Özet
AMAÇ: Beslenme durumu geriatrik hastalarda; mortalite ve deliryum gelişiminde önemli rol oynamaktadır. Deliryum, özellikle geriatrik hasta grubunun önemli bir sağlık problemi olup uzamış hastane yatışı, artmış mortalite ve artmış sağlık maliyetlerine sebep olmaktadır. Beslenme durumunun belirlenmesinde prognostik nutrisyonel indeks (PNI), nutrisyonel risk indeksi (NRI), nutrisyonel risk taraması (NRS 2002) ve geriatrik nutrisyonel risk indeksi (GNRI) gibi farklı indeksler kullanılmakta olup GNRI yaşlı hastalar için önerilen yeni bir indekstir. Çalışmamızda femur fraktürü nedeni ile hastanemize başvuran geriatrik hastalarda, birincil olarak prognostik nutrisyonel indeks (PNI) ve geriatrik nutrisyonel risk indeksi (GNRI) ile değerlendirilen beslenme durumunun, postoperatif deliryumu öngörmede ve mortaliteyi tahmin etmede etkilerini belirleyerek bu iki indeksi duyarlılık ve özgüllük açısından karşılaştırmayı amaçladık. Ayrıca preoperatif bakılan PNI ve GNRI skorlarının hastane yatış süresi, yoğun bakım yatış süresi ve intraoperatif hipotansiyon gibi peroperatif komplikasyonlar ile ilişkili olup olmadığını da saptamayı planladık. İkincil olarak deliryumu etkileyebilecek diğer faktörlerin (demografik veriler, laboratuvar verileri, preoperatif ve postoperatif yatış süresi, cerrahi süresi, kanama ve intraoperatif hipotansiyon, postoperatif yapılan kan transfüzyonu) postoperatif deliryum üzerine etkilerini saptamayı amaçladık. GEREÇ VE YÖNTEM: Çalışmamız, T.C. Sağlık Bakanlığı Sağlık Bilimleri Üniversitesi Sancaktepe Şehit Prof. Dr İlhan Varank Sağlık Uygulama ve Araştırma Hastanesi Anesteziyoloji ve Reanimasyon Kliniği'nde, hastanemiz Etik Kurulundan (21.04.2021 tarihli 2021/142 karar numarası) onay alındıktan sonra, ''Prospektif Gözlemsel Klinik Çalışma'' olarak başlatıldı. Mayıs 2021 ile Aralık 2021 tarihleri arasında elektif şartlarda femur kırığı nedeniyle spinal anestezi altında opere olacak, postoperatif yoğun bakım takip endikasyonu bulunan, 65 yaş üzeri ASA III-IV risk grubu ve spinal anestezi uygulanan 77 hasta çalışmaya dahil edildi. Hastaların preoperatif değerlendirilmesinde; demografik verileri, Charlson komorbidite indeks (CCI), prognostik nutrisyonel indeks (PNI), geriatrik nutrisyonel risk indeksi (GNRI), mini mental test skoru (MMT) sonuçları kaydedildi. xi Tüm hastalara spinal anestezi uygulandı ve intraoperatif kanama miktarı, cerrahi süre (hastalar operasyon odasına alındıktan sonra operasyon bitimine kadar geçen süre), intraoperatif hipotansiyon gelişip gelişmediği kaydedildi. Hastalar postoperatif yoğun bakım ünitesine transfer edildi. Hastaların ilk 24 saatte VAS skorları (4.,12. ve 24. saatte) kaydedildi. Deliryum takibinde; preoperatif, postoperatif 1.günde ve taburculuk öncesi Konfüzyon Değerlendirme Metodu (CAM/CAM-ICU) testi bakılıp kaydedildi. Olgular deliryum görülme durumuna göre iki gruba (VAR/YOK) ayrıldı. Hastaların preoperatif, postoperatif 1.günde ve taburculukta; rutinde yapılan tam kan, koagülasyon, kan şekeri, serum elektrolitleri, böbrek fonksiyon testleri ve karaciğer fonksiyon testlerinin sonuçları kaydedildi. Tüm laboratuvar verileri ve kan transfüzyonu yapılıp yapılmadığı mevcut hasta takip formları ve hastane işletim sisteminden (HIS) bakılarak kaydedildi. Ayrıca hastalarda femur kırığı olduktan sonra cerrahiye kadar geçen süre, postoperatif yoğun bakım yatış süresi, postoperatif hastane yatış süresi ve toplam hastane yatış süresi kaydedildi. Taburcu olan hastalarda 3 aylık mortalite hasta ve hasta yakınlarına telefonla ulaşılarak kaydedildi. Preoperatif takiplerinde; albümin infüzyonu alan, mini mental skoru 21 altında olan, deliryum tespit edilen hastalar çalışmaya alınmadı. Preoperatif ve intraoperatif kan transfüzyonu yapılan hastalar çalışmadan çıkarıldı. BULGULAR: