Özet
GİRİŞ VE AMAÇ: COVID-19; Koronavirüs hastalığı 2019; insanları etkileyen, şiddetli akut solunum yolu sendromu koronavirüs 2'nin (SARS-CoV-2) neden olduğu bulaşıcı bir solunum yolu hastalığıdır. İlk olarak 2019 yılında Çin'in Wuhan şehrinde keşfedilmiş hastalık, görüldüğünden bu yana dünya çapında yayılarak 2019-2020 koronavirüs pandemisine yol açmıştır. Dünya geneline yayılan 2019 koronavirüs salgınının ülkemizdeki ilk vakası Türkiye Cumhuriyeti Sağlık Bakanlığı tarafından 10 Mart 2020'de açıklandı. Ülkedeki virüse bağlı ilk ölüm ise 15 Mart 2020'de gerçekleşti. Sağlık Bakanlığı 1 Nisan 2020'de koronavirüs vakalarının tüm Türkiye'ye yayıldığını açıkladı. Covid-19 pnömonisinin aşılanması yeni olarak başlanmış olup yeterli değildir, ayrıca etkin bir tedavisi halen bulunmamaktadır. Bu nedenle bulaşın erken tanınması, tedavi protokolünün erken ve etkin düzenlenmesi, yatan hastalarda laboratuvar parametrelerinin, risk faktörlerinin, skorlama sistemlerinin kullanılması, pandemi nedeniyle hastane başvurularının, servis ve yoğun bakım yatışlarının fazla olacak olması ön görülecek olursa; hastane yatış gerekliliği ve süresi, hangi hastaların mortal seyredeceğinin ön görülmesi amaçlanmıştır. GEREÇ VE YÖNTEM: Sağlık Bilimleri Üniversitesi Şişli Hamidiye Etfal Eğitim ve Araştırma Hastanesi klinik araştırmalar etik kurul onayı ile Acil Tıp Kliniği'ne başvuran PCR sonucu pozitif olan, Covid-19 tanısı almış ve acil servisten hastaneye yatışı verilen hastalar dahil edildi. Hastaların yaş, cinsiyet, ek hastalıkları, vital bulguları, üre, kreatinin, sodyum, glukoz, ferritin, d-Dimer, laktat, hemogram ve kan gazı sonuçları, hastanede yattıkları süre ve mortalite bilgileri hastane bilgi yönetim sisteminden eksiksiz olarak ulaşılan 312 hasta çalışmaya dahil edilmiştir. Dahil edilen hastaların CURB-65 ve PSI skorları hesaplanmıştır. BULGULAR: Çalışmaya dahil edilen toplam 312 hastanın 175'i (%56,1) erkek ve 137'si (%43,9) kadın olup; yaş ortalaması 58,2±16,1 idi. Hastaların mortalite oranı %16 olarak saptandı. Hastaların mortalite gün sayısı 12,6±7,5 olarak, hastanede yatış süresi 10,6±6,4 idi. Hastaların 185'inde (%59,3) ek hastalık yoktu. 127 (%40,7) ek hastalığı olan kişilerin ise hastalıkları şöyleydi; Diyabetes Mellitus (DM) 50 (%16), ix Hipertansiyon (HT) 72 (%23,1), Koroner Arter Hastalığı (KAH) 27 (%8,7), Astım-Kronik Obstrüktif Akciğer Hastalığı (KOAH) 21 (%6,7), Kronik Böbrek Yetmezliği (KBY) 18 (%5,8), Konjestif Kalp Yetersizliği (KKY) 6 (%1,9), Serebro Vasküler Olay (SVO) 4 (%1,3), bilinen kanser 4 (%1,3), diğer hastalıklar 34 (%10,9) idi. CURB-65 skorunun ortalaması 0,79±1,01, PSI skorunun ortalaması 79,6±39,9, PSI risk sınıflamasının ortalaması 2,65±1,44 iken risk sınıflamasının dağılımı; sınıf Ⅰ'de 101 (%32,4), sınıf Ⅱ'de 55 (%17,6), sınıf Ⅲ'de 47 (%15,1), sınıf Ⅳ'de 70 (%22,4), sınıf Ⅴ'te 39 (%12,5) hasta idi. CURB-65, PSI ve PSI risk sınıfı skorlamaları yaş, d-dimer, ferritin, NLR ve internasyon süresi ile pozitif yönde anlamlı saptandı (p<0,001, tümünde). CURB-65 ve PSI skoru mortalite günü ile pozitif yönde anlamlı iken (p<0,004 ve p<0,044), PSI risk sınıflaması ile negatif yönde anlamlı saptandı (p<0,073). Laktat ise CURB-65, PSI ve PSI risk sınıfı skorlamalarının tümüyle negatif yönde anlamlı idi (p<0,218, p<0,075, p<0,082, sırasıyla). Ek hastalığı olanların CURB-65, PSI ve PSI risk sınıf skorlamaları olmayanlara göre istatiksel olarak anlamlı yüksekti (p<0,001, tümünde). Ek hastalıklar ayrı ayrı incelendiğinde; DM CURB-65 ile istatiksel olarak anlamlı değilken (p<0,078), PSI ve PSI risk sınıfı ile anlamlı olarak yüksekti (p<0,001). HT ve KAH; CURB-65, PSI ve PSI risk sınıfı skorlamalarının tümüyle istatiksel olarak anlamlı idi (p<0,001, tümünde). Astım ve KOAH; CURB-65, PSI ve PSI risk sınıfı skolarmalarıyla istatiksel olarak anlamlı sonuç vermedi (p<0,389, p<0,889, p<0,654, sırasıyla). KKY'de ise; CURB-65 ile istatiksel olarak anlamlı değil iken (p<0,602), PSI ve PSI risk sınıfı skorlamaları ile anlamlı idi (p<0,047, p<0,040, sırasıyla). Mortalite ile sonuçlanan vakalarda CURB-65, PSI ve PSI risk sınıfı skorlamaları, mortal olmayanlara göre istatiksel olarak anlamlı saptandı (p<0,001, tümünde). ROC (Reciever Operator Characteristics) Eğrisi altındaki alan en yüksek 0,851 ile PSI skoru, ikinci 0,833 ile PSI risk sınıfı skoru, en düşük ise 0,795 ile CURB 65 skorlaması idi. Mortaliteyi belirlemede; CURB 65 skoru için %86,0 sensivite, %61,1 spesifite, PSI skoru için %82,0 sensivite, %70,6 spesifite, PSI risk sınıfı skoru için %76,0 sensivite, %72,9 spesifite idi. SONUÇ: Çalışmamızda, yüksek CURB-65, PSI skorları ve PSI risk sınıflaması değeri mortaliteyi öngörmede anlamlı prediktörler olarak saptandı. Bu parametrelerin ilk başvuruda COVID-19'lu hastalarda laboratuvar bulguları ile konfirme edilerek mortalitenin prediktörleri olarak kullanılması değerlendirilebilir

INTRODUCTION AND AIM: Coronavirus disease 2019; Severe acute respiratory syndrome that affects humans is an infectious respiratory illness caused by coronavirus 2 (SARS-CoV-2). First discovered in 2019 in Wuhan, China, the disease has spread worldwide since its appearance, causing the 2019-2020 coronavirus pandemic. 2019 is the first case in our country to spread worldwide outbreak of coronavirus Republic of Turkey Ministry of Health announced on March 10 by 2020. The first death due to the virus in the country occurred on March 15, 2020. The Ministry of Health has announced that it spread all over Turkey in case of coronavirus April 1, 2020. Vaccination of Covid-19 pneumonia has just begun and is not sufficient, and there is still no effective treatment. Therefore, if it is anticipated that early detection of the contamination, early and effective regulation of the treatment protocol, the use of laboratory parameters, risk factors and scoring systems in inpatients, hospital admissions, service and intensive care admissions due to the pandemic; the requirement and duration of hospitalization, predicting which patients will be mortal. MATERIALS AND METHODS: Patients who applied to the Emergency Medicine Clinic with the approval of the Clinical Research Ethics Committee of Health Sciences University Şişli Hamidiye Etfal Training and Research Hospital, who had a positive PCR result, were diagnosed with Covid-19 and were hospitalized from the emergency department were included. 