Özet

Amaç: Kübital tünel sendromu (KüTS) tanılı hastalarda egzersiz tedavisine ek olarak uygulanan kinezyolojik bantlamanın ağrı, fonksiyonellik, yaşam kalitesi ve ultasonografik ulnar sinir morfolojisi üzerine etkilerini araştırmaktır. Gereç ve Yöntem: Çalışmamız tek merkezli, prospektif, randomize kontrollü olarak planlandı. Çalışmaya elektromiyografi ile KüTS tanısı konmuş, dahil edilme ve dışlama kriterlerine uygun 45 hasta dahil edildi. Hastalar üç gruba randomize edildi. Birinci gruba ulnar sinir mobilizasyon egzersizleri verildi, ikinci gruba egzersiz ve kinezyolojik bantlama(KB), üçüncü gruba ise egzersiz ve sham bantlama uygulandı. Hastalar tedavi öncesi, tedavi sonrası 1. ayda ve tedavi sonrası 3. ayda değerlendirildi. Ağrıyı değerlendirmek için Numerik Rating Skala (NRS), nöropatik ağrı için NRSna ve Douleur Neuropathique 4 (DN4) ölçeği, dizabilite için Hızlı Kol, Omuz ve El Özürlülük Ölçeği (Q-DASH), yaşam kalitesi için Short Form-12 (SF-12) Yaşam Kalitesi Ölçeği, klinik düzelmenin takibi için Global Değişim Değerlendirme (GRC) Ölçeği, kavrama gücü ölçümü için el dinamometresi ve ulnar sinir kesitsel alanı (KA) ve ekojenitesini değerlendirmek için ultrasonografi (US) kullanılmıştır. Bulgular: Uygulanan tedaviler sonrası 1. ay ve 3. ay kontrollerinde her üç grupta da incelenen bütün parametrelerde anlamlı iyileşmeler gözlemlendi (p < 0.001). NRS, NRSna, DN4, SF-12 mental bileşeni, GRC skorlarındaki iyileşme ve ultrasonografi ile ölçülen ulnar sinir çapındaki azalma KB+egzersiz grubunda diğer gruplara kıyasla anlamlı düzeyde daha yüksek bulundu (p < 0.001). Özellikle KB uygulanan grupta 1. ay kontrollerinde NRS ve NRSna değerlerinde gözlemlenen başlangıca göre azalma oranı, diğer gruplara kıyasla anlamlı derecede daha fazlaydı (p < 0.001 ve p = 0.008). Q-DASH, SF-12 fiziksel bileşeni, ulnar sinir ekojenitesi ve kavrama gücündeki iyileşme KB+egzersiz grubunda daha fazla bulunmuş olsa da gruplar arasında istatistiksel olarak anlamlı bir fark saptanmadı. Korelasyon analizinde ise ulnar KA, NRS, NRSna ve DN-4 ekojenite değişiklikleri ile, NRSna DN-4 ile pozitif yönde korele iken NRS skorları SF-12 mental bileşeni ile negatif yönde korele saptanmıştır. Sonuç: Kübital tünel sendromlu hastaların tedavisinde egzersiz tedavisine eklenen KB ile ağrı, parestezik şikayetler ve ulnar sinir çapında daha fazla azalma; fonksiyonellikte ve hasta tarafından hissedilen iyilik halinde daha fazla artış sağlanabilir. KB, KüTS tedavisinde kullanılabilecek etkili ve tamamlayıcı bir seçenek olarak kullanılabilir.

Objective: To investigate the effects of kinesiology taping added to exercise therapy on pain, functionality, quality of life, and ultrasonographic ulnar nerve morphology in patients diagnosed with cubital tunnel syndrome (CuTS). Materials and Methods: This single-center, prospective, randomized controlled study included 45 patients diagnosed with CuTS by electromyography who met the inclusion and exclusion criteria. Patients were randomized into three groups. The first group received ulnar nerve mobilization exercises, the second group received exercise combined with kinesiology taping (KT), and the third group received exercise combined with sham taping. Evaluations were performed before treatment, at the 1st month after treatment, and at the 3rd month after treatment. The Numeric Rating Scale (NRS) was used to assess pain; NRS for neuropathic pain (NRSnp) and the Douleur Neuropathique 4 (DN4) questionnaire were used to evaluate neuropathic pain; the Quick Disabilities of the Arm, Shoulder, and Hand Questionnaire (Q-DASH) was used to assess disability; the Short Form-12 (SF-12) Quality of Life Scale was used to evaluate quality of life; the Global Rating of Change (GRC) Scale was used to monitor clinical improvement; hand dynamometry was used to measure grip strength; and ultrasonography (US) was used to evaluate ulnar nerve cross-sectional area (CSA) and echogenicity. Results: Significant improvements were observed in all evaluated parameters in all three groups at the 1st- and 3rd-month follow-ups after treatment (p < 0.001). Improvements in NRS, NRSnp, DN4, the mental component of SF-12, GRC scores, and the reduction in ulnar nerve diameter measured by ultrasonography were significantly greater in the KT+exercise group compared to the other groups (p < 0.001). In particular, the reduction rates in NRS and NRSnp scores at the 1st-month follow-up were significantly greater in the KT group compared to the other groups (p < 0.001 and p = 0.008, respectively). Although greater improvements were observed in Q-DASH, the physical component of SF-12, ulnar nerve echogenicity, and grip strength in the KT+exercise group, no statistically significant differences were found between the groups. Correlation analysis revealed that changes in ulnar nerve CSA were positively correlated with NRS, NRSnp, DN4, and echogenicity changes; NRSnp was positively correlated with DN4; whereas NRS scores were negatively correlated with the mental component of SF-12. Conclusion: Adding kinesiology taping to exercise therapy in patients with cubital tunnel syndrome may provide greater reductions in pain, paresthetic symptoms, and ulnar nerve diameter, as well as greater improvements in functionality and patient- perceived well-being. Kinesiology taping may be considered an effective and complementary treatment option in the management of CuTS.
