Özet

AMAÇ: Bu çalışmada, kliniğimizde Kasım 2009 – Mart 2014 tarihleri arasında 7 mm ile 20 mm arasındaki böbrek taşları nedeniyle ESWL uygulanan 120 hastanın verileri incelenerek, NKBT ile ölçülen taş özelliklerinin ESWL başarısına olan etkisi incelendi. YÖNTEM ve GEREÇ: Ankara Üniversitesi Tıp Fakültesi Üroloji Anabilim Dalı'nda Kasım 2009 – Mart 2014 tarihleri arasında 7 mm ile 20 mm arasındaki böbrek taşları nedeniyle ESWL uygulanan 120 hasta çalışmaya dahil edildi. Tüm hastalar ESWL öncesi NKBT ile değerlendirilerek taş dansitesi, cilt – taş mesafesi, taş yerleşimi ve taş boyutu değerleri kaydedildi. ESWL tedavisi sonrası 1. ayda hastalar NKBT ve USG ile değerlendirildi ve taş özelliklerinin ESWL tedavisinin başarısına olan etkileri incelendi. BULGULAR: Çalışmaya dahil edilen 120 olgunun yaş aralığı 19-72, ortalama yaş 42,65 (± 14,3), ortalama VKİ değerleri 23,32 kg/m2 (± 3,66), taş boyutu ortalama 11,95 mm (± 2,66), taş dansitesi ortalama 898,75 HU (± 240,53), ve cilt – taş mesafesi ortalama 96,23 mm (± 23,86), olarak hesaplandı. Çalışma sonunda 80 hastada (%66,7) taşsızlık sağlanırken 40 hastada (%33,7) rezidü taş saptandı. Taşsızlık sağlanan hastaların 28'inde (%23,3) klinik önemsiz rezidüel fragman (KÖRF) mevcuttu. Taşsızlık sağlanan grubun ortalama taş dansitesi 826,8 HU (± 212,71), rezidü taşı kalan grubunsa ortalama taş dansitesi 1042,67 HU (± 230,25) olarak bulundu (p<0,001). Cilt – taş mesafesi taşsızlık sağlanan grupta ortalama 89,34 mm (± 17,42) ölçülürken; rezidü taşları kalanlarda ortalama 110,02 mm (± 28,81) olarak saptandı (p<0,001). Alt kaliks taşları ile pelvis taşlarının ESWL başarısı arasındaki fark istatistiksel olarak anlamlı bulundu (p=0,009). Ayrıca, pelvis taşları, üst ve orta kaliks taşları bir grup olarak değerlendirildiğinde ve bu grup alt kaliks taşlarıyla karşılaştırıldığında; ESWL başarısının yine istatistiksel olarak anlamlı olduğu görüldü (%71 vs. %45; p=0,024). Taş boyutu, taşsızlık sağlanan grupta ortalama 11,6 mm (±2,52) iken, rezidü taşları kalan grupta ortalama 12,65 mm (± 2,84) idi (p=0,053). Yapılan çok değişkenli analizlerde taş dansitesi (p=0,002; OR:1,004 [1,001-1,006]), cilt – taş mesafesi (p=0,005; OR:1,030 [1,009-1,051]) ve taşın alt kaliks yerleşimli olmasının (p=0,047; OR:3,193 [1,018-10,018]) taşsızlık sağlamada anlamlı olduğu saptandı. Taş boyutu ise taşsızlığı göstermede istatistiksel olarak anlamlı bulunmadı (p=0,151). Yapılan ROC analizinde 960 HU değerinin %77,5 duyarlılık ve %64 özgüllük ile ESWL tedavisi sonrasında taşsızlığı göstermede anlamlı olabileceği gösterildi. SONUÇ: Literatürde ESWL tedavisini öngörmede kullanılacak faktörlerle ilgili yapılmış çalışmalar mevcuttur. Çalışmamızda 960 HU ve üzerindeki değerlerde taş dansitesine sahip ve cilt – taş mesafesi uzun olan hastaların ESWL tedavisinden daha az fayda görecekleri belirlenmiştir. Bu nedenle ESWL tedavisine uygun hastaları seçerken, NKBT çekerek bu kriterleri göz önünde bulundurmak faydalı olacaktır.

PURPOSE: Between November 2009 and March 2014, 120 patients with kidney stones ranging between 7 – 20 mm were treated in our department with ESWL. The effect of stone characteristics in non-contrast CT on treatment outcome was evaluated. MATERIALS and METHODS: Between November 2009 and March 2014, 120 patients who undergone to ESWL treatment at Department of Urology, School of Medicine, Ankara University were included in this study. All patients had a NCCT before ESWL treatment and stone density, distance from skin to stone, stone location and stone size were recorded. Patients were re-evaluated with NCCT and USG 1 month after ESWL and the effect of stone parameters on treatment outcome was evaluated. Mann – Whitney U, Wilcoxon W and Pearson Chi – Square tests and ROC analysis were used for statistical analysis and p<0,05 were accepted for statistical significance. RESULTS: Mean age of the patients was 42,65 (± 14,3), mean BMI was 23,32 kg/m2 (± 3,66), mean stone size was 11,95 mm (± 2,66), mean stone density was 898,75 HU (± 240,53) and mean distance from skin to stone was 96,23 mm (± 23,86). At the end of study; 80 patients (%66,7) were stone free and ESWL treatment was not successful for 40 patients (%33,7). Of those 80 stone free patients 28 (%28,3) had clinically insignificant residual stone fragments. In the stone free group mean stone density was 826,8 HU (± 212,71) whereas in patients with unsuccessful treatment it was found to be 1042,67 HU (± 230,25) (p<0,001). Mean distance from skin to stone was 89,34 mm (± 17,42) and 110,02 mm (± 28,81) in stone free group and in patients with unsuccessful treatment, respectively (p<0,001). The difference of ESWL success between lower calyceal and pelvis located stones was statistically significant (p=0,009). Also; pelvis, middle calyceal and upper calyceal stones grouped together and compared with lower calyceal stones, ESWL success rates was higher and this finding was also statistically significant (%71 vs. %45; p=0,024). There were no statistically significant difference for stone size between stone free and unsuccessful treatment groups (p=0,053). In multivariate analysis, stone density (p=0,002; OR: 1,004 [1,001-1,006]), distance from skin to stone (p=0,005; OR: 1,030 [1,009-1,051]) and lower calyceal stones (p=0,047; OR: 3,193 [1,018-10,018]) were found to be statistically significant predictors for being stone free. On the other hand, stone size was not found to be a statistically significant factor for being stone free (p=0.151). In the ROC analysis, 960 HU was found to be the threshold value with %77,5 sensitivity and %64 specifity for determining ESWL success. CONCLUSION: In the literature, many studies took place for determining factors affecting ESWL success. In our study we detected stone density ≥960 HU and longer skin to stone distance values as strong predictors of unsuccessful treatment. Thus, these factors should have to be taken into account for selecting patients for ESWL treatment.