312 patients, whose age, gender, comorbidities, vital signs, urea, creatinine, sodium, glucose, ferritin, d-Dimer, lactate, hemogram and blood gas results, hospital stay and mortality information were obtained from the hospital information management system, were included in the study. CURB 65 and PSI scores of the included patients were calculated. RESULTS: Of the 312 patients included in the study, 175 (56.1%) were men and 137 (43.9%) were women; mean age was 58.2 ± 16.1 years. The mortality rate of the patients was found to be 16%. The number of mortality days was 12.6 ± 7.5 and the duration of hospitalization was 10.6 ± 6.4 days. 185 (59.3%) of the patients had no additional disease. The diseases of 127 (40.7%) people with comorbid diseases were xii as follows; Diabetes Mellitus (DM) 50 (16%), Hypertension (HT) 72 (23.1%), Coronary Artery Disease (CAD) 27 (8.7%), Asthma-Chronic Obstructive Pulmonary Disease (COPD) 21 (6,7%), Chronic Renal Failure (CRF) 18 (5.8%), Congestive Heart Failure (CHF) 6 (1.9%), Cerebro Vascular Event (CVE) 4 (1.3%), known cancer 4 (1.3%), other diseases were 34 (10.9%). The average of the CURB-65 score was 0.79 ± 1.01, the mean of the PSI score was 79.6 ± 39.9, the mean of the PSI risk classification was 2.65 ± 1.44; 101 (32.4%) in class Ⅰ, 55 (17.6%) in class Ⅱ, 47 (15.1%) in class Ⅲ, 70 (22.4%) in class Ⅳ, class Ⅴ 39 (12.5%) patients were. CURB-65, PSI and PSI risk class scores were found to be positively significant with age, d-dimer, ferritin, NLR, and duration of hospitalization (p<0.001 in all). While the CURB-65 and PSI score were positively significant with the day of mortality (p<0.004 and p<0.044), it was found to be negatively significant with the PSI risk classification (p<0.073). Lactate was completely negative for CURB-65, PSI and PSI risk class scores (p<0.218, p<0.075, p<0.082, respectively). CURB-65, PSI and PSI risk class scores of patients with comorbidities were statistically significantly higher than those without (p<0.001, in all). When additional diseases are examined separately; While DM was not statistically significant with CURB-65 (p<0.078), it was significantly higher with PSI and PSI risk class (p<0.001). HT and CAD; CURB-65, PSI and PSI risk class scores were all statistically significant (p<0.001, in all). Asthma and COPD; CURB-65 did not give statistically significant results with PSI and PSI risk class scores (p<0.389, p<0.889, p<0.654, respectively). In CHF; while it was not statistically significant with CURB-65 (p<0.602), it was significant with PSI and PSI risk class scores (p<0.047, p<0.040, respectively). CURB-65, PSI and PSI risk class scores in cases with mortality were statistically significant compared to non-mortal ones (p<0.001, in all). The area under the ROC (Reciever Operator Characteristics) Curve was the highest PSI score with 0.851, the second was the PSI risk class score with 0.833, and the lowest was the CURB-65 score with 0.795. In determining mortality; It was 86.0% sensitivity, 61.1% specificity for the CURB 65 score, 82.0% sensitivity, 70.6% specificity for the PSI score, 76.0% sensitivity and 72.9% specificity for the PSI risk class score. CONCLUSION: In our study, high CURB-65, PSI scores and PSI risk stratification were found to be significant predictors of mortality. It can be evaluated that these parameters are used as predictors of mortality by confirming with laboratory findings in patients with COVID-19 at the first application
