Özet

Amaç: Biz bu çalışmada torakotomi cerrahisi uygulanan hastalarda torakal erektör spina plan bloğu uygulaması ile perioperatif intravasküler lidokain infüzyonunun postoperatif dönemde ağrı skorlarına olan etkisini ve postoperatif opioid tüketimine olan etkisini karşılaştırmayı amaçladık. Materyal ve Metod: Yerel etik kurulu onayı alındıktan sonra Erzurum Atatürk Üniversitesi Tıp Fakültesi Araştırma Hastanesinde torokotomi cerrahisi uygulancak ASA (American Society of Anesthesiologist) sınıflaması I-III olan, yaşları 18-65 arasında değişen ve çalışmayı kabul eden gönüllü 60 hasta çalışmaya dahil edildi. Hastalar randomize olarak üç farklı gruba ayrıldı. Birinci grup sadece Erektör spina plan bloğu uygulanan ES grup (n=20), ikinci grup sadece intravenöz lidokain infüzyonu uygulanan L grubu (n=20) ve üçüncü grup ise kontrol P grubu (n=20) olacak şekilde oluşturuldu. Anestezi indüksiyonu üç grupta da aynı olacak şekilde verildi. Postoperatif dönemde operasyon bitiminden 30 dakika önce 0.1 mg/kg morfin, 1000 mg paresetamol ve deksketoprofen uygulandı. Postoperatif dönemde parasetamol eşit aralıklarla dört kez tekrarlandı. VAS (vizüel analog skalası) skoru 4 ve üzeri olan hastalara 100 mg tramadol yapıldı. Her grupta da hastalara PCA (Hasta kontröllü analjezi) cihazı yükleme dozu 50 mcg, boluz dozu 20 mcg, bazal infüzyon dozu 20 mcg ve kilitli kalma süresi 20 dakika olacak şekilde ayarlandı ve takıldı. Post-anestezik uyanma ünitesinde (PACU) postoperatif dönemde 1-2-4-8-12-24 ve saatlerde istirahatte ve öksürüklü VAS skorları not edildi. Postoperatif dönemde 0-4, 4-8, 8-12, 12-24 saat aralıklarında tüketilen toplam opioid miktarı not edildi. 24. saat sonunda tüketilen toplam opioid miktarı hesaplandı. Peroperatif dönemde remifentanil tüketim miktarları not edildi. Bulgular: Gruplar arasında yaş, kilo, boy ve BMI, cinsiyet, operasyon tipi ve süresi, anestezi süresi, ASA sınıflaması, torakotomi prosedürü, cilt insizyon uzunluğu, kot retraksiyon süresi ve uzunluğu açısından gruplar arasında anlamlı bir fark bulunmadı (p>0,05). Gruplar arasında postoperatif opioid tüketimi açısından istatistiksel olarak 4-8 saat ve 8-12 saat dilimleri arasında anlamlı fark gözlenmezken; 0-4 saat diliminde Grup ES ve Grup L gruplarının Grup P' ye üstün olduğu görülmüştür; 12-24 saat diliminde Grup ES ve Grup L, Grup P ile karşılaştırıldığında opioid tüketimi istatistiksel olarak anlamlı düşük bulundu (p<0.05). 24 saatlik opioid tüketimlerine bakıldığında Kontrol gurubuna göre hem ES hem de lidokain grubunda istatistiksel olarak daha az tüketim olmuştur (p=0.024). (ES vs. Kontrol p=0.043, Lidokain vs. Kontrol p=0.009). İntraoperatif dönemde her üç gruptada remifentanil tükemtimleri bakımından anlamlı fark bulunmamasına rağmen lidokain grubunda bu tüketimin daha az, ESP blok uygulanan grupta ise daha fazla olduğu görüldü (p>0,05). Ek analjezi gereksinimi açısından gruplar arasında yapılan karşılaştırmada gruplar arasında istatistiksel olarak anlamlı fark ortaya çıktı (p<0,05). Ek analjezi gereksinimi Grup ES ve Grup L' de kontrol grubuna kıyasla daha az olduğu görüldü. İstirahat VAS skorları kıyaslandığında; PACU, 1.saat, 2.saat ve 4. Saat VAS skorları arasında istatiksel olarak anlamlı bir fark görülmüştür (p<0.05). 8.saat, 12.saat ve 24.saat VAS istirahat değerlendirmeleri açısından ise her üç grup arasında istatistiksel olarak anlamlı bir fark bulunmamıştır (p<0.05). Ancak tüm zaman dilimlerindeki istirahat VAS skorları kıyaslandığında hem Grup ES hem de Grup L' deki skorlar Grup P'ye kıyasla daha düşük bulunmuştur. Dinamik VAS verileri karşılaştırıldığında; PACU'da, 1.saat ve 2.saat değerlendirmelerinde istatiksel olarak anlamlı bir fark bulunmuştur (p<0.05). Hem Grup ES, hem Grup L; Grup P ile kıyaslnadığından üstün bulunmuştur. Sonuç: Sonuç olarak hem torasik ESP bloğu yöntemi hem de i.v. lidokain infüzyon yöntemi torakotomi uygulanacak hastalarda etkili analjezik yöntem olabileceğini, postoperative dönemde ek analjezik ihtiyacını ve opioid tüketim miktarını azalttığını, bunlarla birlikte erken postoperatif dönemde istirahatte ve aktivitede VAS skorlarını tolere edilebilir düzeylere getirebildiğini gördük. Bu veriler ışığında i.v. lidokain infüzyonu ve ESP bloğunun torakotomi cerrahilerinde intraoperatif ve postoperatif analjezi amacıyla kullanılabileceğini düşünmekteyiz ancak bu iki tekniğin analjezik açıdan birbirine herhangi bir üstünlüğü olduğunu gösteremedik.

