Özet

Amaç: Çalışmamız, unilateral ya da bilateral/triplejik spastik serebral palsi tanılı hastalarda üst ekstremiteye yönelik robot yardımlı rehabilitasyonun üst ekstremite fonksiyonlarına, el becerilerine, spastisite düzeyine ve yaşam kalitesine olan etkisini araştırmak ve robot yardımlı üst ekstremite rehabilitasyonunun, konvansiyonel rehabilitasyon yöntemlerine göre üstünlüğü olup olmadığını incelemek amacıyla yapılmıştır. Gereç ve Yöntem: Prospektif, kontrollü gözlemsel çalışma olarak planlanan çalışmamız Temmuz 2021-Haziran 2023 tarihleri arasında Ankara Bilkent Şehir Hastanesi Fizik Tedavi ve Rehabilitasyon Hastanesi pediatrik rehabilitasyon servisinde tedavi gören, 5-15 yaş aralığında, unilateral ya da bilateral/triplejik spastik serebral palsi tanılı 32 hasta ile gerçekleştirildi. 16 hasta çalışma grubuna, 16 hasta kontrol grubuna dahil edildi. Çalışma grubuna konvansiyonel rehabilitasyon programına ek olarak Armeo Spring pediatrik ekzoskeletal robotik sistemi ile robot yardımlı üst ekstremite rehabilitasyon programı, kontrol grubuna sadece konvansiyonel rehabilitasyon programı uygulandı. Konvansiyonel rehabilitasyon programı haftada 5 gün ve günde 60 dakika, robotik rehabilitasyon programı haftada 3 gün ve günde 30 dakika sürecek şekilde uygulandı. Tedaviler toplam 4 hafta sürdü. Hastaların üst ekstremite fonksiyonlarını değerlendirmek için Üst Ekstremite Beceri Kalitesi Testi (QUEST), el becerilerini değerlendirilmek için ABILHAND Kids- El Beceri Ölçeği ve Kutu ve Blok Testi, spastisite düzeyini değerlendirmek için Modifiye Ashworth Skalası (MAS), yaşam kalitesini değerlendirmek için KINDL Yaşam Kalitesi Ölçeği kullanıldı. Değerlendirmeler her iki grupta da tedavi başlangıcında, tedavi bitiminde ve tedavi sonrası 2. ayda yapıldı. Bulgular: Robotik tedavi grubundaki hastaların tedavi öncesi, tedavi bitimi ve tedavi sonrası ikinci ay QUEST, MAS-dirsek fleksörleri, ABILHAND-Kids, Kutu ve Blok Testi ve KINDL skorları arasında anlamlı fark saptandı (p<0,05). Ancak MAS-omuz adduktörleri, MAS-el bileği fleksörleri ve MAS-parmak fleksörleri skorları arasında anlamlı bir fark bulunmadı (p>0,05). Kontrol grubundaki hastaların tedavi öncesi, tedavi bitimi ve tedavi sonrası ikinci ay QUEST (QUEST-D hariç), MAS-dirsek fleksörleri, Kutu ve Blok Testi ve KINDL skorları arasında anlamlı fark bulunurken (p<0,05), QUEST-D (koruyucu ekstansiyon), ABILHAND-Kids, MAS-omuz adduktörleri, MAS-el bileği fleksörleri ve MAS-parmak fleksörleri skorları arasında anlamlı bir fark saptanmadı (p>0,05). Çalışma grubunda, kontrol grubuna kıyasla tedavi sonrası MAS-dirsek fleksörleri, ABILHAND-Kids ve Kutu ve Blok Testi değişimleri anlamlı düzeyde fazla idi (p<0,05). Yani robotik tedavi grubu bu parametreler açısından üstün bulundu. Ancak bu fark tedavi sonrası 2. ayda devam etmemekteydi (p>0,05). Sonuç: Çalışmamızda tedavi sonunda hem konvansiyonel rehabilitasyon alan grupta hem de konvansiyonel rehabilitasyon ile birlikte robotik tedavi alan grupta üst ekstremite fonksiyonlarında, el becerilerinde, yaşam kalitesinde iyileşme ve dirsek fleksörlerindeki spastisitede azalma saptandı ve bu iyileşme halinin tedavi sonrası 2. ayda da devam ettiği görüldü. Ayrıca, tedavi sonunda robotik rehabilitasyon alan grupta el becerilerindeki iyileşmenin ve dirsek fleksörlerindeki spastisitede azalmanın konvansiyonel rehabilitasyon alan gruba göre anlamlı olarak daha fazla olduğu saptandı. Sonuçlarımız konvansiyonel fizyoterapiye eklenen üst ekstremiteye yönelik robotik tedavinin faydalı olduğunu göstermektedir.