Çalışma yaşları 65 ile 93 arasında olan, 28'i (%39.4) erkek ve 43'ü (%60.6) kadın olmak üzere toplam 71 olgu ile yapıldı. Yaş ortalaması 80.20±6.39 yıldı. Vücut kitle indeksi (VKI) 17.6 ile 44.9 kg/m2 arasında değişmekte olup, ortalaması 27.44±5.24 kg/m2 'di. %91.5'inin ASA skoru 3'tü. Olguların 35'in de(%49.3) postoperatif 1.günde, 14'ün de (%21.2) ise taburculukta deliryum görüldü. Postoperatif 1.günde ve taburcu olurken deliryum görülen olgularda preoperatif PNI ve GNRI değerleri düşük saptandı. Postoperatif 1.gün(p:0.016; p<0.05) ve taburculuk(p:0.0076; p<0.05) deliryumunu öngörmede; PNI'ın GNRI'dan daha etkin olduğu bulundu. 3 aylık mortalite oranı ise %15.5'ti. PNI skorunun 3 aylık mortaliteyi öngörmede de GNRI skoruna göre daha anlamlı olduğu saptandı (p:0.032; p<0.05). xii Preoperatif hemoglobin ve albümin düşüklüğü ile kan transfüzyonu yapılmış olmasının da deliryumu arttırıcı etkisi olduğu saptandı. SONUÇ: Çalışmamızda femur fraktürü nedeniyle spinal anestezi altında opere olmuş ve postoperatif yoğun bakımda takip edilen geriatrik hastalarda, postoperatif 1.günde ve taburculukta deliryum görülen olguların preoperatif PNI ve GNRI değerlerinin anlamlı düşük, postoperatif yoğun bakım yatış süresi, postoperatif hastane yatış süresi ve toplam hastane yatış gün sürelerinin daha uzun olduğu saptandı. Taburculuğunda deliryum görülen olgularda 3 aylık mortalite oranı anlamlı daha yüksek saptandı. Preoperatif PNI ve GNRI skorları karşılaştırıldığında, hem postoperatif 1.gün ve taburculuk deliryumunu hem de 3 aylık mortaliteyi öngörmede preoperatif PNI değerinin GNRI dan daha etkin olduğu saptandı. Beslenme durumunun yanında, preoperatif hemoglobin ve albümin düşüklüğü olan ya da kan kan transfüzyonu yapılan hastalarda da deliryum riskinin arttığını gözlemledik

PURPOSE: The nutritional status of geriatric patients plays an important role in delirium and mortality. Delirium is a significant health problem, especially in the geriatric patient group, causing prolonged hospitalization, increased mortality, and increased healthcare costs. Different indices such as the prognostic nutritional index (PNI), nutritional risk index (NRI), nutritional risk screening (NRS 2002), and geriatric nutritional risk index (GNRI) are used to determine nutritional status, and GNRI is a new index recommended for elderly patients. In our study, we aimed to determine the effects of nutritional status evaluated primarily by the prognostic nutritional index (PNI) and geriatric nutritional risk index (GNRI) on predicting postoperative delirium and mortality in geriatric patients admitted to our hospital due to femur fracture. We also planned to determine whether preoperative PNI and GNRI scores are associated with perioperative complications such as hospitalization duration, ICU stay, and intraoperative hypotension. Secondly, we aimed to determine the effects of other factors that may affect delirium (demographic data, laboratory data, preoperative and postoperative hospitalization duration, surgical duration, bleeding and intraoperative hypotension, postoperative blood transfusion) on postoperative delirium. METHODS: Our study was initiated as a“Prospective Observational Clinical Study”after obtaining approval from our hospital's Ethics Committee (decision number 2021/142, dated 21.04.2021) at the T.C. Sağlık Bakanlığı Sağlık Bilimleri Üniversitesi Sancaktepe Şehit Prof. Dr İlhan Varank Health Application and Research Hospital Anesthesiology and Reanimation Clinic. From May 2021 to December 2021, 77 patients who were 65 years of age and older, had an ASA III-IV risk group, and underwent spinal anesthesia due to femur fracture under elective conditions, and had an indication for postoperative intensive care follow-up were