Özet
VIII-ÖZET HBV infeksiyonları tüm dünyada olduğu gibi ülkemizde de önemli bir sağlık sorunudur. Özellikle taşıyıcı durumda bulunan olgular, gerek hepatit B'nin yayılmasında etkin olmakta, gerekse kişide siroz, hepatoselüler karsinoma ve kronik karaciğer hastalığının gelişmesinde primer rolü oynamaktadır. Bu olgularda kimi zaman rutin seroloji ve serokonversiyonlar yetersiz kalmakta ve virus DNA'sının gösterilmesi zorunlu olabilmektedir. Bu çalışmada 42 kronik B hepatit ön tanılı olgunun serumunda sıvı faz (solüsyon) moleküler hibridizasyon yöntemi ile aktif viral replikasyonun temel belirleyicisi olduğu benimsenen HBV DNA araştırılmıştır. 42 olgunun 19'unda (%45.2) HBV DNA saptanmıştır. Bu oran HBeAg olumlu 16 olguda 12 (%75), HBeAg olumsuz 26 olguda ise 7 (%26.9) dirve kaynaklarla uyumlu bulunmuştur. Sıvı faz (solüsyon) moleküler hibridizasyonun serumda HBV DNA araştırılmasında basit, kolay uygulanabilirliği yanında duyarlı ve kantitatif sonuç verebilen bir yöntem olduğu sonucuna varılmıştır. Tüm kronik hepatitli olgularda tanı ve prognozun belirlenmesi yanında sağaltım endikasyonunun konulması ve gerekirse antiviral sağaltımın izlenmesinde HBV DNA'nın araştırılması büyük önem taşımaktadır. 58

IX-SUMMARY Hepatitis B infections are important health problems in Turkey as well as in the world. Especially asymptomatic carriers have an effective role in the transmission of the hepatitis B while they play the primary role in the progression to cirrhosis, chronic liver disease and heptocellular carcinoma. In these cases routine serological tests and seroconversion are sometimes unsufficient and the detection of the viral DNA is absolutely necessary. In this study the sera of 42 chronic hepatitis B suspected cases were searched for HBV DNA which is accepted as the main marker of viral replication by the liquid phase (solution) hybridization method. HBV DNA was detected in 19 of 42 cases (45.2%). 12 out of 16 (75%) HBeAg positive and 7 out of 26 (26.9%) HBeAg negative patients were positive for HBV DNA. Our results correlated with the previous reports. It is concluded that the solution hybridization test is a simple, easily performable and a sensitive method which can give quantitative information. The detection of HBV DNA in all chronic hepatitis cases has great importance for the diagnosis and the progression of the disease as well as for the decision and the follow-up of the treatment. 59
