Özet
Nöroanestezi pratiğinde santral venöz kateterizasyon; yaygın olarak kullanılmakta, sıklıkla da vasküler infratentoryal cerrahilerde tercih edilmekte ve intrakraniyal cerrahi prosedürün spesifik tipine göre değişkenlik göstermektedir. Oturur pozisyonda intrakranial kitle ve anevrizmaya yönelik cerrahi işleme alınan hastalarda santal venöz kateterizasyonun tercihinde venöz hava embolisi riski önemli rol oynamaktadır. Ayrıca santral venöz kateterizasyon; santral venöz basınç ölçümü, perioperatif sıvı-ilaç uygulaması ve hemodinami takibi açısından da önem taşımaktadır. Erişim kolaylığı ve cerrahi sahadan uzak olması nedeniyle kliniğimizde oturur pozsiyonda kranial cerrahi yapılacak hastalarda sağ subklavian ven tercih edilmektedir. Kateter ucunun doğru pozisyonda yerleşimi; kateterin efektif kullanımı, santral venöz basıncın doğru ölçümü ve özellikle nöroanestezi pratiğinde intraoperatif venöz hava embolisi yönetimi düşünüldüğünde ayrı bir önem kazanmaktadır. Çalışmamızda oturur pozisyonda kranial cerrahi geçiren erişkin hastalarda, rutin intraoperatif subklavian ven kateterizasyon uygulamasının ardından kateter pozisyonunun kontrolünde rutin olarak kullanılan postoperatif AG (AG) ile elde edilecek verilerin taşınabilir el tipi USG ile kardiyak ve vasküler görüntülemede elde edilecek verilerle karşılaştırılması amaçlanmıştır. Çalışmamıza Selçuk Üniversitesi Tıp Fakültesi Yerel Etik Kurulu'nun 24.03.2021 tarih, 2021/06 toplantı sayılı ve 2021/160 sayılı kararı ile Temmuz 2021-Eylül 2022 arasında Selçuk Üniversitesi Tıp Fakültesi Hastanesinde oturur pozisyonda elektif beyin cerrahisi operasyonu yapılan ve subklavian ven kateteri yerleştirilen 18-85 yaş aralığında 50 hasta dahil edilmiştir. Hastaların demografik verileri, cerrahiye ait veriler, santral venöz kateterizasyon ile ilgili veriler kaydedilerek ultrasonografik değerlendirme yapılmıştır. Subkostal ve apikal dört boşluk pencerelerden hem supin pozisyonda hem oturur pozisyonda ultrasonografi gerçekleştirilmiştir. Ultrasonografi ile sağ atrium görünümü, sağ atrium içinde kateter varlığı, serum fizyolojik uygulamasını takiben sağ atriumda mikrokabarcık oluşumu değerlendirilmiştir. Postoperatif rutin alınan akciğer grafisinde karinayı anatomik işaret noktası alan 2 farklı tanımlamaya göre santral venöz kateter ucunun pozisyonu 2 gruba ayrılmıştır. Bu 2 farklı AG verileri ile ultrasonografi verileri karşılaştırılarak değerlendirilmiştir. Veriler R version 4.2.1 programı; Shapiro-Wilk testi, Q-Q grafikleri, Levene testi, DeLong's testi, t-testi, Welch'in t-testi veya Yates süreklilik düzeltmeli ki-kare testi ile değerlendirilmiş, p<0,05 anlamlı olarak kabul edilmiştir. Verilerimizi değerlendirdiğimizde kateter ucunun sağ atriumda görülüp görülmemesine ilişkin supin pozisyonda her iki pencerede elde edilen ultrasonografi ile 2 farklı tanımlamaya göre AG karşılaştırıldığında istatistiksel olarak anlamlı bulunmuştur (p=0.017, p=0.023). Her iki pozisyonda ve her iki pencerede elde edilen ultrasonografi ile 2 farklı tanımlamaya göre AG karşılaştırıldığında anlam daha da artmıştır (p=0.006, p=0.002). Kateter ucunun sağ atriumda görülüp görülmemesine ilişkin USG değerlendirme sonuçlarının yaş (supin subkostal pencere hariç), cinsiyet, VKİ, toraks ön-arka mesafesi ve cilt kateter mesafesinden bağımsız olduğu sonucuna varılmıştır. Sonuç olarak, çalışmamız santral venöz kateterizasyonda doğru kateter uç pozisyonunun değerlendirilmesinde ultrasonografinin kullanımının kolay, pratik uygulanabilir ve radyasyon içermemesi gibi avantajları olmasına rağmen AG'nin yerine geçemeyeceğini göstermektedir. Ultrasonografi ile kateter ucu değerlendirmesi yapılacak ise birkaç farklı pencereden değerlendirmeler yapılmalıdır. Santral venöz kateter yönelimini değerlendirmek için mikrokabarcık testi anlamlı görülmemiştir.

