Özet
Alopesi areata (AA); saç, sakal gibi vücutta kıllı herhangi bir alanı etkileyen, skar yapmayan saç kaybı ile alevlenme ve remisyonlarla seyreden organa spesifik otoimmün bir hastalıktır. Etyopatogenezi tam olarak bilinmemekle beraber otoimmün hipotez üzerinde durulmaktadır. Hastalığın otoimmün patogeneze dayandırılmasının temel nedenlerinden biri bazı sistemik otoimmün hastalıklarla birlikte görülebilmesi ve bu hastalıklarla ilişkili otoantikorların saptanmasıdır.Çalışmadaki amacımız AA ile birlikte görülebilen otoimmün hastalık sıklığını saptayarak etyopatogenezde otoimmünitenin rolünü tartışmak, alopesi areatanın klinik ve demografik özelliklerini belirlemek idi.Çalışma kapsamına İstanbul Tıp Fakültesi Deri ve Zührevi Hastalıklar Anabilim Dalı Polikliniği'nde Ağustos 2011-Ocak 2012 tarihleri arasında AA tanısı ile izlenmiş yaşları 3-65 arasında değişen 134 hastanın kayıtları; yaş, cinsiyet, klinik tip, alopesi tutulum alanı, hastalık şiddeti, atak sayısı, hastalık süresi, son atak süresi, hastalığın başlangıç yaşı, nevus flammeus varlığı, tırnak tutulumu, emosyonel ve/veya fiziksel stres hikayesi, eşlik eden otoimmün hastalık hikayesi, ailede AA ve diğer otoimmün hastalık hikayesi, ailede ve hastada atopi hikayesi, AA'ya eşlik edebilen sistemik otoimmün hastalık tanısına yönelik kliniğimizde rutin olarak yapılan laboratuvar verileri retrospektif olarak incelendi. Saçlı deri tutulumu olan hastaların kıl kaybı yüzdesi Severity of Alopecia Tool (SALT) skoruna göre hesaplandı. Elde edilen veriler arasındaki ilişki ki-kare testi (X2) ve Fisher kesin ki-kare testi metodları ile analiz edildi. İstatistiksel anlamlılık p<0.05 ve iki yönlü olarak kabul edildi.Çalışmaya alınan 134 hastanın 55'i kadın (%41), 79'u erkek (%59), erkek:kadın oranı 1.4:1, yaş ortalaması 24.92±14.6 yıl idi. Hastaların %82.8'i 40 yaşın altındaydı. Hastalarda hastalık başlangıç yaşı 2-63 yaş arasında değişmekte olup, ortalaması 20.10±14.5 yıl olarak tespit edildi. Hastaların %53'ünde hastalık başlangıç yaşının 20 yaşın, %89.6'sında hastalık başlangıç yaşının 40 yaşın altında olduğu saptandı. Hastaların %72.4'ü bir atak geçirmişti. İki ve üstü atak; erkeklerde kadınlara göre (p=0.015), hastalık süresi 1 yıl ve üstü olanlarda hastalık süresi 1 yıl altında olanlara göre (p<0.001) ve juvenil başlangıç erişkin başlangıca göre (p=0.034) istatistiksel olarak anlamlı bulundu. Hastaların %86.5'inde AA, %3.7'sinde alopesi totalis (AT), %9.7'sinde alopesi universalis (AU) mevcuttu. Hastaların %77.6'sında hafif, %22.4'ünde şiddetli tutulum olduğu tespit edildi. Şiddetli tutulum; kadınlarda