Özet

Benign prostat hiperplazisinin tedavisinde en etkili yöntem olmaya devam eden transüretral prostatektomi“altın standart”olarak kabul edilmektedir. Bununla birlikte transüretral prostatektomi önemli oranda bir morbiditeyi beraberinde taşımaktadır. Bilinen elektrocerrahi ilkelerinin basitçe uyarlanması ile transüretral prostatetektomin istenen etkileri, kanama ve sıvı absorbsiyonu gibi morbiditeler görülmeksizin, TVP ile elde edilebilmektedir. Bu çalışmada, elektrovaporizasyon yöntemini kullanan TVP ile TURP karşılaştırılarak incelenmiştir. Benign prostat hiperplazili 21 hastaya TVP ve 20 hastaya TURP uyguladık. Ortalama 10.2 + 0,9 ay olguları takip ettik. Yöntemlerin etkinliğini değerlendirmek için; AUA semptom skoru, uroflowmetri bulguları, miksiyon sonrası rezidüel idrar miktarı, prostat ağırlığı, üretral kateterin çekilme süresi, kan hematokrit ve sodyum değerlerindeki değişiklikler, ile takip süresince görülen komplikasyonlar karşılaştırıldı. TVP; ameliyat sırasında minimal kanama olması, üretral kateterin erken dönemde çekilebilmesi, postoperatif hastanede kalış süresinin çok kısa olması nedeniyle oldukça etkin bulunmuştur. Operatif ve postoperatif dönem verileri oldukça başarılı olan TVP, benign prostat hiperplazisi tedavisinde TURP'ye alternatif bir tedavi yöntemi olabileceği görülmüştür.

Transurethral resection, the most effective treatment modality for benign prostatic hyperplasia, is considered“ gold standard ”. However, it carries a significant risk of morbidity as well. TVP is able to achieve the desired effects of transurethral prostatectomy with simple application of well known electrosurgical principles. Besides, TVP has much less risk of bleeding and water absorption when compared to TURP. This study has designed to compare TURP with TVP which uses electrovaporization. Twenty one patients with BPH underwent TVP, while the number of patients undergoing TURP was 20. Mean follow up of the patients was 10.2+ 0.9 months. In order to evaluate the effectiveness of the procedures, we compared the groups in terms of: AUA symptom scores, uroflowmetric values, postvoiding residual urine volume, prostatic weight, catheterization period, the changes in hematocrit and decrease in sodium levels, and late complications encountered during follow up. TVP was found to be quiet effective as it causes rninimal bleeding during operation and requires short hospitalization. TVP which was found to be effective during operation and post operative period can be considered as an alternative to TURP for the treatment of benign prostatic hyperplasia.
