Özet

Giriş: Tüm dünyayı etkisi altına alan COVID-19 multiorgan tutulumuyla seyreden yüksek mortalite ve morbidite oranları görülen viral bir enfeksiyondur. Hastalığın yayılımı ve hastalığa bağlı ölümlerin önlenmesi için bir çok aşı geliştirilmiş olup altta yatan kronik hastalığı olan bireylere öncelik verilerek dünya genelinde hızla bağışıklama kampanyaları başlatılmıştır. Ancak kullanımda olan aşılar ile ilgili güvenlik kaygıları oldukça yaygın olup bu durumun bağışıklama kampanyalarının önündeki en önemli engel olduğu vurgulanmaktadır. Çalışmamızda 12-18 yaş grubu kronik karaciğer hastalarının COVID-19 aşılanma oranları, aşılanma üzerine etkili faktörler ve aşı sonrası yan etkilerin belirlenmesi amaçladık. Materyal ve Metod: Gazi Üniversitesi Tıp Fakültesi Çocuk Gastroenteroloji Bilim Dalı tarafından kronik karaciğer hastalığı tanısı ile takip edilen 12-18 yaş arasında 50 çocuk ve bir velisi hasta grubuna dahil edildi. Genel Çocuk Polikliniği'ne başvuran hasta grubu ile benzer yaş grubunda 100 gönüllü çocuk ve bir velisi kontrol grubuna dahil edildi. Katılımcı olan çocuklara“COVID-19 Aşısı Hakkındaki Düşünceler”anketi ve yan etki değerlendirme formu yöneltildi, velilere ise“COVID-19 Aşı Okuryazarlığı”ölçeği uygulanarak veriler elde edildi. Bulgular: Çalışmamızda aşı hakkındaki olumlu düşünce oranları hasta grubunda %62 (n=31), kontrol grubunda %50 (n=50) olarak saptanırken iki grup arasında anlamlı bir fark saptanmamıştır (p>0,05). Hasta grubunda kadınlarda olumlu düşünce oranı %79,3 (n=23 olarak gözlenmiş olup; erkek bireylere göre anlamlı derecede daha yüksek bulunmuştur (p<0,05). Lisans düzeyinde eğitim seviyesindeki velilerin çocuklarının olumlu düşünce oranı hasta grubunda %90 (n=18), kontrol grubunda %93 (n=26) olarak saptanmış olup diğer gruba göre yüksekti (p<0,01). Kontrol grubunda COVID-19 nedeniyle yakınını kaybetme öyküsü olan bireylerin olumlu düşünce oranı (%81,8, n=9), olmayanlara (%46,1, n=41) göre daha yüksek bulunmuştur (p<0,05). Hasta ve kontrol grubu arasında olumlu düşünce nedenleri incelendiğinde“Doktor/sağlık personeli tarafından tavsiye edildiği için”seçeneğinin hasta grubunda %80,6 (n=25), kontrol grubuna %46 (n=23) göre anlamlı düzeyde daha fazla tercih edildiği görüldü (p<0,05). Aşı hakkında olumlu düşünceye sahip bireylerin aşılanma oranları hasta (%90,3, n=28) ve kontrol grubunda (%64, n=32) olumsuz yanıt veren bireylerin aşılanma oranlarına göre daha yüksek bulunmuştur (p<0,001). Çalışmamıza dahil edilen toplam 150 çocuk katılımcı içinde 64 olgu (%42,6) aşılanmıştı. Kontrol grubunda COVID-19 aşılanma oranı; %36 (n=36), hasta grubunda %56 (n=28) olarak saptanmış olup hasta grubunda aşılanma oranı anlamlı derecede yüksek bulundu (p<0,05). Hasta grubundaki kadınların aşılanma oranı (%69, n=20), erkeklere (%38,1, n=8) göre daha yüksek saptandı (p<0,05). Lisans düzeyinde eğitime sahip velilerin çocuklarında aşılanma oranı (hasta grubu; %90 , kontrol grubu; %78,6) diğer eğitim düzeyilerine göre daha yüksek saptanmıştır (p<0,001). Aşılar için sosyal medyayı bilgi kaynağı olarak kullananlarda, kullanmayanlara göre aşılanma oranları, hasta grubunda %72, kontrol grubunda da %31 oranında düşmekte idi (p<0,05). Sağlık çalışanlarından bilgi alan bireylerin aşılanma oranlarında hasta grubunda 20,4 kat, kontrol grubunda da 3,55 kat artış saptanmıştır (p<0,001). Hasta grubundaki velilerin COVID-19 aşı okuryaarlık düzeyi kontrol grubuna göre daha yüksek gözlendi (p<0,05). Evde aşılı birey bulunanların aşılanma oranları, hasta grubunda (%75,7, n=28) ve kontrol grubunda (%51,5, n=35) olup, bulunmayanlara göre yüksek bulunmuştur (p<0,001). Aşılanma öyküsü olan 64 katılımcıda toplam 127 doz COVID-19 aşı uygulaması sonrası görülen yan etkiler incelendi. Yan etki verileri hasta ve kontrol grubu arasında karşılaştırıldı hasta grubunda yan etki görülme oranı (%66,7, n=44) ile kontrol grubundaki yan etki görülme oranı (%70,5, n=43) arasında bir fark saptanmamıştır (p>0,05). Hasta grubunda mRNA bazlı aşı türleriyle yan etki görülme oranı (%76,9, n=40) , diğer aşı türlerine (%28,6, n=4) göre daha yüksek bulunmuştur (p<0,001). Çalışmamıza dahil edilen hasta grubundaki aşılanmış bireylerin %42,8'i (n=12) immünsupresan tedavi kullanmaktaydı; bu bireyler için sistemik yan etki görülme oranı (%19,4, n=6), kullanmayanlara göre daha düşük saptanmış olup aradaki fark istatistiksel olarak anlamlı bulunmuştur (p<0,05). Sonuç: Çalışmamızda kronik karaciğer hastası olan çocukların aşı hakkındaki düşünceleri ve aşılanma oranları için literatüre yeni bilgiler sağlanmıştır. Bireylerin ailelerinde aşılı bireylerin bulunması, velilerinin eleştirel/iletişimsel beceri düzeyleri ve aşılar hakkında bilgi kaynağı olarak doktor/sağlık çalışanı tercih etmelerinin aşılanma oranlarını olumlu etkilediği gözlenmiş olup bilgi kaynağı olarak sosyal medya tercihi ile aşılanma oranları arasında negatif bir ilişki saptanmıştır. Bu veriler ile bağışıklama kampanyaları için yeni görüşler sağlanmaktadır. Yan etki görülmesi üzerine kronik karaciğer hastası olma durumunun bir etkisi görülmemiş olup aşının hasta grubu için güvenli olduğuna yeni bir kanıt sunulmaktadır.

