Özet

Bu araştırma, prematüre bebek annelerinde Watson'ın modeli ile uygulanan kanguru bakımının laktasyon sürecine ve kaygı düzeyine etkisini belirlemek amacıyla yapılmıştır. Ön test– son test kontrol gruplu yarı deneysel araştırmanın evrenini, bir üniversite hastanesinin yenidoğan yoğun bakım ünitesinde yatan prematüre bebekler ve anneleri oluşturmuştur. Girişim grubuna 45, kontrol grubuna 45 anne-bebek çifti dâhil edilmiştir. Veriler Anne-bebek tanıtıcı bilgiler formu, Durumluk Kaygı Ölçeği, Emzirme öz-yeterlik ölçeği – kısa formu, LATCH Emzirme Tanılama Ölçüm Aracı, Günlük Süt Miktarı Kayıt Formu ve Watson İyileştirme Süreçlerine Göre Hasta Memnuniyeti Değerlendirme Formu ile toplanmıştır. Kontrol grubuna herhangi bir girişimde bulunulmamıştır. Girişim grubundaki annelerle doğumdan sonra anne kadın doğum servisine geldikten sonraki ilk 1 saat içinde görüşülerek, Durumluk Kaygı Ölçeği ve Emzirme Öz- Yeterlilik Ölçeği uygulanmıştır. Girişim grubu annelere Watson insan bakım modeli temelinde üç gün süreyle günde iki kez 40 dakika süren kanguru bakımı uygulanmıştır. Verilerin değerlendirilmesinde sayı, yüzde dağılımları, Kolmogorov-Smirnov, Independent Sample-t test, Mann-Whitney U, Wilcoxon, Friedman, Pearson χ2 testi kullanılmıştır. Girişim grubundaki annelerin son değerlendirme kaygı puanlarının ön değerlendirmeye göre önemli fark göstermediği ancak son değerlendirmenin kontrol grubununa göre istatistiksel olarak önemli düzeyde düşük olduğu saptanmıştır (p<0,05). Girişim grubundaki annelerin emzirme öz-yeterlilik puanlarının kontrol grubuna göre anlamlı ölçüde yükseldiği (p<0,001) saptanmıştır. Girişim ve kontrol grubunun son değerlendirme LATCH puanlarının ön değerlendirme puanlarına göre anlamlı düzeyde arttığı (p<0,001), LATCH puanındaki artışın girişim grubunda kontrol grubuna göre anlamlı düzeyde daha fazla olduğu (p<0,001) belirlenmiştir. Girişim ve kontrol grubundaki annelerin süt miktarının her gün bir önceki güne göre istatistiksel olarak anlamlı düzeyde artığı (p<0,001) ancak girişim grubunda süt miktarının tüm günlerde kontrol grubuna göre daha yüksek olduğu (p<0,05) bulunmuştur. Çalışma sonunda girişim grubundaki annelerin memnuniyet puan ortalamaları kontrol grubuna göre anlamlı düzeyde yüksek bulunmuştur (p<0,001). Araştırma bulguları prematüre yenidoğan annelerine düzenli şekilde uygulanan ve Watson insan bakım modeliyle desteklenen kanguru bakımının anne kaygı düzeyini azalttığı ve emzirme etkinliğini artırdığını götsermektedir. Kanguru bakımının Watson insan bakım modeli ile yenidoğanlara uygulanması ve prematüre bebek annelerinde bu bakımın etkinliğini değerlendiren yeni çalışmalar yapılması önerilir.

This research was carried out to determine the effects of kangaroo care applied via Watson's model on lactation process and anxiety level on mothers of premature infants. The universe of this quasi-experimental study with pretest-posttest control group design was consisted of premature infants hospitalized in neonatal intensive care unit of a university hospital and their mothers. There were 45 mother-infant pairs in both the intervention group and the control group. The data were collected with demographic form for mother and infant, the State Anxiety Scale, the Breastfeeding self-efficacy scale - short form, LATCH Breasfeeding Diagnosis Measurement Tool, Daily Milk Wolume Checklist and Patient Satisfaction Evaluation Form According to Watson's İmprovement Process. No similar intervention was provided for the control group. The mothers in the intervention group were interviewed at the inpatient clinic of Gynecology and Obstetrics Department within the first hour of childbearing, and the State Anxiety Scale and the Breastfeeding Self-Efficacy Scale were applied. Kangaroo care was applied to the mothers in the intervention group for forty minutes twice a day for three days based on the Watson's Human Caring model. Percentage, frequency, Kolmogorov-Smirnov, Independent Sample-t test, Mann-Whitney U, Wilcoxon, Friedman, Pearson x2 tests were used for data evaluation. No significant difference was found between posttest and pretest anxiety scores of mothers in the intervention group; but posttest scores of the intervention group was significantly lower compared to control group (p <0,05). Posttest Breastfeeding Self-Efficacy Scale scores of mothers in both groups were significantly higher than the pretest scores (p <0,001), and this increase was higher in the intervention group (p <0,001). Similarly, the LATCH scores of the intervention and the control groups significant increase compared to the pretest scores (p <0.001), and this increase in the LATCH score was significantly higher in the intervention group than the control group (p<0,001). The milk wolume of mothers in the intervention and the control groups was significantly higher compared to the day before (p<0,001) but the milk wolume of the intervention group remained higher than the control group throughout the study (p<0,05). The mean satisfaction score of the mothers in the intervention group was found significantly higher than the control group (p<0,001). Results of the study show that KC, supported by Watson's Human Caring Model, decreases level of mother anxiety and increases efficient breastfeeding when applied regularly to mothers of premature newborns. Application of KC based on Watson's Human Caring model to newborns, and future research evaluating the effect of this care on mothers of premature infants are recommended.