Objective: Our study aims to investigate the effect of robot-assisted rehabilitation for the upper extremity on upper limb functions, manual dexterity, spasticity level and quality of life in patients diagnosed with unilateral or bilateral/triplegic spastic cerebral palsy and to examine whether robot-assisted upper extremity rehabilitation is superior to conventional rehabilitation methods. Materials and Methods: Our study, planned as a prospective, controlled observational study, was conducted between July 2021 and June 2023 with 32 patients diagnosed with unilateral or bilateral/triplegic spastic cerebral palsy, aged 5-15, who were treated at the pediatric rehabilitation clinic of Ankara Bilkent City Hospital Physical Therapy and Rehabilitation Hospital. 16 each patients were included in the study and control groups. The study group received a robot-assisted upper extremity rehabilitation program with a Armeo Spring pediatric robot in addition to conventional physiotherapy, and the control group received only conventional physiotherapy program. The conventional physiotherapy program was applied 5 days a week and 60 minutes a day, and the robotic rehabilitation program was applied 3 days a week and 30 minutes a day. The treatments lasted a total of 4 weeks. Quality of Upper Extremity Skills Test (QUEST) was used to evaluate the upper limb functions of the patients, ABILHAND Kids- Manual Ability Measure and Box and Block Test were used to evaluate manual dexterity, Modified Ashworth Scale (MAS) was used to evaluate spasticity level, KINDL Quality of Life Scale was used to evaluate quality of life. Evaluations were performed in both groups at the beginning of treatment, at the end of treatment and two months after treatment. Results: A significant difference was found between the QUEST, MAS-elbow flexors, ABILHAND-Kids, Box and Block Test and KINDL scores of the patients in the robotic group before treatment, at the end of treatment and two months after treatment (p<0,05). However, no significant difference was found between MAS-shoulder adductors, MAS-wrist flexors and MAS-finger flexors scores at these three evaluation (p>0,05). In the control group, a significant difference was found between the QUEST (except QUEST-D), MAS-elbow flexors, Box and Block Test and KINDL scores of before the treatment, at the end of the treatment, and two months after the treatment (p<0.05). However, no significant difference was found between QUEST-D (protective extension), ABILHAND-Kids, MAS-shoulder adductors, MAS-wrist flexors and MAS-finger flexors scores in the control group. In the study group, changes in MAS-elbow flexors, ABILHAND-Kids and Box and Block Test were significantly higher at the end of the treatment compared to the control group (p<0.05). In other words, the robotic treatment group was found to be superior in terms of these parameters. However, this difference did not persist two months after treatment (p>0.05). Conclusion: In our study, improvements in upper limb functions, manual dexterity, quality of life and decrease in elbow flexor spasticity were found in both the conventional rehabilitation group and the robotic rehabilitation group at the end of treatment, and it was observed that this improvement continued two months after the treatment. In addition, at the end of the treatment, it was found that the improvement in manual dexterity and the decrease in elbow flexor spasticity in the robotic rehabilitation group were significantly higher than in the conventional rehabilitation group. Our results show that robotic treatment for the upper extremity added to conventional physiotherapy is beneficial.
