Özet

Giriş ve Amaç: Çalışmamızın amacı kolorektal kanser nedeni ile opere edilen hastaların histopatolojik, onkolojik ve cerrahi sonuçlarının ve komplikasyonlarının analiz edilmesi ve reoperasyon nedenlerinin değerlendirilmesidir. Mataryal ve Metod: Çalışmaya Ümraniye Eğitim Araştırma Hastanesi'ne 2010-2015 tarihleri arasında genel cerrahi kliniğinde kolorektal karsinom tanısıyla opere olan toplam 473 hasta dahil edildi. Hastalara ait bilgiler hastane kayıtlarından retrospektif olarak elde edilerek analizler yapıldı. Bulgular: Toplam 473 hastanın 267'si (%56.4) erkek, 206'sı (%43.6) kadın idi. Çalışmamızda sağkalım oranı %44 olarak bulundu. Cinsiyetler arasında erken komplikasyon (p=0.221) ve reoperasyon (p=0.118) oranları açısından fark yoktu. Kolorektal kanser tanısı konulan olgu sayılarının yıllara göre artış gösterdiği görüldü. En sık görülen reoperasyon nedenleri; ileostomi kapatılması (%9.3) ve kolostomi kapatılması (5.3) idi. Yetmiş yaş ve üzeri olan gruptaki erken komplikasyon oranı (%10.8) daha alt yaş gruplarına göre anlamlı yüksekti (p=0.022), bu gruptaki reoperasyon oranı (%21.1) diğer yaş gruplarına göre anlamlı düşüktü (p<0.001). Transvers kolonda tümör yerleşimi bulunan hastalarda erken komplikasyon oranının (%31.6) diğer yerleşimlere göre anlamlı yüksek olduğu (p=0.031), ancak distal bölgelerde yerleşimi olanlarda reoperasyon oranının (%38.4) diğer yerleşimlere göre anlamlı yüksek olduğu görüldü (p=0.025). Acil olarak opere edilen hastalarda erken komplikasyon oranı (%12.8) elektif cerrahi planlanan hastalara göre anlamlı yüksekti (p<0.001). Kolorektal karsinomların histopatolojik tanısında olguların %46,6'sının adenokarsinom olduğu görüldü, bu hastalarda hayatta kalma oranı (%63.1) diğer tanı konulan hastaların oranından (%38.5) anlamlı olarak yüksekti (p=0.026). Hastaların %7.9'unda erken komplikasyon görüldü. Erken komplikasyonu olanlarda reoperasyon oranı anlamlı düşük bulundu.(p<0.001). Hastaların %38.3'ü reopere olmuştu. Hastaların %2.1'inde ise nüks görüldü. Tartışma ve Sonuç: Çalışmamızda birçok değişken ile erken komplikasyon, reoperasyon ve sağkalım oranlarını analiz ettik. Sonuç olarak; kolorektal karsinomlu hastaların prognozunda yaş, tümör yerleşim yeri, ameliyatın acil olarak uygulanması, ostomi kapatma süresi, kanserin türü, post-op yatış süresi, çıkarılan lenf nodu sayısı, takip süresi, pT evresi ve erken komplikasyon varlığının etkili ve belirleyici olabileceğini bulduk. Bu verilerin gelecek çalışmalara ışık tutabileceği düşüncesindeyiz.

Objective: The aim of this study was to analyze the histopathological, oncological and surgical results and complications of patients who underwent surgery for colorectal cancer, and to evaluate the causes of reoperation. Material and Methods: A total of 473 patients who were operated for the diagnosis of colorectal carcinoma in the General Surgery Clinics of Ümraniye Training and Research Hospital between 2010 and 2015 were included in the study. The data of the patients were retrospectively obtained from the hospital records and analyzed. Results: Of the 473 patients, 267 (56.4%) were male, and 206 (43.6%) were female. The survival rate in our study was 44%. There was no difference between the genders in terms of early complication (p = 0.221) and reoperation rates (p = 0.118). The number of cases diagnosed with colorectal cancer increased with years. The most common causes of reoperation were ileostomy closure (9.3%) and colostomy closure (5.3). The rate of early complications in the group of 70 years of age (10.8%) was significantly higher than in the lower age group (p = 0.022), and the reoperation rate in this group (21.1%) was significantly lower than the other age groups (p <0.001). The rate of early complication was significantly higher in the patients with tumor placement in the transverse colon (31.6%) than the other sites (p = 0.031), but the rate of reoperation (38.4%) in the distal regions was significantly higher in comparison to the other sites (p = 0.025). The rate of early complication (12.8%) was significantly higher in patients who underwent emergency surgery compared to the patients who underwent elective surgery (p <0.001). The histopathological diagnosis of colorectal carcinomas revealed 46.6% of the cases with adenocarcinoma, and the survival rate (63.1%) in these patients was significantly higher than the rate of other diagnosed patients (38.5%) (p = 0.026). Early complication was seen in 7.9% of the patients. Reoperation rate was significantly lower in patients with early complications (p <0.001). A total of 38.3% of the patients were reoperated, and 2.1% of the patients had recurrence. Conclusion: Our study is one of the most comprehensive studies of colorectal carcinoma surgery performed in our country. In the study, we analyzed the early complications, reoperation and survival rates with many variables. In conclusion, we found that age, tumor location, emergency operation, ostomy closure time, type of cancer, post-op hospital stay, number of lymph nodes removed, follow-up period, pT stage and early complications can be effective and determinant in the prognosis of patients with colorectal carcinoma. We consider that these data can shed light on future studies.
