Özet
Hem kaynak kıtlığı hem de artan sağlık maliyetleri nedeniyle ülkelerin sağlık sistemlerinin performans ölçümü ve karşılaştırılması önemli bir konu haline gelmiştir. Bu nedenle, bu tez 2000-2010 dönemini kapsayan kapsamlı bir veri seti kullanarak OECD ülkelerindeki sağlık sistemlerinin performansını analiz etmektedir. Çalışmada, OECD sağlık istatistiklerinden elde edilen toplam sağlık çıktılarının üretimindeki göreli teknik etkinlikler, etkinlik ölçümü için en uygun yöntem kullanılarak değerlendirilmiştir. Çalışmada literatürde yaygın olarak kullanılan iki çıktı değişkeni (bebek ölüm oranı ve yaşam beklentisi), üç sağlık girdisi (hekim sayısı, hastane yatak sayısı, sağlık harcamaları) ve üç sosyal çevre girdisi (karbondioksit emisyonu, alkol tüketimi ve okul yaşam beklentisi) kullanılmıştır. Hem girdi hem de çıktı yönelimli Veri Zarflama Analizi (VZA) sonuçları 32 ülkeden 6 tanesinin etkin olduğunu göstermektedir. Ayrıca, ampirik bulgular herhangi bir ülkenin kaynaklarının kullanımında teknik etkin olma veya olmama durumunun, o ülkenin sağlık çıktı düzeyi ile ilişkili olmadığını da ortaya koymaktadır.

It became an important issue to assess the performance of health systems across countries due to the scarcity of resources and increasing health care costs. Therefore, this thesis contributes to the estimation of productive performance of OECD countries by employing a comprehensive dataset covering the period 2000-2010. By using one of the most convenient technique for efficiency measurement, it is analyzed the relative technical efficiency in the production of aggregate health outcomes by using OECD health data. The study uses the two most common health outcome measure (infant mortality rate and life expectancy), three health inputs (the number of physicians, the number hospital beds, health expenditure) and three social environmental inputs (CO2 emissions, alcohol consumption and school life expectancy). Application of data envelopment analysis (DEA) indicates that six countries out of 32 are found to be efficient with regard to both input- and output-oriented approaches. More importantly, empirical findings reveal that any country can be technically efficient or inefficient in the use of its resources regardless of the level of its health outcome.
