Özet

Çalışmamızda kronik inmeli hastalarda üst ekstremite el fonksiyonlarının gelişiminde yoğunlaştırılmış konvansiyonel tedaviye ek olarak uygulanan modifiye zorunlu kullanım hareket terapisinin (mZKHT), tek başına uygulanan yoğunlaştırılmış konvansiyonel tedaviye göre etkinliği olup olmadığını karşılaştırmak amaçlandı. Bu amaçla katılımcıların nörofizyolojik gelişim, spastisite, fonksiyonel bağımsızlık, motor gelişim, fonksiyonellik ve depresyon parametreleri değerlendirildi. Kronik inmeli 40 hasta Grup 1'de 20 ve Grup 2'de 20 olmak üzere randomize edilerek iki gruba ayrıldı. Birinci gruba yoğunlaştırılmış konvansiyonel tedaviye ek olarak mZKHT eklendi. İkinci gruba ise yalnızca yoğunlaştırılmış konvansiyonel tedavi uygulandı. Yoğunlaştırılmış konvansiyonel tedavi grubu günde 2 saat, haftada 5 gün toplam 30 seans tedavi aldı. mZKHT grubu ise gün içerisinde birer saat arayla 6 saat bir omuz askısıyla kısıtlanıp yoğunlaştırılmış konvansiyonel tedaviye ek olarak 2 saat egzersiz programına alındı. Toplam haftada 5 gün 30 seans tedavi uygulandı. Hastaların tedavi öncesi (TÖ), tedavi sonrası (TS) ve tedavi sonrası üçüncü ay (TS-3.ay) değerlendirmelerinde Brunnstrom Evrelemesi, Modifiye Ashworth Skalası (MAS), Fonksiyonel Bağımsızlık Ölçeği (FBÖ), Fugl-Meyer Değerlendirmesi (FMD), Motor Aktivite Günlüğü-28 (MAG-28) ve Beck Depresyon Ölçeği (BDÖ) kullanıldı. Gruplar arasında yaş, cinsiyet, boy, kilo, vücut kitle indeksi (VKİ), eğitim, meslek, paretik el, serebrovasküler olay (SVO) tipi ve hastalık süresi bakımından istatistiksel olarak anlamlı fark yoktu. Grupların başlangıçtaki Brunnstrom evresi, MAS değerlendirme skorları, FBÖ puanları, FMD üst ekstremite motor skalası, MAG-28 ve BDÖ değerleri arasında istatistiksel olarak anlamlı bir fark bulunmadı. TS dönemde, her iki grupta da Brunnstrom evresi, FBÖ puanları, FMD üst ekstremite motor skalası ve MAG-28 skorlarında anlamlı düzeyde iyileşme saptandı. Bu iyileşme halinin TS-3. ayda da devam ettiği görüldü. Ancak iki grup arasında istatiksel olarak anlamlı fark saptanmadı. BDÖ değerleri açısından iki grupta ve gruplar arasında anlamlı bir değişiklik saptanmadı. TS dönemde, MAS değerlendirme skorlarında iki grup arasında fark saptanmadı. Sonuç olarak, kronik inmeli hastalarda yoğunlaştırılmış konvansiyonel tedavi programına ilave olarak uygulanan mZKHT'nin üst ekstremitede motor iyileşmeye ek yararı olduğu saptanmamıştır.

In our study, it was aimed to compare whether modified constraint-induced movement therapy (mCIMT) applied in addition to intensive conventional therapy in the development of upper extremity hand functions in patients with chronic stroke is effective compared to intensive conventional therapy administered alone. For this purpose, neurophysiological development, spasticity, functional independence, motor development, functionality and depression parameters of the participants were evaluated. 40 patients with chronic stroke were randomized into two groups as 20 in Group 1 and 20 in Group 2. In addition to the intensive conventional treatment, mCIMT was added to the first group. The second group received only intensive conventional therapy. The intensive conventional treatment group received a total of 30 sessions of treatment 2 hours a day, 5 days a week. The mCIMT group, on the other hand, was restricted with a shoulder sling for 6 hours at 1 hour intervals during the day and included in a 2-hour exercise program in addition to the intensive conventional treatment. A total of 30 sessions of treatment were applied 5 days a week. Brunnstrom Staging, Modified Ashworth Scale (MAS), Functional Independence Scale (FIM), Fugl-Meyer Assessment (FMA), Motor Activity Log-28 (MAG-28) and Beck Depression in the pre-treatment, post-treatment and third month evaluations Scale (BDI) was used. There was no statistically significant difference between the groups in terms of age, gender, height, weight, body mass index (BMI), education, occupation, paretic hand, type of cerebrovascular event (CVE) and disease duration. There was no statistically significant difference between the groups' initial Brunnstrom stage, MAS assessment scores, FIM scores, FMD upper extremity motor scale, MAG-28 and BDI values. In the post-treatment period, a significant improvement was found in both groups in Brunnstrom stage, FIM scores, FMD upper extremity motor scale and MAG-28 scores. It was observed that this recovery continued in the 3rd month after the treatment. However, there was no statistically significant difference between the two groups. There was no significant difference in BDI values in the two groups and between the groups. There was no difference between the two groups in terms of MAS evaluation scores in the post-treatment period. As a result, it has not been determined that mCIMT applied in addition to the intensive conventional treatment program in chronic stroke patients has an additional benefit for motor recovery in the upper extremity.
