Özet
Giriş: Nörogelişimsel bozukluklar, erken beyin gelişimindeki aksaklıklar sonucu ortaya çıkan, bilişsel, duyusal ve motor işlevlerde yetersizlikle seyreden ve çocukların % 2-3'ünü etkileyen heterojen bir grup hastalıktır. Entelektüel yetersizlik ve global gelişim geriliği bu grubun önemli bileşenleri olup, olguların %25-50'sinde genetik nedenler rol oynar. Sinir sistemi gelişiminde gen ekspresyonunun hassas düzenlemesi kritik olduğundan, özellikle kromatin düzenleyici genler ve splicing mekanizmasında görev alan bileşenlerdeki bozukluklar hastalık patogenezinde önemlidir. Splicing sürecini yöneten spliceosome kompleksinin temel bileşenleri olan snRNA genlerindeki varyantların, pre-mRNA işlenmesi bozarak nörogelişim için gerekli proteinlerin hatalı sentezine yol açtığı ve bunun da geniş bir fenotip spektrumunda nörogelişimsel bozukluklara neden olduğu gösterilmiştir. Özellikle RNU2-2, RNU4-2, RNU5B-1 ve RNU4ATAC genleri ile ilişki ortaya konmuş, RNU5A-1 ise aday gen olarak öne çıkmıştır. Standart genetik testlerde çoğu zaman değerlendirilemeyen bu snRNA genleri, tanı konulamayan hastalarda önemli bir tanısal boşluğu açıklayabilecek potansiyele sahiptir ve moleküler etiyolojinin aydınlatılması klinik açıdan giderek daha fazla önem kazanmaktadır. Yöntem: Bu çalışmaya, Ege Üniversitesi Tıp Fakültesi (EUTF) Çocuk Genetik Hastalıkları Bilim Dalı'nda izlenen ve standart genetik değerlendirmelere rağmen etiyolojisi aydınlatılamayan global gelişim geriliği ve/veya entelektüel yetersizlik tanılı 102 hasta dahil edildi. Olguların klinik, demografik ve genetik verileri hasta dosyalarından elde edildi. Hastaların rutin tanısal yaklaşımda uygulanmış olan ES verileri hedef genler açısından yeniden değerlendirildi. Mevcut genetik analizlerde (karyotip, kromozomal mikroarray, ekzom sekanslama) tanı konulamayan hastalarda, seçilen snRNA genleri (RNU2-2, RNU4-2, RNU5A-1, RNU5B-1 ve RNU4ATAC) hedef alınarak varyant analizi gerçekleştirildi. Saptanan varyantların patojeniteleri toplum sıklığı, literatür verileri ve segregasyon analizleri ile yorumlandı. Bulgular: Çalışma kapsamında hedeflenen snRNA genleri (RNU2-2, RNU4-2, RNU5A-1, RNU5B-1 ve RNU4ATAC) için PCR ve dizi analizi yöntemleri %100 kapsayıcılık ve yüksek okuma derinliğiyle standardize edildi. Yapılan taramalar sonucunda üç hastada (%2,9) klinik fenotiple uyumlu patojenik/olası patojenik varyantlar saptanarak kesin moleküler tanıya ulaşıldı. Olgu 36'da daha önce ClinVar veri tabanında patojenik olarak kaydedilen n.64_65insT varyantı heterozigot olarak saptandı ve yapılan aile segregasyonu ile de novo olduğu ortaya konuldu. Olgunun klinik bulguları ReNu sendromu ile uyumlu olarak değerlendirildi. Olgu 86 ve Olgu 92'de her ikisinin de RNU4ATAC geninde biallelik patojenik varyant taşıdığı belirlendi. Olgu 86'da daha önce ClinVar veri tabanında patojenik olarak kaydedilmiş olan RNU4ATAC geninde n.16G>A varyantı homozigot olarak saptandı. Yapılan segregasyon sonucunda anne ve babanın taşıyıcı olduğu görüldü. Aynı şekilde Olgu 92'de RNU4ATAC geninde n.51G>A ve n.111G>A varyantları birleşik heterozigot olarak saptandı. Olguda belirlenen her iki varyant ClinVar veri tabanında patojenik olarak belirtilmiştir. Yapılan aile segregasyonunda etkilenmiş kardeşinde de varyantların birleşik heterozigot olduğu ve annenin n.51G>A varyantını heterozigot olarak taşıdığı gösterildi. Ancak babadan kan örneği alınamadığı segregasyon analizi yapılamadı. Her iki hastanın da klinik bulguları RNU4atacopathy ile uyumlu olarak değerlendirildi. Sonuçlar: Bu çalışma, standart genetik testler ile kapsanmayan snRNA genlerinin (RNU4-2, RNU2-2, RNU5A-1, RNU5B-1, RNU4ATAC) nörogelişimsel bozuklukların etiyolojisinde önemli bir rolü olduğunu göstermiştir. RNU4-2 ve RNU4ATAC varyantlarının klinik etkileri kohortumuzda doğrulanmış ve genotip-fenotip ilişkisine katkıda bulunulmuştur. Standart moleküler analizlerle tanı konulamayan hasta grubunda nadir varyantlar saptanarak tanı oranı artırılmıştır. Elde edilen bulgular, RNU gen analizinin nörogelişimsel hastalıkların tanısal sürecine dahil edilmesi gerekliliğini vurgulamaktadır.