Introduction: COVID-19, which affects the whole world, is a viral infection with multiorgan involvement and high mortality and morbidity rates. Many vaccines have been developed to prevent the spread of the disease and death due to the disease, and immunization campaigns have been initiated around the world, giving priority to individuals with underlying chronic diseases. However, safety concerns about vaccines in use are quite common, and it is emphasized that this is the most important obstacle to immunization campaigns. In our study, we aimed to determine the COVID-19 vaccination rates, factors affecting vaccination and post-vaccine side effects of chronic liver patients between the ages of 12-18. Materials and Methods: 50 children between the ages of 12-18, followed by Gazi University Faculty of Medicine, Department of Pediatric Gastroenterology with the diagnosis of chronic liver disease, and one of their parents were included in the patient group. 100 volunteer children in the same age group as the patient group who applied to the General Pediatrics Polyclinic and one of their parents were included in the control group. The“Thoughts About the COVID-19 Vaccine”questionnaire and the side effect evaluation form were directed to the participating children, and the data were obtained by applying the“COVID-19 Vaccine Literacy”scale to the parents. Results: In our study, the rate of positive thoughts about the vaccine was found to be 62% (n=31) in the patient group and 50% (n=50) in the control group, while no significant difference was found between the two groups (p>0.05). In the patient group, the rate of positive thoughts in women was observed as 79.3% (n=23); it was found to be significantly higher than male individuals (p<0.05). Positive thinking rate of the children of parents with undergraduate education level was determined as 90% (n=18) in the patient group and 93% (n=26) in the control group, which was higher than the other group (p<0.01). In the control group, the rate of positive thoughts of individuals with a history of losing a relative due to COVID-19 (81.8%, n=9) was higher than those who did not (46.1%, n=41) (p<0.05). When the reasons for positive thinking between the patient and control group were examined, it was seen that the option“Because it was recommended by the doctor/health personnel”was significantly more preferred in the patient group than 80.6% (n=25) and 46% (n=23) in the control group ( p<0.05). Vaccination rates of individuals with positive thoughts about the vaccine were found to be higher in the patient (90.3%, n=28) and control groups (64%, n=32) compared to the vaccination rates of individuals who gave a negative response (p<0.001). Among the 150 children included in our study, 64 cases (42.6%) were vaccinated. COVID-19 vaccination rate in the control group; It was found to be 36% (n=36) and 56% (n=28) in the patient group, and the rate of vaccination was found to be significantly higher in the patient group (p<0.05). The vaccination rate of women in the patient group (69%, n=20) was found to be higher than that of men (38.1%, n=8) (p<0.05). The rate of vaccination in the children of parents with undergraduate education (patient group; 90%, control group; 78.6%) was found to be higher than those of other education levels (p<0.001). Vaccination rates were 72% lower in the patient group and 31% in the control group, compared to those who did not use social media as a source of information for vaccines (p<0.05). Vaccination rates of individuals who received information from healthcare professionals increased 20.4 times in the patient group and 3.55 times in the control group (p<0.001). The COVID-19 vaccine literacy level of the parents in the patient group was higher than the control group (p<0.05). Vaccination rates of those who had vaccinated individuals at home were higher in the patient group (75.7%, n=28) and control group (51.5%, n=35) compared to those who did not (p<0.001). Side effects observed after 127 doses of COVID-19 vaccine in 64 participants with a history of vaccination were examined. Side effect data were compared between the patient and control groups. There was no difference between the incidence of side effects in the patient group (66.7%, n=44) and the incidence of side effects in the control group (70.5%, n=43) (p>0 .05). In the patient group, the incidence of side effects with mRNA-based vaccine types (76.9%, n=40) was found to be higher than with other vaccine types (28.6%, n=4) (p<0.001). 42.8% (n=12) of the vaccinated individuals in the patient group included in our study were using immunosuppressant therapy; the rate of systemic side effects for these individuals (19.4%, n=6) was found to be lower than for non-users, and the difference was statistically significant (p<0.05). Conclusion: In our study, new information has been provided to the literature about the thoughts and vaccination rates of children with chronic liver disease. It has been observed that the presence of vaccinated individuals in the families of the individuals, the critical/communicative skill levels of their parents, and the preference of a doctor/health worker as a source of information about vaccines have a positive effect on vaccination rates, and a negative relationship has been found between social media preference as a source of information and vaccination rates. These data provide new insights for immunization campaigns. There was no effect of chronic liver disease on the occurrence of side effects, and new evidence is presented that the vaccine is safe for the patient group.
