Özet
Bu araştırma, COVID-19 pandemisi sürecinde aktif olarak çalışan hemşirelerde vicdan algısı, vicdan stresi ve tükenmişlik arasındaki ilişkinin incelenmesi amacıyla tanımlayıcı, kesitsel ve ilişki arayıcı olarak yapılmıştır. Araştırmanın örneklemini Türkiye sınırları içinde pandemi sürecinde COVID-19 tanılı hastaların izlendiği acil servis, yoğun bakım servisleri ve yataklı servislerinde en az 5 aydır aktif çalışan, COVID-19 pandemisi sürecinde ücretsiz izin kullanmayan, çalışmaya gönüllü olarak katılan 354 hemşire oluşturmuştur. Araştırmanın verileri Hemşire Bilgi Formu, Vicdan Algısı Ölçeği (VAÖ), Vicdan Stresi Ölçeği (VSÖ) ve Maslach Tükenmişlik Ölçeği (MTÖ) kullanılarak toplanmıştır. Verilerin değerlendirilmesinde tanımlayıcı istatistikler, Pearson korelasyon analizi, ki kare, bağımsız örneklem t testi, tek yönlü varyans analizi, Levene, Bonferroni, Tamhane's T2 testleri ile model testi için path analizi kullanılmıştır. Araştırmaya katılan hemşirelerin VAÖ puan ortalaması 60,33±12,51, VAÖ duyarlılık alt boyut puan ortalaması 52,13±12,51, VAÖ otorite alt boyut puan ortalaması 8,20±2,80, VSÖ puan ortalaması 83,85±40,64, MTÖ Duygusal Tükenme alt boyut puan ortalaması 21,19±8,33, MTÖ kişisel başarı alt boyut puan ortalaması 20,65±5,27 ve MTÖ Duyarsızlaşma alt boyut puan ortalaması 7,47±4,32'dir. VAÖ puanı ile VSÖ ve MTÖ Kişisel Başarı alt boyut puanı arasında pozitif yönde, MTÖ Duyarsızlaşma alt boyut puanı arasında negatif yönde istatistiksel olarak anlamlı ilişki bulunmaktadır (p<0,05). Path modeline göre VAÖ'nin, VSÖ ve MTÖ Kişisel Başarı alt boyut puanları üzerinde pozitif, MTÖ Duyarsızlaşma alt boyut puanları üzerinde negatif, VSÖ'nin MTÖ Duygusal tükenme, MTÖ Duyarsızlaşma alt boyut puanı üzerinde pozitif, MTÖ Kişisel Başarı alt boyut puanın MTÖ Duygusal Tükenme ve MTÖ Duyarsızlaşma alt boyut puanı üzerinde negatif bir etkisi bulunduğu belirlenmiştir. Araştırmadan elde edilen sonuçlar doğrultusunda; hemşirelerin vicdan algısına, vicdan stresine ve yaşadıkları vicdan temelli tükenmişliğe farkındalık oluşturulması, hasta bakım kalitesinin arttırılmasına yarar sağlaması açısından hemşirelerin bu konuda güçlendirilmesi ve psikolojik sağlamlıklarının desteklenmesi önerilmektedir.

This research was conducted as a descriptive, cross-sectional, and correlational study to analyze the relationship between the perception of conscience, stress of conscience, and burnout in nurses working actively during the COVID-19 pandemic. The research sample comprised 354 nurses who volunteered to participate in the study, never took unpaid leave during the COVID-19 pandemic, and were actively working for a minimum of 5 months in emergency services, intensive care services, and bedded services where patients diagnosed with COVID-19 disease were followed up within the borders of Turkey during the pandemic.The research data were collected with the Nurse Information Form, the Perceptions of Conscience Questionnaire (PCQ), the Stress of Conscience Questionnaire (SCQ), and the Maslach Burnout Inventory (MBI). Descriptive statistics, Pearson's correlation, the chi-squared test, the independent samples t-test, the one-way analysis of variance, Levene's test, the Bonferroni correction, and Tamhane's T2 test, as well as the path analysis to test the model, were used in the evaluation of research data.The means of scores obtained by participant nurses from the PCQ and its Sensitivity Sub-Scale and Authority Sub-Scale were successively 60.33±12.51, 52.13±12.51, and 8.20±2.80, next, the mean of nurses' SCQ scores was 83.85±40.64, and lastly, the means of scores obtained by nurses from the Emotional Exhaustion Sub-Scale, Personal Accomplishment Sub-Scale, and Depersonalization Sub-Scale of the MBI were respectively 21.19±8.33, 20.65±5.27,and 7.47±4.32. Nurses' PCQ scores had statistically significant positive relationships with their SCQ scores and MBI Personal Accomplishment Sub-Scale scores and a negative relationship with their MBI Depersonalization Sub-Scale scores (p<0.05).As per the Path model, it was discerned that nurses' PCQ scores had positive effects on their SCQ and MBI Personal Accomplishment Sub-Scale scores and a negative effect on their MBI Depersonalization Sub-Scale scores, next, nurses' SCQ scores had positive effects on their MBI Emotional Exhaustion Sub-Scale and Depersonalization Sub-Scale scores, and lastly, nurses' MBI Personal Accomplishment Sub-Scale scores had negative effects on their MBI Emotional Exhaustion Sub-Scale and Depersonalization Sub-Scale scores.In light of the results obtained from the research, it is recommended that nurses' awareness about the perception of conscience, the stress of conscience, and their conscience-based burnout be raised, nurses be strengthened on the topic to help the quality of patient care go up, and nurses' psychological resilience be supported.