Objective: In this study, we aimed to compare the effect of thoracic erector spina plan block and perioperative intravascular lidocaine infusion on postoperative pain scores and postoperative opioid consumption in patients undergoing thoracotomy surgery. Materials and Methods: After the approval of the local ethics committee, 60 patients with ASA (American Society of Anesthesiologist) classification I-III, aged between 18-65, who were going to undergo thoracotomy surgery at Erzurum Atatürk University Medical Faculty Research Hospital, and accepted the study, were included in the study. The patients were randomly divided into three different groups. The first group was formed as the ES group (n=20) in which only Erector spina plane block was applied, the second group was the L group (n=20) in which only intravenous lidocaine infusion was applied, and the third group was the control group P group (n=20). Anesthesia induction was given in the same way in all three groups. In the postoperative period, 0.1 mg/kg morphine, 1000 mg parcetamol and dexketoprofen were administered 30 minutes before the end of the operation. Paracetamol was repeated four times at equal intervals in the postoperative period. 100 mg tramadol was administered to patients with a VAS (visual analog scale) score of 4 and above. In each group, the PCA device was adjusted and inserted as 50 mcg loading dose, 20 mcg bolus dose, 20 mcg basal infusion dose, and 20 minutes lock-in time. Resting and coughing VAS scores were noted at 1-2-4-8-12 and 24 hours postoperatively in the post-anesthesia care unit (PACU). The total amount of opioid consumed at intervals of 0-4, 4-8, 8-12, and 12-24 hours in the postoperative period was noted. The total amount of opioid consumed at 24 hours was calculated. Remifentanil consumption amount were noted in the peroperative period. Results: According to the data we obtained, there was no significant difference between the groups in terms of age, weight, height and BMI, gender, type and duration of operation, duration of anesthesia, ASA classification, thoracotomy procedure, skin incision length, rib retraction time and length (p>0,05). While there was no statistically significant difference between the groups in terms of postoperative opioid consumption between 4-8 hours and 8-12 hours; Group ES and Group L groups were found to be superior to Group P in the 0-4 time zone; opioid consumption was found to be statistically significantly lower in Group ES and Group L compared to Group P in the 12-24 hour period (p<0.05). Considering the 24 hour opioid consumption, there was statistically less consumption in both ES and lidocaine groups compared to the control group (p=0.024). (ES vs. Control p=0.043, Lidocaine vs. Control p=0.009). Although there was no significant difference in remifentanil consumption in all three groups in the intraoperative period, it was observed that this consumption was lower in the lidocaine group and higher in the ESP block group (p>0,05). There was a statistically significant difference between the groups in the comparison made between the groups in terms of rescue analgesia requirement (p<0.05). Rescue analgesia requirement was found to be less in Group ES and Group L compared to the control group. When resting VAS data are compared; there was a statistically significant difference between PACU, 1st hour, 2nd hour and 4th hour VAS data (p<0.05). There was no statistically significant difference between the three groups in terms of 8th hour, 12th hour and 24th hour VAS resting evaluations (p<0.05). However, when the resting VAS values in all time periods were compared, the scores in both Group ES and Group L were found to be lower compared to Group P. When dynamic VAS data is compared; a istatistically significant difference was found in PACU, 1st hour and 2nd hour evaluations (p<0.05). Both Group ES and Group L; It was found to be superior when compared to Group P. Conclusion: As a result, we have seen that both thoracic ESP block and IV lidocaine infusion method can be effective analgesic methods in patients undergoing thoracotomy, reduce the need for additional analgesics and the amount of opioid consumption in the postoperative period and bring VAS scores at rest and activity to tolerable levels in the early postoperative period. In the light of these data, we think that IV lidocaine infusion and ESP block can be used for intraoperative and postoperative analgesia in thoracotomy surgeries, but we could not show that these two techniques are superior to each other in terms of analgesics.