Central venous catheterization is widely used in neuroanesthesia practice. It is especially preferred in vascular infratentorial surgeries. In the sitting position, the risk of venous air embolism is important in the preference of central venous catheterization in patients undergoing surgery for intracranial mass and aneurysm. In addition, central venous catheterization is vital in central venous pressure measurement, perioperative fluid-drug administration, and hemodynamic monitoring. The right subclavian vein is preferred in patients who will undergo cranial surgery in a sitting position in our clinic due to its ease of access and distance from the surgical area. Placement of the catheter tip in the correct position is crucial in neuroanesthesia practice for effective catheter use, accurate measurement of central venous pressure, and intraoperative venous air embolism management. Chest X-ray is routinely used to control the catheter position after routine intraoperative subclavian vein catheterization in adult patients who are undergoing cranial surgery in the sitting position. This study aims to compare the data obtained via postoperative chest X-ray with those obtained in cardiac and vascular imaging via portable USG. The Local Ethics Committee approved our study of Selcuk University Faculty of Medicine on 24.03.2021 with meeting number 2021/06 and judgment number 2021/160. Between July 2021 and September 2022, 50 adult patients aged 18-85 years who underwent cranial surgical operation with subclavian vein catheterization under general anesthesia in Selcuk University Faculty of Medicine Hospital were included in the study. Demographic data of the patients, data related to surgery, data related to central venous catheterization were recorded and ultrasonographic evaluation was performed. Ultrasonography was performed from the subcostal and apical four-chambers view in both the supine and sitting positions. The appearance of the right atrium, the presence of a catheter in the right atrium, and the formation of microbubbles in the right atrium following saline administration were evaluated by ultrasonography. In addition, the position of the central venous catheter tip was divided into two groups according to 2 different definitions, which included the carina as the anatomical landmark in the postoperative chest X-ray. These two different chest X-ray data and ultrasonography data were compared and evaluated. The data were assessed using the R version 4.2.1 program; Shapiro-Wilk test, t-test, Q-Q graphs, Levene test, DeLong's test, t-test, Welch's t-test, or Yates continuity-corrected chi-square test. p<0.05 was considered significant. When the data were evaluated, it was found statistically significant when the ultrasonography obtained in both views in the supine position regarding whether the catheter tip was visible in the right atrium was compared with the chest X-ray according to 2 different definitions (p=0.017, p=0.023). The significance is further increased when the ultrasonography obtained in both positions and in both views is compared with chest X-ray according to 2 different definitions (p=0.006, p=0.002). It was concluded that the results of USG evaluation regarding the visibility of the catheter tip in the right atrium were independent of age (except for the supine subcostal view), gender, BMI, anterior-posterior thorax distance, and skin-catheter distance. In conclusion, our study shows that although using ultrasonography to evaluate the correct catheter tip position in central venous catheterization has advantages such as being easy, practical, and radiation-free, it cannot substitute chest X-ray. If the catheter tip is to be evaluated by ultrasonography, evaluations should be made from several different views. Microbubble testing was not considered significant for assessing central venous catheter orientation.