erkeklere göre (p=0.029), juvenil başlangıçta erişkin başlangıca göre (p=0.001), 1 yıl ve üstü hastalık süresinde 1 yıl altı hastalık süresine göre (p=0.001), 1 yıl ve üstü son atak süresinde 1 yıl altı son atak süresine göre (p<0.001) istatiksel olarak anlamlı bulundu. Hastaların %14.9'unda nevus flammeus mevcuttu. Nevus flammeusu olanlarda şiddetli tutulum istatistiksel olarak anlamlı idi (p=0.017). Hastaların %26.1'inde tırnak tutulumu gözlendi. En sık pitting bulgusu saptandı. Tırnak tutulumu; şiddetli tutulumda (p=0.015), hastalık süresi 1 yıl olan grupta (p=0.021), juvenil grupta (p<0.001), AT/AU grubunda (p=0.007) istatiksel olarak anlamlı bulundu. Hastaların %27.6'sında aile hikayesi vardı ve juvenil grupta istatistiksel olarak anlamlı düzeyde yüksek idi (p=0.034). Hastalık şiddeti ile aile hikayesi arasında; hafif tutulumda %26, şiddetli tutulumda %33.3 aile hikayesi saptandı. Hastalarda atopi hikayesi %8.2 bulundu ve atopi hikayesi ile hastalık şiddeti arasında istatistiksel olarak anlamlı bir ilişki bulunmadı (p>0.05). Hastaların %16.4'ünde otoimmün hastalık; otoimmün tiroidit %11.9, vitiligo %1.5 oranında tespit edildi. Otoimmün hastalık kadınlarda erkeklere göre (p=0.034), ailesinde otoimmün hastalık hikayesi olanlarda olmayanlara göre (p=0.011) daha yüksek idi. Cinsiyetle ile nevus flammeus, tırnak tutulumu, aile hikayesi, atopi arasında anlamlı ilişki saptanmadı (p>0.05). Hastalık başlangıç yaşı ile cinsiyet, nevus flammeus, atopi, otoimmün hastalık arasında anlamlı ilişki bulunmadı (p>0.05). Hastalık şiddeti ile atak sayısı, atopi, otoimmün hastalık arasındaki ilişki istatistiksel olarak anlamsızdı (p>0.05). Hastalık tipi ile atak sayısı, cinsiyet, nevus flammeus, aile hikayesi, atopi, otoimmün hastalık arasında anlamlı ilişki yoktu (p>0.05). Son atak süresi ile atak sayısı, cinsiyet, hastalık başlangıç yaşı, nevus flammeus, tırnak tutulumu, aile hikayesi, atopi, otoimmün hastalık arasında anlamlı ilişki gözlenmedi (p>0.05). Hastalık süresi ile hastalık başlangıç yaşı, cinsiyet, nevus flammeus, aile hikayesi, atopi, otoimmün hastalık arasında anlamlı ilişki saptanmadı (p>0.05).Sonuç olarak otoimmün hastalık oranının literatür verilerine uygun olarak yüksek saptanması AA etyopatogenezinde otoimmünitenin varlığını destekler bir bulgudur. Erken başlangıç yaşı, tırnak tutulumunun varlığı, nevus flammeus varlığı, ailede AA hikayesi varlığı ve uzun süreli hastalık öyküsünün kötü prognoz göstergesi olduğu bu araştırma ile bir kez daha ortaya kondu. Atopi ve otoimmün hastalık birlikteliğinin prognozla ilişkisi olmadığı saptandı.

