Özet

Amaç: Bu çalışmada amacımız yoğun bakım ünitesinde yatışı yapılmış COVID-19 ve Non COVID-19 hastaların yoğun bakıma kabul anında veya yoğun bakım takibindeki ilk 72 saat içinde çekilmiş olan beyin tomografilerinin incelenerek COVID-19'un neden olduğu beyin hasarlarını tespit etmektir. Bununla birlikte yatış anındaki ve yatışının yedinci günündeki laboratuvar verilerinin incelenerek prediktif bir özellik olup olmadığını saptamaktır. Materyal ve Metot: 01.10.2020-01.03.2021 tarihlerinde KSBÜ Tıp Fakültesi YBÜ'lerine COVID-19 pnömonisi ve akut stroke tanısı almış olan 30 hasta Grup-1 olarak kaydedildi. COVID-19 ekarte edilmiş ve akut stroke nedeniyle yatırılan 30 hasta Grup-2 olarak kaydedildi. Hastaların ilk 72 saat içinde çekilmiş olan beyin tomografi raporları incelendi. Hastaların yaş, cinsiyet, ek hastalık varlığı, yoğun bakım ünitesi (YBÜ) kalış süresi, mortalite durumu, hemogram parametreleri, total biyokimya parametreleri ve koagulasyon parametreleri incelendi. İki grup arasındaki verileri karşılaştırıldı. İstatistiksel anlamlılık seviyesi p<0,05 olarak kabul edilmiştir. Bulgular: Gruplar arasında yapılan karşılaştırmada hastaların demografik verilerinin, mortalite oranlarının, toplam yatış gün sayısının istatistiksel açıdan anlamlı farklı olmadığı sonuçlandı. İncelenen beyin tomografilerinde hem COVID-19 hasta grubunda hem de Non-COVID-19 hasta grubunda hemoraji görülme oranları daha sık sonuçlandı. COVİD 19 hastalarının 21 (%70) hasta da intrakranial hemoraji, 9 (%30) hastada serebral infarkt saptandı. İntrakranial hemoraji geçirmiş olan hasta grubunda en sık sağ hemisferde ve en sık frontal lob tutulumu olduğunu bulduk. Kanama tipi ise %90 oranla en sık intraparankimal kanama bulduk. Hastaların %42'sinde şift olduğu saptandı. Non-COVİD hasta grubunda 22 (%73,3) hastada serebral hemoraji, 8 (%26,7) hastada ise serebral infarkt saptadık. İntrakranial hemoraji geçirmiş olan hasta grubunda en sık sol hemisferde ve en sık frontal lob tutulumu olduğunu bulduk. Kanama tipi ise %90 oranda intraparankimal kanama bulduk. Hastaların %31,8'sinde şift olduğunu bulduk. Akut infarkt saptanan COVID-19 hastalarının %77'sinde sağ hemisferde ve %33'ünde sol hemisferde infarkt gözlendi. En sık tutulum yeri %55 ix oranda paryetal bölge oldu. Serebellar infarkt ise hastaların %22'sinde gözlendi. NON COVID-19 hastalarının %75'inde sağ hemisferde %25'inde sol hemisferde infarkt gözlendi. Hastaların %50'sinde paryetal lob tutulum olduğu gözlendi. Serebellar infarkt ise %12,5 oranda gözlendi. Hastaların laboratuvar verileri arasında yatış anında COVID-19 hasta grubunda D-DİMER, PLT, nötrofil, AST, ALT, BUN, ÜRE istatistiksel olarak anlamlı yüksek bulundu. PaO2/FiO2 oranları COVID-19 hasta grubunda istatistiksel olarak anlamlı düşük bulundu. Tartışma ve Sonuç: COVID-19'un viral patojenitesi ile birlikte koruyucu tedavi yaklaşımlarının nörolojik komplikasyonları etkilemekle birlikte iskemik veya hemorajik tipinin oransal olarak değiştirdiğinin ve prognoza kötü etkisinin olduğu sonucuna vardık.

Aim: In this study, our aim is to determine the brain damage caused by COVID-19 by examining the brain tomographies taken at the time of admission to the intensive care unit or within the first 72 hours of the intensive care follow-up of COVID-19 and NonCOVID-19 patients hospitalized in the intensive care unit. In addition, it is to determine whether there is a predictive feature by examining the laboratory data at the time of hospitalization and on the seventh day of hospitalization. Materıals and Method: Between 01.10.2020 and 01.03.2021, 30 patients diagnosed with COVID-19 pneumonia and acute stroke in KSBU Faculty of Medicine ICUs were registered as Group-1. COVID-19 was excluded and 30 patients hospitalized for acute stroke were recorded as Group-2. Brain tomography reports of the patients taken within the first 72 hours were analyzed. Age, gender, presence of additional disease, intensive care unit (ICU) stay, mortality status, hemogram parameters, total biochemistry parameters and coagulation parameters of the patients were analyzed. The data between the two groups were compared. Statistical significance level was accepted as p<0.05. Results: In the comparison between the groups, it was concluded that the demographic data of the patients, mortality rates, and the total number of hospitalization days were not statistically different. The rates of hemorrhage in both the COVID-19 patient group and the non COVID-19 patient group resulted more frequently in the brain tomographies examined. Intracranial hemorrhage was detected in 21 (70%) patients and cerebral infarction in 9 (30%) patients of COVID-19 patients. We found that the most common right hemisphere and frontal lobe involvement was the most common in the group of patients who had had intracranial hemorrhage. We found the most common intraparenchymal hemorrhage with a rate of 90% in the type of hemorrhage. Shifts were found in 42% of the patients. In the non COVID-19 patient group, we found cerebral hemorrhage in 22 (73.3%) patients and cerebral infarction in 8 (26.7%) patients. We found that the most common left hemisphere and frontal lobe involvement was the most common in the patient group who had intracranial xi hemorrhage. We found the most common intraparenchymal hemorrhage with a rate of 90% in the type of hemorrhage. We found that 31.8% of the patients had shifts. In COVID-19 patients with acute infarction, 77% had right hemisphere and 33% left hemisphere infarcts. The most common site of involvement was the parietal region with a rate of 55%. Cerebellar infarction was observed in 22% of the patients. Infarcts were observed in the right hemisphere in 75% of NON COVID-19 patients and in the left hemisphere in 25%. Parietal lobe involvement was observed in 50% of the patients. Cerebellar infarction was observed with a rate of 12.5%. Among the laboratory data of the patients, D-DIMER, PLT, neutrophils, AST, ALT, BUN, and UREA were found to be statistically significantly higher in the COVID-19 patient group at the time of hospitalization. PaO2/FiO2 ratios were found to be statistically significantly lower in the COVID-19 patient group. Conclusion: We concluded that while the viral pathogenicity of COVID-19 and preventive treatment approaches affect neurological complications, ischemic or hemorrhagic type proportionally changes and has a bad effect on prognosis.
