Özet

Bu in vitro çalışmanın amacı; iki marjininde derin marjin elevasyonu (DME) gereksinimi olan, endodontik tedavi görmüş alt birinci molar dişlerde; endokron, overley, postlu ve postsuz tam kron restorasyonların biyomekanik davranışlarını üç boyutlu sonlu elemanlar analizi (SEA) ile değerlendirmektir. Çalışmada, çekilmiş sağlam bir alt birinci molar diş temel alınarak üç boyutlu bir model oluşturulmuştur. Model üzerinde kanal tedavisi simüle edilmiş; distal ve lingual duvarların subgingival seviyede kaybedildiği aşırı madde kaybı senaryosu kurgulanmıştır. Overlay gruplarında pulpa odası farklı kompozit rezin sistemleri ile build-up yapılarak restore edilmiştir. Çalışma kapsamında oluşturulan gruplar şu şekildedir: G1: DME gereksinimi olmayan Endokron (Kontrol) G2: DME uygulanan Endokron G3: DME + Geleneksel Kompozit (GK) build-up + Overlay G4: DME + EverX build-up + Overlay G5: DME + Ribbond post + GK build-up + Overlay G6: DME + Kavite tabanına Ribbond şerit + GK build-up + Overlay G7: DME + GK Core + Tam Kron G8: DME + Ribbond Post + GK Core + Tam Kron İndirekt restorasyonların tamamı lityum disilikat blok olarak modellenmiştir. Tüm modellere 300 N büyüklüğünde, dişin uzun aksına dikey ve 30° açıyla oblik olacak şekilde kuvvet uygulanmıştır. Diş dokusu, siman tabakası, kor materyali ve DME bölgesinde oluşan Von Mises ve maksimum asal gerilim değerleri analiz edilmiştir. Analiz sonuçlarına göre, oblik yükleme altında tüm modellerde dikey yüklemeye oranla daha yüksek stres değerleri saptanmıştır. Dikey yükleme altında diş dokusunda en düşük stres G2 (Endokron) grubunda, oblik yükleme altında ise G3 grubunda gözlenmiştir. Siman tabakasında ve DME'nin diş dokularına bağlandığı bölgelerde, oblik yükleme senaryosunda G3 grubu, G2'den daha az stres birikimi sergilemiştir. DME bölgesinde en düşük gerilim değerleri tam kron restorasyonlarda (G7 ve G8) kaydedilirken; G4 grubunda, özellikle oblik yükleme altında daha yüksek stres birikimi saptanmıştır. Tam kron restorasyonların diş dokularında ve DME-diş bağlantı bölgelerinde, G2 ve G3 gruplarına kıyasla daha az stres oluştuğu gözlenmiştir. Fiberle güçlendirilmiş kompozit rezin içeren overlay restorasyonlar (G4, G5 ve G6); diş dokuları, siman, DME ve kor materyallerinde diğer gruplara oranla daha yüksek stres değerleri göstermiştir. Derin marjin elevasyonu (DME) uygulamasının, restorasyonların mekanik özellikleri üzerinde olumsuz bir etkisi bulunmadığı saptanmıştır. Kalan diş dokusunun dayanımı ve restorasyonun retansiyonu açısından pulpa odasının geleneksel kompozit rezin ile restore edilmesi, çalışmada değerlendirilen diğer tasarımlara (fiber takviyeli sistemler vb.) kıyasla biyomekanik açıdan daha avantajlı bulunmuştur.

The aim of this in vitro study was to evaluate the biomechanical behavior of endocron, overlay, and full crown (with and without post) restorations in endodontically treated mandibular first molars requiring deep margin elevation (DME) on two margins using three-dimensional finite element analysis (FEA). A 3D model was generated based on a sound extracted mandibular first molar. Root canal treatment was simulated, and an extensive tooth loss scenario was created by modeling the loss of distal and lingual walls at the subgingival level. In the overlay groups, the pulp chamber was restored using different composite resin systems for build-up. The experimental groups were categorized as follows: • G1: Endocrown without DME requirement (Control) • G2: Endocrown with DME • G3: DME + Conventional Composite (CC) build-up + Overlay • G4: DME + EverX build-up + Overlay • G5: DME + Ribbond post + CC build-up + Overlay • G6: DME + Ribbond fiber at the cavity floor + CC build-up + Overlay • G7: DME + CC Core + Full Crown • G8: DME + Ribbond Post + CC Core + Full Crown All indirect restorations were modeled as lithium disilicate blocks. A 300 N load was applied to all models vertically and at a 30° oblique angle to the long axis of the tooth. Von Mises and maximum principal stress values were analyzed in the tooth structure, cement layer, core material, and the DME region. Oblique loading resulted in higher stress values across all models compared to vertical loading. Under vertical loading, the lowest stress in the tooth structure was observed in G2 (Endocrown), while G3 showed the lowest stress under oblique loading. In the cement layer and the regions where DME connects to tooth tissues, G3 exhibited less stress accumulation than G2 under oblique loading. While the lowest stress values in the DME region were recorded in full crown restorations (G7 and G8), G4 showed higher stress accumulation, particularly under oblique loading. Full crown restorations exhibited less stress in tooth tissues and DME-tooth interface regions compared to G2 and G3. Overlay restorations containing fiber-reinforced composite resins (G4, G5, and G6) demonstrated higher stress values in tooth tissues, cement, DME, and core materials compared to the other groups. The application of deep margin elevation (DME) was found to have no negative effect on the mechanical properties of the restorations. Regarding the strength of the remaining tooth structure and restoration retention, restoring the pulp chamber with conventional composite resin was found to be biomechanically more advantageous than the other restoration designs (e.g., fiber-reinforced systems) evaluated in this study.
