Özet
Çalışma doğum sonrası 4. aydaki kadınların cinsel işlevlerinin ve etkileyen faktörlerin belirlenmesi amacıyla tanımlayıcı olarak yapılmıştır. Araştırmanın örneklemini Ankara'daki 7 Ana Çocuk Sağlığı ve Aile Planlaması Merkezine kayıtlı, doğum sonrası 4.ayda olan 100 kadın oluşturmuştur. Verilerin toplanmasında araştırmacı tarafından hazırlanan veri toplama formu, IFSF (Index of Female Sexual Function) ve EPDS (Edinburgh Postnatal Depression Scale) kullanılmıştır. Verilerin analizinde SPSS 15 istatistiksel analiz programı kullanılmıştır. Verilerin değerlendirilmesinde ortalama, tek yönlü varyans analizi, Tukey HSD testi, Kruskal Wallis testi, Fisher Exact test, Pearson Chi-Square testi, Levene testi, Student t testi ve Pearson Korrelation analizi kullanılmıştır. Çalışmada doğum sonrası 4. ayda kadınların %60'ının cinsel disfonksiyon için riskte oldukları saptanmıştır. Kadınların %66'sı doğumdan sonraki ilk cinsel ilişkilerinin yalnızca eşinin isteği ile gerçekleştiğini belirtmiştir. Kadınların %41'i doğumdan sonra cinsel ilişkilerinde ağrı yaşadığını belirtirken, %24.7'si doğumdan 4 ay sonra halen ağrı hissettiğini bildirmiştir. Kadınların doğum sonrası dönemde cinsel istek, cinsel ilişki sıklığı, cinsel ilişki memnuniyeti ve cinsel yaşam memnuniyetinin gebelik öncesine göre düşük olduğu belirlenmiştir. Çalışmamızda kadının eğitim düzeyinin, eşin eğitim düzeyinin eşin çalışma durumunun, üreme sistemine ait şikayeti olma durumunun, doğumdan sonraki ilk cinsel ilişkiyi isteme durumunun ve EPDS puanının cinsel fonksiyonlar üzerine belirgin etkisi olduğu belirlenmiştir. IFSF alt puanları incelendiğinde şu bulgular saptanmıştır: Doğum şekli ve üreme sistemi şikayetleri cinsel istek, klitoral duyarlılık ve cinsel ilişki doyumu üzerine; doğum sayısı cinsel istek, klitoral duyarlılık ve cinsel ilişkide rahatsızlık/ağrı üzerine; emzirme ve postpartum depresyon ise cinsel istek üzerine etkilidir. Bu sonuçlar doğrultusunda ebe ve hemşirelerin, doğum sonrası dönemdeki kadın ve eşini bu dönemde görülebilecek cinsel sorunlar, nedenleri ve çözüm yolları hakkında bilgilendirmeleri ve danışmanlık yapmaları önerilmiştir.

This research was carried out as a descriptive study in order to determine women?s sexual functions four months after delivery and factors effect these functions. The sample comprised of 100 women were chosen from women within their fourth postpartum month and registered to seven of Mother-Child Health and Family Planning Centers in Ankara. Data Collecting Form prepared by the researcher, IFSF (Index of Female Sexual Function) and EPDS (Edinburgh Postnatal Depression Scale) were used in collecting data. SPSS (Statistical Package for Social Sciences) 15.0 for Windows were used to analyzing of data. In data evaluation mean, oneway Anova test, Tukey HSD test, Kruskal Wallis test, Fisher?s Exact test, Pearson Chi-Square test, Levene test, Student t test and Pearson Correlation were used. It was determined that 60% of women within a 4-month postnatal period were at risk for sexual disfunction. 66% of women mentioned that resumptioned sexual intercourse after delivery only with their husbands? desire. 41% of women said that they had dyspareunia after delivery. 24.7% of women had dyspareunia at four mounths of postpartum. Sexual desire, frequency of sexual intercourse, satisfaction of sexual intercourse and satisfaction of sexual life in postpartum period was lower than before pregnancy. It was determined that status of education, education status of husband, employment status of husband, semptoms of reproductive system, desire for first inretcourse after delivery (only husband/women and husband) and EPDS scores have a significant effect on sexual functions. When the domains of IFSF were investigated, these results were founded: method of delivery and semptoms of reproductive system effect sexual desire, clitoral sensitivity and sexual intercourse satisfaction; number of delivery effects sexual desire, clitoral sensitivity and discomfort/pain at sexual penetration; breastfeeding and PPD (Postpartum Depression) effects sexual desire. According to the results of research it was suggested for the midwiferies/nurses to enlighten and guide the postpartum women and their partners on any possible problems and disorders to be experienced during this period, their causes and solutions.
