Özet
Amaç: Anestezi süreci preoperatif değerlendirme ve uygun anestezi planı geliştirme ile başlar. Preoperatif değerlendirmenin ilk adımı, hastanın hekimle tanışması ve hekime olan güvenini sağlamak, anestezi hakkında bilgilendirmektir. Preoperatif değerlendirme aşamasında ayrıntılı olarak hastanın hikayesi sorgulanmalı ve dikkatli bir fizik muayene yapılmalıdır. Bu çalışma ile preoperatif ve intraoperatif anestezi kayıtlarının düzenli tutulmasındaki eksiklerimizi retrospektif olarak değerlendirmeyi amaçladık.Gereç ve Yöntem: Çalışmada 2005- 2010 yılları arasında Hacettepe Üniversitesi Tıp Fakültesi ameliyathanesinde cerrahi operasyon geçiren bin hastanın dosyası retrospektif olarak tarandı. Anestezi ön değerlendirme formunda bulunan ad- soyad, yaş, cinsiyet, boy, ağırlık, ASA, mallampati, geçirilmiş anestezi öyküsü, alerji bilgileri, sigara- alkol- madde kullanımı, ailede anestezik problem öyküsü, fizik muayene bilgileri, laboratuar bilgileri, hastanın öyküsü gibi parametrelerin ve intraoperatif anestezi formunun düzenli tutulup tutulmadığının, intraoperatif komplikasyon kaydedilip kaydedilmediğini değerlendirdik.Bulgular: Hasta dosyalarının retrospektif değerlendirilmesi sonucunda; Preoperatif ve intraoperatif anestezi kayıtlarının bir çoğunun %50'nin üzerinde kaydedilmediğini saptadık. Yüzde 6,7 oranla en az kaydedilen parametre boy, en fazla kaydedilen parametrelerden biri %59,9 ile ad- soyad ve diğeri %59 ile laboratuar bilgileri oldu.Sonuç: Çalışmamız; Hastanemiz ameliyathanesinde preoperatif ve intraoperatif anestezi bilgilerinin eksik tutulduğunu ve kayıtlar tutulurken gerekli özenin gösterilmediğini ortaya koymuştur.

Goal:The anesthesia process begins with preoperative evaluation and developing a suitable anesthesia plan. The first step of preoperative evaluation is to introduce the patient to the doctor, establish the trust and to fill in the patient about the anesthaesia. During the preoperative phase the history of the patient should be thoroughly investigated and a careful physical examination should be carried out. With this study it was aimed to retrospectively evaluate the mistakes in keeping of preoperative and intraoperative anesthesia records.Material and Method: In the study, records of one thousand patients who had undergone surgical operation between 2005-2010 in Hacettepe University Hospital was scanned retrospectively. The evaluation was based on anesthesia pre-evaluation form parametars such as, name-surname, age, sex, height, weight, ASA, mallampati, previous anesthesia records, allergic information, smoking, alcohol or substance abuse, anesthesic problem history in family, physical examination information, laboratory information and history of the patient. It was also checked whether if the intraoperative anesthesia form was filled out and if any intraoperative complication was recorded.Findings: From of the retrospective evaluation of patient records, it was found out that preoperative and intraoperative anesthesia records were not kept for most of the arameters for over %50 of the patients. The lest recorded parameter was height with %6,7, the most recorded parameter was name-surname with %59,9 and laboratory information with %59. Result: The study laid out the fact that the preoperative and intraoperative anesthesia records are not kept with required attention to detail in the surgery of our hospital.
