Özet

Giriş: COVID-19 pandemisinin çocukları ve ebeveynleri ruhsal açıdan önemli derecede etkilediği tahmin edilmektedir. Bu çalışmanın amacı COVID-19 pandemisinin ilk dönemlerinde hastanede yatırılarak takip edilen çocuk hastaların ve ebeveynlerinin yatış nedeniyle yaşadıkları kaygı düzeylerini, depresyon ve travma belirtilerini değerlendirmektir. Hastalar ve Yöntem: Mayıs 2020 – Aralık 2020 tarihleri arasında gerçekleştirilen bu çalışmaya üç grupta yaşları 7 – 18 yaş arasında değişen toplam 132 çocuk ve anne dahil edildi. Erciyes Üniversitesi Tıp Fakültesi Çocuk Hastanesine yatan çocuklardan COVID-19 şüphesi/tanısı ile yatırılan 50 hasta çocuk ve annesi birinci (Grup-1), COVID-19 dışı tanılarla yatırılan 51 hasta çocuk ve annesi ikinci (Grup-2) ve evde olan 31 sağlıklı çocuk ve annesi üçüncü gruba (Grup-3) dahil edildi. Kesitsel olarak planlanan bu çalışmada çocuklar için DSKE, ÇBDÖ, ÇTSSTÖ ölçekleri ve ebeveynler için PCL–C, BAÖ, BDE, AHÇYTÖ, BÇSÖ ölçekleri kullanıldı. Bulgular: Gruplar arasında çocukların cinsiyet ve yaşı açısından fark yoktu (sırasıyla; p=0.210, p=0.052). Her üç grupta çocukların DKEP SKEP ve ÇTSSTÖ puanlarının yüksek olduğu ancak gruplar arasında fark olmadığı tespit edildi (sırasıyla; p=0.062, p=0.352, p=0.533). ÇBDÖ puanları açısından gruplar arasında fark yoktu (p=0.703). Annelerin PCL-C puanları her üç grupta yüksekti ancak gruplar arasında fark yoktu (p=0.464). Çocukları hastanede yatan annelerin (grup-1 ve 2) BAÖ puanları çocukları evde olan annelerden yüksek bulundu (sırasıyla; p<0.001, p=0.003). Annelerin BDE puanları her üç grupta benzerdi (p=0.260). Çocukları COVID-19 şüphesi ile yatan annelerin BÇSÖ-Problem çözme puanları grup-2 ve 3'den düşüktü (her ikisi için p<0.001). Her üç grupta çocukların DKEP ve ÇTSSBÖ puanları ile annelerin PCL-C, BAÖ ve BDE puanları arasında pozitif, grup-1 ve 2'de annelerin problem çözme puanları arasında negatif korelasyon vardı. Sonuç: Pandemiyi yaşayan çocuklar ve anneleri TSSB riski taşımaktadır. Evde olan çocukların hastanede yatan çocuklar kadar kaygılı olduğu, çocukları hastanede yatan annelerin ise çocukları evde olanlardan daha fazla anksiyete yaşadığı anlaşılmaktadır. Çocukları hastanede yatan ve özellikle COVID-19 şüphesi/tanısı ile yatan annelerin problem çözme becerileri olumsuz etkilenmektedir. Pandemiyi yaşayan çocukların ve ebeveynlerinin bu bulgular açısından desteklenmesi ve klinik takiplerinin sağlanması gerekmektedir. Bu konuda daha fazla çocuğun dahil edildiği, uzun dönem etkilerin ortaya konabileceği kapsamlı çalışmalara ihtiyaç vardır.

Aim: It is estimated that the COVID-19 pandemic significantly affects children and parents psychologically. The aim of this study is to evaluate the anxiety levels, depression, and trauma symptoms experienced by pediatric patients and their families who were hospitalized in the early stages of the COVID-19 pandemic. Patients and Methods: A total of 132 children aged between 7 and 18 years and mothers in three groups were included in this study, which was carried out between May 2020 and December 2020. Among the children hospitalized in Erciyes University Children's Hospital, 50 children hospitalized with the suspicion/diagnosis of COVID-19 and their mothers were the first (Group-1), 51 children hospitalized with non-COVID-19 diagnoses, and their mothers were the second (Group-2), and 31 healthy children were at home and their mothers were included in the third group (Group-3). In this cross-sectional study, STAIC, CBAI, CPTSDI scales for children and PCL – C, BAI, BDI, PARI, CSS scales for parents were used. Results: There was no difference between the groups in terms of gender and age of the children (p=0.210, p=0.052, respectively). It was determined that the SAIC TAIC and CPTSDI scores of the children were high in all three groups, but there was no difference between the groups (p=0.062, p=0.352, p=0.533 respectively). There was no difference between the groups in terms of CBAI scores (p=0.703). PCL-C scores of the mothers were high in all three groups, but there was no difference between the groups (p=0.464). BAI scores of mothers whose children were hospitalized (groups-1 and 2) were higher than mothers whose children were at home (p<0.001, p=0.003, respectively). The mothers' BDI scores were similar in all three groups (p=0.260). CSS-Problem solving scores of mothers whose children were hospitalized with suspected COVID-19 were lower than group-2 and 3 (p<0.001 for both). There was a positive correlation between the SAIC and CPTSDI scores of the children in all three groups and the PCL-C, BAI, and BDI scores of the mothers, and a negative correlation between the mothers' problem-solving scores in groups-1 and 2. Conclusion: Children and their mothers experiencing the pandemic are at risk of PTSD. It is understood that children at home are as anxious as children in hospital, and mothers whose children are in hospital experience more anxiety than those whose children are at home. Problem-solving skills of mothers whose children are hospitalized and who are hospitalized with the suspicion/diagnosis of COVID-19 are adversely affected. Children experiencing the pandemic and their parents should be supported in terms of these findings, and clinical follow-up should be provided. There is a need for comprehensive studies in which more children are included, and long-term effects can be revealed.