included in the study. During the preoperative evaluation of the patients, demographic data, Charlson comorbidity index (CCI), prognostic nutritional index (PNI), geriatric nutritional risk index (GNRI), and mini-mental test score (MMT) results were recorded. xiv All patients underwent spinal anesthesia, and the amount of intraoperative bleeding, surgical duration (the time elapsed from when the patients were taken to the operating room until the end of the operation), and whether intraoperative hypotension occurred were recorded. Patients were transferred to the postoperative intensive care unit. VAS scores (at 4th, 12th, and 24th hour) were recorded for all patients in the first 24 hours. In delirium monitoring, the confusion assessment method (CAM/CAM-ICU) test was performed and recorded preoperatively, on postoperative day 1, and prior to discharge. The results of routine complete blood count, coagulation, blood glucose, serum electrolytes, renal function tests, and liver function tests were recorded for all patients preoperatively, on postoperative day 1, and at discharge. All laboratory data and whether blood transfusion was performed were recorded by reviewing the patient follow-up forms and the hospital operating system (HIS). Additionally, the time elapsed between femur fracture and surgery, postoperative intensive care unit (ICU) stay, postoperative hospital stay, and total hospital stay were recorded in patients. Three-month mortality in discharged patients was recorded by telephone contact with patients and their families. Patients who received albumin infusion, had a mini-mental score below 21, or were identified with delirium during preoperative follow-up were not included in the study. Patients who received preoperative and intraoperative blood transfusions were also excluded from the study. RESULTS: The study was conducted with a total of 71 cases, 28 (39.4%) of whom were male and 43 (60.6%) were female, with ages ranging from 65 to 93. The mean age was 80.20±6.39 years. Body mass index (BMI) ranged from 17.6 to 44.9 kg/m2 with a mean of 27.44±5.24 kg/m2. 91.5% of the cases had ASA score of 3. Delirium was observed in 35 (49.3%) cases on the first postoperative day and in 14 (21.2%) cases at discharge. Preoperative PNI and GNRI values were found to be low in cases where delirium was observed on the first postoperative day and at discharge. PNI was found to be more effective than GNRI in predicting delirium on the first postoperative day (p:0.016; p<0.05) and at discharge (p:0.0076; p<0.05). The three-month mortality rate was 15.5%. PNI score was found to be more significant than GNRI score in xv predicting three-month mortality (p:0.032; p<0.05). It was also observed that preoperative low hemoglobin and albumin levels and blood transfusions increased the risk of delirium. CONCLUSION: In geriatric patients who underwent surgery under spinal anesthesia due to femur fracture and were followed up in the postoperative ICU, it was found that the preoperative PNI and GNRI values of cases where delirium was observed on the first postoperative day and at discharge were significantly low, and the postoperative ICU stay, postoperative hospital stay, and total hospital stay durations were higher. The three-month mortality rate was found to be significantly higher in cases where delirium was observed at discharge. When comparing preoperative PNI and GNRI scores, it was found that preoperative PNI value was more effective than GNRI in predicting both postoperative 1-day and discharge delirium and three-month mortality. In addition to nutritional status, it was observed that delirium risk increased in patients with preoperative low hemoglobin and albumin levels or those who received blood transfusions.