Alopecia areata (AA) is a tissue-specific autoimmune disease affecting any hairy area of the body, such as hair and beard and it is characterized by non-scatricial hair loss with exacerbations and remissions. Although its etiopathogenesis is not known, autoimmune factors have been suggested. The major indicators for an autoimmune pathogenesis is the association of AA with some systemic autoimmune diseases and detection of autoantibodies.This study aims to determine the frequency of autoimmune diseases associated with AA in order to discuss the role of autoimmunity in the etiopathogenesis and to identify the clinical and demographic characteristics of AA.The records of 134 AA patients (age range 3-65 years) who were diagnosed in the dermatology and venereology outpatient clinic of Istanbul Medical Faculty between August 2011 and January 2012 were retrospectively analyzed for age, sex, clinical type of AA, area of involvement, disease severity, the number of attacks, the duration of illness, the duration of last attack, age of onset, the presence of nevus flammeus, nail involvement, the history of emotional and/or physical stress, the history of concomitant autoimmune disease, the family history for AA or other autoimmune diseases, family and individual history of atopy and routine laboratory findings which were performed for a possible association of systemic autoimmune disease. The percentage of hair loss in patients with involvement of the scalp was calculated according to Severity of Alopecia Tool (SALT) score. Chi-square (X2) and Fisher's exact test were used for statistical analysis. p<0.05 was considered statistically significant.Among 134 patients included in the study, 55 were women (41%) and 79 were males (59%), male to female ratio was 1.4:1 and the mean age was 24.92 ± 14.6 years; 82.8% were < 40 years of age. The age of onset varied from 2 to 63 years, the mean age 20.10 ± 14.5 years. The age of onset was < 20 years in 53% and < 40 years in 89.6% of patients. An attack occurred in 72.4% of patients. The frequency of two or more attacks were higher in men in comparison to women (p=0.015), in patients with disease duration of 1 year in comparison with disease duration <1 year (p<0.001) and in juvenile onset in comparison with adult onset (p=0.034). Alopecia areata, alopecia totalis (AT), alopecia universalis (AU) was found in 86.5%, in 3.7% and in 9.7% of the patients, respectively. 77.6% of patients had mild and 22.4% had severe involvement. The frequency of severe involvement was higher in women in comparison with men (p=0.029), in juvenile onset in comparison with adult onset (p=0.001), in patients with disease duration 1 year in comparison with disease duration <1 year (p<0.001), in patients with last attack duration 1 year in comparison with last attack duration <1 year (p <0.001). 14.9 % of patients had nevus flammeus. Severe involvement was statistically significant in those with nevus flammeus (p=0.017). 26.1% of patients had nail involvement. Signs of pitting were most commonly detected. The frequency of nail involvement was higher in severe involvement in comparison with mild involvement (p=0.015), in patients with disease duration 1 year in comparison with disease duration <1 year (p=0.021), in juvenile group in comparison with adult group (p<0.001), in AT/AU group in comparison with AA group (p=0.007). 27.6% of the patients had a family history. The frequency of family history was higher in juvenile group in comparison with adult group (p=0.034). A family history was present in 26% of those with mild involvement and in 33.3% with severe involvement. The history of atopy was found in 8.2% of patients and no statistically significant difference was found between atopy and the severity of the disease (p>0.05). An autoimmune disease was found in 16.4 of the patients , including in autoimmune thyroiditis 11.9%, vitiligo in 1.5% of the patients. The frequency of autoimmune disease was higher in women in comparison to men (p=0.034), in patients with a family history of autoimmune disease in comparison to the patients without a family history of autoimmune disease (p=0.011). No statistically significant difference between gender and nevus flammeus, gender and nail involvement, gender and family history, gender and atopy (p>0.05). There was no statistically significant difference between age at onset and gender, age at onset and nevus flammeus, age at onset and atopy, age at onset and autoimmune disease (p>0.05). No statistically significant difference was found between the disease severity and the number of attacks, the disease severity and atopy, the disease severity and autoimmune disease (p> 0.05). There was no statistically significant difference between the type of illness and the number of attacks, the type of illness and gender, the type of illness and nevus flammeus, the type of illness and family history, the type of illness and atopy, the type of illness and autoimmune disease (p>0.05). No statistically significant difference was found between the duration of last attack and the number of attacks, the duration of last attack and gender, the duration of last attack and age of onset, the duration of last attack and nevus flammeus, the duration of last attack and nail involvement, the duration of last attack and family history, the duration of last attack and atopy, the duration of last attack and autoimmune disease (p>0.05). There was no statistically significant difference between the duration of disease and age of onset, the duration of disease and gender, the duration of disease nevus flammeus, the duration of disease and family history, the duration of disease and atopy, the duration of disease and autoimmune disease (p>0.05).In conclusion, the findings in our study revealing a high proportion of AA patients having autoimmune disease, is consistent with the literature, thus supports the presence of autoimmunty in the etiopathogenesis of AA. This study once again showed that early age of onset, presence of nail involvement, presence of nevus flammeus, and family history of AA and long-term disease were predictive of poor prognosis. The association of atopy with autoimmune disease was not found to be related to the prognosis.
