Özet

ÖZET Karakaya, MG., Spastisitede Kullanılan Antispastik Pozisyonda Uzun Süreli Germe Yöntemlerinin Hemodinamik Yanıtlara Etkileri, Hacettepe Üniversitesi Sağlık Bilimleri Enstitüsü Fizik Tedavi ve Rehabilitasyon Programı Doktora Tezi, Ankara, 2003. Bu çalışma, beyin yaralanması nedeniyle yoğun bakım servisinde yatan hastalara uygulanan iki farklı antispastik pozisyonda uzun süreli germe yönteminin (el ve Johnstone splinti ile), hemodinamik ve solunum cevaplan ile motor ünite aksiyon potansiyeli (MÜAP) üzerine etkisini belirlemek amacıyla planlandı. Çalışmaya beyin yaralanması olan 23 hasta (ortalama yaş: 50.96±15.99 yıl) alındı. Hastaların ortalama arterial basınç (mmHg), kalp hızı (atım/dk), solunum frekansı (soluk/dk) ve oksijen saturasyonu (%) değerleri takip edildi. Spastik kaslardan elde edilen MÜAP'lar (uV) EMG biofeedback cihazı ile değerlendirildi. M. Gastrocnemius ve M. Biceps brachii, el ve Johnstone splinti ile 20 dakika süresince gerildi. Tüm ölçümler başlangıç, 20 dakikalık germe ve 15 dakikalık toparlanma süresince 3'er dakikalık aralıklarla gerçekleştirildi. Bir kasa bir gün İçinde tek germe yöntemi uygulandı ve ölçümler her hasta için dört ardışık günde gerçekleştirildi. Kalp hızı ve solunum frekansının her iki kas için özellikle germelerin başlarında, ortalama arterial basıncın ise M. Gastrocnemius germeleri sırasında arttığı görüldü (p<0.05). Oksijen saturasyonunun M. Gastrocnemius'un el ile gerilmesinin başında gösterdiği kısa süreli artışın (p<0.05) dışında germelerden etkilenmediği bulundu (p>0.05). Splint ile germe MÜAP değerlerini azaltırken (p<0.05), el ile yapılan germede böyle bir etki görülmedi (p>0.05). Sonuçta, uygulanan germe tekniklerinin hemodinamik ve solunum yanıtlarına etkilerinin klinik açıdan dikkati çekecek boyutta olmadığı, Johnstone splintinin spastisite inhibisyonunda etkili olduğu ve bu nedenle yoğun bakım düzeyindeki beyin yaralanmalı hastalarda güvenle kullanılabileceği düşünüldü.

VI ABSTRACT Karakaya MG., Effects of Prolonged Stretching Methods at Antispastic Position on Hemodynamic Responses in Spasticity, Hacettepe University Health Sciences Institute, PhD Thesis in Physiotherapy and Rehabilitation, Ankara, 2003. This study was planned to investigate the effects of two different prolonged stretching methods (manually and by Johnstone splint) at antispastic position on hemodynamic and respiratory responses, and motor unit action potentials (MUAP)(uV) of patients with brain injuries in intensive care unit. Twenty-three patients (mean age: 50.96±15.99 years) with brain injuries were included in this study. Mean arterial pressure (MAP) (mmHg), heart rate (pulse/min), respiratory rate (breath/min) and oxygen saturation (%) values were monitorised. MUAPs of spastic muscles were evaluated by using an electromyographic (EMG) biofeedback device. M. Gastrocnemius and M. Biceps brachii were stretched manually and by Johnstone splints for a duration of 20 minutes. All measurements were performed at the baseline, during 20 minutes of stretching and 15 minutes of recovery periods by intervals of three minutes. Only one stretching method was performed on one muscle in a day and, measurements were completed in four consecutive days for each patient. Heart and respiratory rates were found increased especially at initial phases of stretchings for both muscles and MAP increased during M. Gastrocnemius stretching (p<0.05). Oxygen saturation was not influenced by the stretchings (p>0.05), except a short term increase at the initial phases of manual stretching of M. Gastrocnemius (p<0.05). Stretching by splint decreased MUAP values (p<0.05), while manual stretching did not produce such an effect (p>0.05). It was concluded that, the effects of stretching methods on hemodynamic and respiratory responses were not clinically obvious, Johnstone splints were effective in spasticity inhibiton, and so they could be used safely for the patients with brain injuries in intensive care units.
