 girls and 12 boys) between the ages of 3-18 years, who were evaluated by the Forensic Case Evaluation Committee between January 2012 and February 2023, were investigated retrospectively. Results: The mean age of the subjects who participated in our study at the time of first exposure to abuse was 11,13. In the group who withdrew their statements, 5.7 % of the cases had a history of mental illness in the mother, 29.6 % had a history of divorce/separation in the family, 77.4 % had no separate room, 51.6 % had no separate bed, 17, 6% were exposed to parental violence, 20% were subjected to both sexual and physical abuse, 30% were neglected, 36.2% were penetrated, and sexual abuse was repeated in 69.6% of the 48 cases, 61.3 % of the children reported that the abuser was a member of the family, 80.3 % reported that the abuse was accompanied by detention, use of force and threats, 70.4 % reported concealment, 77.5 % reported seeking help, 71.1 % reported disruption of education, at least one of the social consequences such as environmental pressure, separation from the family, 80.3 % of the children's families concealed the sexual abuse, 55.7 % of the children's families sought help, 68.6 % of the children's families blamed/pressured them. Conclusion: Repetition of sexual abuse, abuser being a member of the family, concealment of the abuse by the victim and her family, victims not seeking help, pressure from the environment and family on the victim, and family not helping the victim were found to be significantly higher in accommodation syndrome.
