Özet

Amaç: Coats hastalarında subfoveal nodüllerin gelişim basamaklarını geriye dönük olarak incelemek, subfoveal nodül gelişimine ve prognozuna etki eden faktörleri araştırmak. Gereç Yöntem: Başlangıçta ya da takipleri sırasında subfoveal nodül varlığı ve gelişim oranları incelenerek Coats hastalığında subfoveal nodül prevalansı belirlendi. Subfoveal nodül tespit edilen hastaların tüm kayıtları ve multimodal görüntülemeleri retrospektif olarak incelendi. Subfoveal nodül gelişimi farklı klinik ve görüntüleme özelliklerine sahip 5 evreye ayrıldı (Evre 0-Maküler Eksudasyon, Evre 1-Subfoveal Eksudasyon, Evre 2-Paketlenme, Evre 3-Subfoveal Nodül, Evre 4-Subfoveal Fibrozis/Fibrotik Skar). Evrelerin gelişimine ve prognozuna etki eden faktörler kaydedildi ve analizleri yapıldı. Bulgular: Ortalama başvuru yaşı 6.88±3.9, ortalama takip süresi 31.14±29.5 ay olan 43 Coats hastası (Evre 2A-3A) çalışmaya dahil edildi. Son kontrolde subfoveal nodül prevalansı %97 (42/43) idi. Farklı subfoveal nodül evrelerinde başvuran hastalarda; başvuru yaşı, cinsiyet, takip süresi ve periferik bulguların (telenjiektazi, eksudasyon, iskemi) yayılımı arasında fark izlenmedi. Erken subfoveal nodül gelişen (<9ay) hastaların başvuru yaşı geç subfoveal nodül gelişen hastalara göre daha küçüktü (5.6±3.2 vs 8.9±4.5, p=0.010) ve bu hastalara yapılan yıllık ortalama IV AntiVEGF/Steroid enjeksiyonu sayısı daha azdı (1.4±1.21 vs 3.69±1.29, p=0.001). Erken subfoveal nodül gelişme riskinin 7 yaş altı hastalarda 2.4 kat, yılda 3 ve daha az İV AntiVEGF/Steroid enjeksiyonu yapılan hastalarda ise 4 kat arttığı bulundu. Final EİDGK yüksek (<1.3 logMAR) olan hastalara yapılan yıllık ortalama İV AntiVEGF/Steroid enjeksiyon sayısı, düşük olan hastalardan fazlaydı (p=0,025). Sonuç: Subfoveal nodül Coats hastalarında çok sık görülmektedir ve çalışmamızda prevalansı %97 bulunmuştur. Yoğunlaştırılmış İV AntiVEGF/Steroid tedaviler subfoveal nodül gelişimini engelleyebilir, prognozunu yavaşlatabilir ve daha iyi fonksiyonel sonuçlar sağlayabilir.

Purpose: To review the developmental stages of subfoveal nodules in patients with Coats disease retrospectively and to investigate the factors affecting the development and prognosis of subfoveal nodules. Material and Method: The prevalence of subfoveal nodules in Coats' disease was determined by examining the presence and development rates of subfoveal nodules at baseline or during follow-up. All records and multimodal imaging of patients with subfoveal nodules were reviewed retrospectively. Subfoveal nodule development was divided into 5 stages according to different clinical and imaging features (Stage 0-Macular Exudation, Stage 1-Subfoveal Exudation, Stage 2-Packaging, Stage 3-Subfoveal Nodule, Stage 4-Subfoveal Fibrosis/Fibrotic Scar). The factors affecting the development and prognosis of the stages were recorded and analyzed. Results: The mean age at presentation was 6.88±3.9 and the mean follow-up time was 31.14±29.5 months in 43 patients with Coats' disease (Stage 2A-3A) who included in the study. The prevalence of subfoveal nodules at the final follow-up was 97% (42/43). There was no difference in age, gender, follow-up time and extent of peripheral findings (telangiectasia, exudation, ischemia) in patients with different subfoveal nodule stages at presentation. Patients with early-developing subfoveal nodule (<9 months) were younger than patients with late-developing subfoveal nodule (5.6±3.2 vs 8.9±4.5, p=0.010) and the mean number of IV AntiVEGF/Steroid received per a year was lower in these patients (1.4±1.21 vs 3.69±1.29, p=0.001). It was found that the ris okf early subfoveal nodule development increased 2.4 times in patients younger than 7 years of age and increased 4 times in patients who received 3 or less IV AntiVEGF/Steroid injections per year. Patients with high final BCVA (<1.3 logMAR) had a higher mean number of IV AntiVEGF/Steroid received per year than patients with low BCVA (p=0.025). Conclusion: Subfoveal nodule is very common in Coats' disease and its prevalence was found 97% in our study. Intensive IV AntiVEGF/Steroid treatments can prevent subfoveal nodule development, slow its prognosis and provide better functional outcomes.