Introduction: Neurodevelopmental disorders are a heterogeneous group of diseases that affect 2–3% of children, resulting from disruptions in early brain development and manifesting with impairments in cognitive, sensory, and motor functions. Intellectual disability and global developmental delay are important components of this group, with genetic causes playing a role in 25–50% of cases. Since the precise regulation of gene expression is critical during nervous system development, defects particularly in chromatin-regulating genes and components involved in the splicing mechanism are important in disease pathogenesis. Variants in snRNA genes, which are key components of the spliceosome complex that governs the splicing process, have been shown to disrupt pre-mRNA processing, leading to the aberrant synthesis of proteins required for neurodevelopment, thereby causing neurodevelopmental disorders across a broad phenotypic spectrum. Correlations have been established especially with the RNU2-2, RNU4-2, RNU5B-1, and RNU4ATAC genes, while RNU5A-1 has emerged as a candidate gene. These snRNA genes, which are often not evaluated in standard genetic tests, have the potential to explain a significant diagnostic gap in undiagnosed patients, and elucidating their molecular etiology is becoming increasingly important clinically. Method: This study included 102 patients with global developmental delay and/or intellectual disability who were followed up at Ege University Faculty of Medicine (EUTF), Department of Pediatric Genetics, and whose etiology could not be elucidated despite standard genetic evaluations. Clinical, demographic, and genetic data of the cases were obtained from patient files. ES (exome sequencing) data previously applied in the routine diagnostic approach of the patients were re-evaluated for the target genes. In patients who could not be diagnosed by current genetic analyses (karyotype, chromosomal microarray, exome sequencing), variant analysis was performed targeting selected snRNA genes (RNU2-2, RNU4-2, RNU5A-1, RNU5B-1, and RNU4ATAC). The pathogenicity of the identified variants was interpreted based on population frequency, literature data, and segregation analyses. Results: Within the scope of the study, PCR and sequencing analysis methods for the targeted snRNA genes (RNU2-2, RNU4-2, RNU5A-1, RNU5B-1, and RNU4ATAC) were standardized with 100% coverage and high read depth. As a result of the screenings, pathogenic/likely pathogenic variants compatible with the clinical phenotype were detected in three patients (2.9%), achieving a definitive molecular diagnosis. In Case 36, the n.64_65insT variant, previously recorded as pathogenic in the ClinVar database, was detected as heterozygous, and family segregation revealed it to be de novo. The clinical findings of the case were evaluated as compatible with ReNu syndrome. In Case 86 and Case 92, both were determined to carry biallelic pathogenic variants in the RNU4ATAC gene. In Case 86, the n.16G>A variant in the RNU4ATAC gene, previously recorded as pathogenic in the ClinVar database, was detected as homozygous. Segregation analysis showed that the mother and father were carriers. Similarly, in Case 92, the n.51G>A and n.111G>A variants in the RNU4ATAC gene were detected as compound heterozygous. Both variants identified in this case are specified as pathogenic in the ClinVar database. Family segregation demonstrated that the affected sibling was also compound heterozygous for these variants and the mother carried the n.51G>A variant as heterozygous. However, segregation analysis could not be completed for the father as a blood sample could not be obtained. The clinical findings of both patients were evaluated as compatible with RNU4atacopathy. Conclusions: This study demonstrated that snRNA genes (RNU4-2, RNU2-2, RNU5A-1, RNU5B-1, RNU4ATAC), which are not covered by standard genetic tests, play an important role in the etiology of neurodevelopmental disorders. The clinical impacts of RNU4-2 and RNU4ATAC variants were confirmed in our cohort, contributing to the genotype-phenotype relationship. By detecting rare variants in the patient group that could not be diagnosed with standard molecular analyses, the diagnostic rate was increased. The findings highlight the necessity of integrating RNU gene analysis into the diagnostic process of neurodevelopmental diseases.
